[ROM-PAC_4.2.2] Sd card unexpectedly removed - Xperia Z Q&A, Help & Troubleshooting

I´m using the latest PAC-Rom and got a serious problem with my sdCard. When reading or writing to my
sdCard (SanDisk Ultra 32GB micro sd HC I) I got the message "Sd card unexpected removed". This error
occurs randomly for no clear reason. The dmesg snipped is attached. Any idea what could be wrong?
No problems with this card on stock-fw, using SGS2 and in two different PCs. Formating the card don´t
solve the problem.

Gallery with SD unmount or not inserted issue

G'morning all,
Can someone pls let me know how to fix the Gallery SD unmount or not inserted error?
Or anyone has the same problem as I do.
Basically, fresh install and run CM7 on the internal memory (eMMC), when I tap on the Gallery, it always complains "SD unmount or not inserted ....", and even if I insert an external uSD card (formatted with FAT32), it still complaining.
Am I missing something?
TIA
No one has this kind of problem? for real?

LG Optimus Black Blank SD Card

May I know anybody face "Blank SD Card or Unsupported Filesystem" problems with LG Optimus Black or not?
I'm using Kingmax 16GB class4, and I already faced 3 times of this problem, I asked for exchange of the sd card everytime when the sd card blank, so this is the 3rd sd card that I exchange for warranty from the shop, but after one week the same problem occurs again.
Everytime when this problems occur, I can't either format the sd card with the phone or computer, even I use card reader plug in to pc but no response form the computer (not pop up any message and the card reader drive letter not show out also), and I even try to format the sd card with camera but it gives me the message 'memory card error'.
I downloaded hp tool, sd formater but no luck to format the sd card also.
Sorry for my bad english, because I'm new in android, and I did google then result with a lot of such problems also, so hope that any expert will come with a solution, before I'm going to buy a new others brand sd card, just wan to clarify that is it the SD card defect problems or because the is the problems form android?
ROM using is NOVA v3.1.

Nexus One Thinks All SD Cards Are Blank

I'm having a weird problem with a Nexus One. It had a 2GB microSD card in it that seemed to go corrupt (all the file names went crazy) so I reformatted it and it happened again within minutes. So today I bought a new 8GB microSD card and and the Nexus now gives the error of "Blank SD Card - SD card blank or has unsupported filesystem." It is unable to format the card, but it doesn't give an error. I tried formatting the card to FAT32 on my computer, still the phone thinks it is blank/unsupported.
I'm hoping the SD slot isn't fried... any thoughts?
You could try reformating the SD card in a computer using SDformater with erase on.
I have the same problem for a few weeks now. My phone can't seem to read the sd card at all. Thought it was the sd card so I bought a new one, but problem remains. I know both cards work (have tested them in another phone and the computer reads them). I've tried formatting the card in the phone and reformatting with erase on using SDFormatter in my computer but my phone refuses to read it. Think it might be my sd slot as well...
Currently running CM 7.1.0. Haven't tried a factory reset or flashing another ROM (how would one go about doing that with no sd card?) as I'm not too keen to lose that data if I can help it.
Did you manage to fix it?
I was not able to fix it and after many hours of troubleshooting, so I gave up. Good luck to you, and please reply if you find a solution.

While SD card scan, it says "SD Card is Blank" and "It's safe to remove SD Card" ?

While SD card scan, it says "SD Card is Blank" and "It's safe to remove SD Card" ?
My phone was stuck when I unlocked it to use it a few minutes ago. I pressed the power button for a few seconds to reboot the phone. While rebooting, the sd card scan resulted in two errors with the messages "SD Card Blank" and "It's safe to remove SD card."
I researched this issue online, and most people who had this issue had their cards corrupted or they stopped working, and did not show any data when plugged into a laptop using an adaptor. I used the adaptor that came with my Samsung Class 10 32 GB card, and plugged it in my laptop - all the data is on the card, and I am able to read data from the card, although it is unable to copy some files. So, it seems the card itself is fine, (maybe).
Has anyone else experienced this issue, and how did you solve it? I am on Crickelo v8 ROM, speedmod k3-7 kernel.
EDIT:
While copying data from the sd card to my laptop, it seems the corruption got worse, and now the card is not accessible. I tried formatting the card in the phone, but it's not working. Can someone suggest a good formatting app?
Update:
While I was trying to copy files from my sd card for my computer, the sd card got corrupted. I ended up formatting my sd card using the recommended software (https://www.sdcard.org/downloads/formatter_3/) for my Samsung 32 GB Class 10 high speed card.
