Long OP short,
This issue is annoying, at least for those of us who're facing it.
Frequency of occurring:
-occasionally, 1~2 days device on time ( in my case, some reported to have it more frequent )
Possible Fix:
-Disables locations ( works for some, but not me, still getting it in about 1~2days device on time )
-Uninstalling Google Search Update ( works for some, somewhat true in my case, but still pops up a couple more days than usual )

Finally, someone actually made a thread about this.
I've having this issue too, batch #310k, getting it almost every 1 ~ 24 hrs range, it kind of random, turning off Location seems to help a lil'. WiFi, Data OFF, I rarely use data/wifi, mostly on 2G network for phone calls and sms.
The nexus 5 is the biggest problematic phone I've ever had

I had the exact same problem and did an RMA. Google asked me to a factory reset, which did nothing of course, and then emailed me a shipping label for the return. I got my new phone a week later. No problems with this one.

This is a hardware issue, I had the problem from the second day that I got the phone. If your sim tray is sticking out, chances are that the pins are not lined up against your sim card.
After it got very annoying, I called Google, they confirmed that this was a hardware issue, before I called Google though I tried it with a new sim card and also went fully stock. so after that Google agreed to do the RMA.....
With the replacement phone the sim tray aligns perfectly flush when inserted, it has been now a bit over 10 days since I got the replacement no pop up messages so far.
I don't have that issue luckily, but are there any information about which ones have it, like serial number or building line.

It's hard to say, I'm using the 310k batch, most users who reported are on 310k batch, but some who have gotten their replacement 311k batch and a very few 312k batch are reporting they're still having the issue, like 90min after they first boot their phone with a simcard.
Hopefully it's not a hardware defect, and rather just some software issue with the cellular tower triggering some event on the OS to show that message.
Add me to the list, this just started happening to me.
At this point it happens anytime randomly about once a day.
SIM Card Added Problems too
Me as well. Have a launch day phone (Rev11) 32G Black. Started with the error about two weeks ago. I'm using Straight Talk (ATT). Tried re-seating the SIM tray, blowing out the socket, factory reset, cleaning the SIM card, you name it. I've had it happen on stock 4.4 as well as CM11.
I called Google on Monday after reading the lengthy thread on G+ and after 5 min of troubleshooting, they decided to warranty the phone. The rep I talked with had never heard of the problem. Weird thing is that my wife's N5 (ordered at the same time) doesn't do it at all. She is using T-mobile. Maybe the ATT SIM?
I only hope my replacement is as good as my original. Aside from the slightly loose buttons, my screen was perfect, with no dead pixels or light bleed.
I'll report back when I get it if the new H/W fixes the issue

I found out that i got different simcard`s around , they one that`s making the "new sim card added" problem has a sim chip that is a bit deeper in a circle on the chip . Got a new chip now and no problems anymore so far :good:

I used to have this problem with a microsim cuted on my past phone (nexus 4) ... at least 1 time in a day i got the message that i have to restart my phone because the new addition of the sim card ... lol ... i didnt bother me to much initially ... i always suspect from "the cuted by myself " simcard coming from my old samsung galaxy nexus , tired of the situation i went to my local carrier and i ask for a replacemtent sim (like 4 dollars), they didn´t ask me too much why i want to change my sim card ... after that , never have that issue again ... i think that the old batches of microsim cards have issues ,

I remember having this problem with my S3 and galaxy nexus. I assumed it was the rom or my sim card. So far (knocked on wood), it hasn't happen to me with Nexus 5.

I have never had problems about "sim card inserted , please reboot message " after changing my cutted normal sim card , i start with them , after i cut my full size SIM from my GNEX to put in on my new nexus 4 , after two days i start receiving those messages , very after i receive broadcast messages from my carrier (coincidence ? i don´t know) , before start changing roms or kernels (at that time i used to have stock with franco kernel) i went with my local carrier , change the sim , and problem solved , 6 months without problems , next give my nexus 4 to my wife , and buy a nexus 5 put my old sim card , and start using the phone right away without any problems ... i bought my phone at december 28 2013 batch 311K... i have never had (knock wood lol) problems at all. very happy client
Is everybody stopped receiving this error? Maybe somebody has the solution? I am tired of those random sim card added errors
This error appearance is very irregular: sometimes it occurs few times a week, sometimes only weekly.
Is there anybody who is receiving this error with greater then 310k batch?
P.S. I replaced my sim card to brand new, but this did not solve it.
P.S.2. Sim toolkit does not work after this error.

Pretty long read, but added cliffs at the bottom.
My car has been broken into twice in the past month and a half. The first time they stole my GPS, radar detector (escort passport 9500i), and phone charger and one of those car outlet power strips. The second time was just a couple days ago and they stole my beats headphones, a bunch of change and a baton in my door.
They funny thing is there was no sign of forced entry, no pry marks, broken windows or doosr, or anything. I also have my spare set of keys with me, too, so no one could have gotten them. Either two scenarios could have arised from this.
1. I accidentally left the car doors unlocked( I have a key remote) although I'm 98% sure I locked it because I honestly have OCD about locking my car door.
or
2. Someone got into my house, took my keys(because I leave them on a radiator right by the front door) unlocked my car door, and stole the sh*t went back into my house and put the keys back.
My girlfriend left the house the night the first incident happened and forgot to lock the front door, so scenario 2 is likely.
The second incident, the front door was locked, but the back door was unlocked. So it would be pretty ballsy for a person to go through my back door, and do the same thing in scenario 2.
This really pisses me off and I want to find the person responsible so bad. I had a plan to try to track the person down, if they try to do it again. It doesn't make sense how or why this occurred, but the person must be extremely clever.
I have an old Droid Incredible that I had planned to use to find the perpetrator. I was planning on using the app Lookout and turning on the GPS and leaving it in my car every night until it got stolen again, then track down the coordinates of my phone. The only issue with that was you need working data to use the gps and the only way to do that would be to activate the line on my phone.
I currently have a Rezound and there was no way I was going to take a chance of having that stolen and never seeing it, that's why I chose the Incredible. Only problem is I don't wanna keep activating my Incredible every night and leaving it in my car and then deactivating it in the moring, because that would be very cumbersome.
Would there be any other way to be able to track this person using my Incredible? Also any other input or advice about this situation would be very appreciated: it's a mystery and it's really getting me down and worried about how this is happening.
Also, both times occurred over night, not during the day. Also, I do have a security system on my car so if anyone tried using a master key or lock pick, it would go off.

Okay buy some romen noodles a thermos forbkeeping your hot water. A bat a chair blanket. Hide and camp out your house amd leave the doors unlock and the most important of all have patients..
If that dont work you can set up a camera using uour droid. I seen some apps that can stream to your pc like justin tv

That could work, I can just use my house wifi for that. Also, I have all those things minus the ramen.... Looks like I'm stocking up.
Those pay as you go type of plans seem perfect for this situation. There are some that you can use your own phone and get a plan like 2 GB of data to use within one month. I think its from simple mobile or something like that. look it up. definitely worth it to figure out who stole from you. I would also get a night vision camera and hook it up to a laptop or something . The more evidence the better.

You forgot a THIRD possible solution:
If your car is purchased USED, then who ever sold it to you, may have spare keys.
that is a regular stuff here in Serbia - You buy a car, and a guy who sold it to you already has information (adress, even work info) cause of the paperwork, so he comes by your house and unlocks the car, and steals the stuff from it, or steals the car itself.
Anyways, that actually happened to me once, so I purchased the cheapest motion detector from china, from alibaba. My steering whell had the menchanic protection (the metal lock that goes over it) so there was no fear of him taking it for a drive. and when the guy entered my car I waited for him to fully enter, and locked him in via remote. and then me and my brother got out and disabled his stealing hardware (arms and legs) so that's the way to go by my standards.
the guy actually tried to sue me, but law also doesnt work here, as such cases (fights and such) work only if you have expencive lawyer.
Well I bought it from a very large Acura dealership in another state about 5 or 6 hours away. That was over a year ago, so I don't know how high of a chance that is. Although there always is a chance.
What type of motion detector did you use? I might go that route.

Anyway whatever the case since you have an old phone what I recommend you do is use it with your wifi to run the phone as a remote camera and stream and record the video to your hard drive. When the thief breaks in you'll have a nice clear picture of him taking everything, and the added bonus of the gps location if he/she takes the phone.

I wouldn't be able to use the GPS unless I had a data connection. That phone's inactivated. Unless there's an off chance that where the thief brings my phone is to a place that has wifi and also the chance that it's not secured by a password.

there are chinese *****y products but they work, I saw couple of them in Shenzhen, and they are awesome!
its a box-like little gadget, that has a SIM slot. then, you buy a prepaid simcard, place it in, and it works this way:
it has sound detectors, very sensitive. and when someone makes a sound, it dials your phone number, and it even enables you to listen whatever is going on in the room/space where it's placed.
it has a decent battery, cause the one I tested (considered smugling them back to serbia for sale) held on for 5-6 days with one charge.

Now, about the robbery itself, if the thief had the keys to your car he/she could have simply taken the car itself, so there's a possibility that they know you. Can it be your neighbor?
Oh that there's this, my car prompts me for a passcode on door unlock, and will alarm if passcode wasn't enter within 20 seconds. This sure could help in your situation. The only way to disable the alarm w/o passcode is to pull the battery.
You can also get a cheap proximity alarm for the house door.

Well the Droid incredible doesn't take a Sim card so that wouldn't work. The proximity alarm is a good idea though. Is there something available where I can put it in my car and not alert the person stealing, but let me know?

Anyways, if you dont find prepaid sim, use some sim card from someone else, or buy a sim card with the weakest plan (money save)

I contacted T-Mobile over the phone since I ordered it over the phone and they are shipping me a brand new one overnight. I should receive it tomorrow.
They stated it has not been an issue brought up to them yet so if any of you guys bought it at T-Mobile, go ahead and state there is an issue with it. I just received my case today so it shouldnt be a problem anymore. Sucks that this is definitely an issue though :/

A VERY thin layer of superglue, applied to the phone (not the letters) should do the trick. Make sure you put a very very thin layer, else the superglue will come on top of the letters when squeezed.

Just call T-Mo (or where ever you purchased it from), and tell them to replace it. You should have 15-30 days warranty from the seller (and 1 year from the manufacturer)

they said phone is out of stock for at least 6 weeks and i can return the phone under buyers remorse and there are no t-mo stores near me with that phone in stick. any one have any other ideas? i would be fine with a replacement letter E. will moto replace this or is the best bet to get through t-mo?
I know that to some folks this issue might seem petty but I'd be pretty damn pissed if this happened to my nexus. This is a 700$ phone and I wouldn't accept anything but a perfect phone. I really wish Google/Motorola had got their **** together for this launch, waiting 6 weeks for a replacement is some bull****.

Motorola does offer the warranty, so you can certainly contact them, and they should be able/willing to fix/replace it. Note that they (unlike T-mo) may not replace with a new device - they may instead send you a refurb).
T-Mo may be saying 6 weeks just to discourage you from returning it - you could try to ask them to replace it when available (since the problem happened within the remorse period). YMMV though
I found this problem with mine today.
chatted with moto. they told me that they would replace the phone for me. with the advanced exchange policy, they will send a new phone to me (3-5 business days) before i send mine back to them. She assured me that i would not have to wait on any inventory back order time.

Happened to me a bit as well. Make sure your phones are updated. Haven't tried changing the sim card yet but I know that some early builds of Marshmallow roms had a bug that said the card was removed
Also, for the user that asked how to change sim cards, the slot is built on the top, and can be removed by using a paper clip and pressing into the hole that pushes that sim card holder out of the slot
Same problem to me, it is not firmware problem and not sim card problem, i have got this issue on android 6 and now on android 7, Noone have the right solusion for this ?
Just to chime in, it's been happening to me too, since two months, but I think got worse since the last security update of about a week ago. Turkey region ROM.

It is really a problem.
Use a thin tape and cover the top portion of the Sim card and the tray. Make sure it's a perfect fit or at least sticks onto the tray real good. Problem could be the Sim card moving too much because of the Sim tray isn't a tight fight... Which is a design flaw from Samsung's part. Hit the thanks button if this helped u out.

I got the same problem. Sometimes 2 times a day. Some time 2 times a weeks. Sometimes server times a month. Probably caused by sim tray not hold sim card tightly. I will try to use electric tape to cover the sim if could solve

I also facing the same problem..its always appear "SIMCARD REMOVED..Please restart your device"..its happen to me about 1 and a half week ago after my Galaxy Tab A perform a software update...before this its never happen...is it an ANDROID problem or my Tab A problem?
I was having the same problem, I did everything I could think of to fix it, factory reset etc... I finally took it to ATT and had a new sim put in about 3 mos ago and haven't had the problem since. The tech there told me that it had to do with the new cell tower regulations ( I think he was full of BS and didn't know what he was talking about) and the way the original sim card was coded for Android 6.0 that once it upgraded to 7.0 and started getting the security updates that it was causing conflicts in the phone so I needed a new upraded sim. The good part was that I don't cost anything
Using medical tape is better than electric tape to attach on top of both simcard and simtray.
Hey guys I havent been able to find a thread for people experiencing this issue.
Since yesterday morning I have not been able to connect to Bell LTE, and have only had access to very weak and very intermittent 3G/HSPA. Its to the point where even sending/receiving a txt message has only 20 percent success rate.
I had the Sim card replaced with no improvement, and the people I've spoke to at the Bell store can only tell me its probably firmware issue... they've heard about... Have been told by Samsung that an update coming in approximately a week might fix it.
I've factory reset it a few times already with no improvement, and I'm the middle of downloading the oldest official firmware I can find to see if that will help. Has anyone heard of this issue with Note 5? I'm almost positive its hardware but hoping not as I dread the RMA proccess.
Its a SM-N920W8 NOT rooted running N920W8VLU2AOK5 firmware.
Any help is appreciated

This is very odd. Have you tried flashing arter97.com kernel? That could fix it.

Hey Brava thanks for the reply. I'm trying to avoid rooting the device unless I know there is a very good chance it will fix the issue, because if firmware is not the issue I'm SOL when I try to claim warranty on it.
I have learned through trial and error that I can't go back to older official firmwares. Pretty weird that... I was successful in re-flashing the latest firmware but with no positive result.

Sorry I misread your post. I thought you were rooted. My mistake.
I am rooted on Bell with an nfc sim with lte no issues at all. I would suggest selling your phone on Kijiji and buying a new one. That's what I did when the Bluetooth was broken.
Or even trying a different sim card from a different provider.

Note to self: Don't buy phones off Kijiji (as if I needed another reason).
I'm running that firmware on Bell with no issues - what happened to trigger the change in your case? That is, did you lose LTE right after updating, or at a later point? Any other changes that might have led to the situation?

Hardly 'paranoid' - the risks associated with purchasing off Kijiji are well established (as you yourself just demonstrated). If you told your potential buyers that your phone's Bluetooth was broken, you would be the exception - most do not, and good luck to the buyer who discovers such a fault after-the-fact. Unlike eBay, where you have both eBay Buyer Protection and PayPal protection, with Kijiji purchases you have none - against stolen and/or blacklisted devices, or even just broken ones like yours. Not to mention the risk of outright theft - especially when large sums of money like this are changing hands - which is why some police departments are now offering 'safe zones' for such transactions on their premises. Whether you are willing to run these risks to save money is entirely up to you, of course - but to suggest that others are 'paranoid' because we don't care to do the same when purchasing high-end electronics is both irresponsible and disrespectful.

This might sound really dumb/trivial, but have you confirmed that your APN settings are correct?

I got the newest firmware via OTA late november and it was working fine up until a few days ago. Can't think of any changes that I made that could have caused this, which leads me to believe its hardware. The radio bar indicator will jump between H+, LTE, then nothing randomly, the number of bars usually stay at full though but will periodically drop to 3 bars. Yet I almost always have a "No Service" notification and an "emergency call only" bar.
If found a suggestion to force the phone to connect only to WCDMA/GSM only which helps a tiny bit... still no where near usable.

Hey Ya I did notice the default Bell APN was missing alot of fields so I created a new one with all the fields as per Bell's website, but this only made it worse as far as I can tell.

Yeah, it couldn't be that simple of a fix! haha. Have you looked at the SIM tray to see if there's any damage? Perhaps there's some dust that's causing connectivity issues with the SIM card?

I'm actually having the same issue but with the s6 edge plus. Everything was working fine until the last update. A few days after the update, there was no LTE connection. I tried everything. Wiped cache, changed the sim, did a factory reset but nope nothing worked. Now I'm stuck with H+ which is waaaay slower than LTE. I think this is a firmware issue but I'm not 100% sure. I'm still debating whether to sell the phone or wait for an update.

After a week of using my bell note 5, I had similar thing happen: No Lte., slow and intermittent connections. Not sure why it stopped working but around the time it happened i tried out a new magnetic car mount so maybe that was the problem. Do you have a magnetic mount?

I personally don't have a mount but I doubt that's the problem. I went to the Samsung galaxy store today and they re flashed the latest firmware but that didn't fix it. They said it could be a hardware problem and its going be sent for repair. So hopefully they get it fixed or replace the device. I really don't wanna get a nexus 6p lol
I have the same problem on my note 5 with Rogers. Never had the problem until AOK5 update which I manually flashed because I purchased the Gear VR and I was rooted.
I've tried reflashing the stock firmware; however, that didn't work either. It gives me error. Then I flashed stock tmobile deodexed rom. It hasn't lost signal yet but let's see. I also noticed that my baseband is still AOK5. I believe that might be the culprit of the problem and also the reason I cannot downgrade in Odin. Any help is appreciated.
I have the Bell W8 Rooted and no problems at all. Either root or wait for an update I guess.

I recently installed a new screen protector.
After a day or two I started experiencing phone turned off after I take it off from my back pocket???
It never turned off in my hands or when put aside.
It happened frequently and I noticed sometimes battery stats is varied after turning phone back on, but I think it's normal because the battery Hold on good during the day.
Is there a way to record what is happening after I put it in my back pocket?
Is it accedently pressing the power button and touching on turning it off?
Is there a way to swipe to turn off the phone instead of the new android 11 power menu?

Ordered new battery and battery connector , it's onboard connector, take it to a reparing shop , and got it fixed .
PS
1-I doubt any reparing shop will replace battery connector because it's very smalle and hard process .
2- order 5 battery connectors as i did , it's cheap and in the replacing process accidently burned 2 connector and the third time was a success , so order 4 or 5 just in case.

Rear camera and flash stopped working even after hard reset and format.
I replaced the camera and everything is super.
Update 3
Battery is not charging again and stuck at 50% status.
I want to move on to another phone that has a great camera experience.
I honestly don't know if there is a similar camera experience such as pixel.
I watched dozens of reviews and comparisons of other phones.
I am confused

Seen a few of these at work. Grab top and bottom of phone and twist. See if it turns off. If it does RMA it. My co-worker said he thinks a solder connection gets broke from fatigue from slight bending over and over ie. sitting with phone in back pocket. In the couple cases of this we've seen the complaint has usually involved being in pocket, we give it a twist and it turns off. I guess skinny jeans kill some 4xl's

I twisted it and to my surprise it powered off ? ?.
I have had it with Google phones, no more.
They don't last a year with me.
I had nexus 4 from LG, wifi dies
Pixel 2xl touch screen stopped working, just bought a new screen but it turned out its the mainboard issue.
Pixel 4XL now back pocket issue.
I had older phones from 7 years now still working fine and they endured heavily usage and drops.
No more Google hardware ?
It's good to know about the twisting test. I almost want to try it on mine... but I have no problems right now and I don't want to stress a part that is so far not broken.
My Pixel 4 XL just started doing this last week (Ordered on release day from Google) and I have a RMA replacement on the way. Luckily mine was still under warranty but only by a day.

Google put out a notice recently saying that they extended the warranty for the Google Pixel 4 XL to add another year so you get 2 years now.
Since I repaired a broken back camera glass cover through a local repair shop, so warranty is over.
I found a work around this problem
I put it around my nick or waist through a case phone that can be worn around the nick.
Since my pixel only have this problem while pressed hard.

well... a couple days before Christmas my 4xl starting turning off randomly in my pocket. tmobile is sending a replacement (asurant) ..
did the twist test and it powered off... wtf google.. no face id on the 5? and more problems...I might have to get my first iPhone...smfh

Does any owner and lover of image quality bought another phone and liked its image quality, portrait mode of moving children or object?
My wife's 4Xl's rear camera and flash stopped working last week. I tried all the usual steps to get it back working to no avail. I did a little research and came across a reddit post that helped. I was able to reboot the phone and as soon as it booted back up pull down the top menu and turn the flash on. Surprisingly when I did that the flash came on and the camera started working again. I'm not sure if this a permanent fix but it has been a week without anymore problems. When it stopped working the flash icon in the pull down menu would say that it was in use by the camera. No amount of restarts or factory resets would clear it up but what I did above did the trick. Hopefully this will help someone else.

I googled a lot about camera issues , never came across of what you did.
I purchased and replaced the camera successfully .
But I encountered the battery issues one more time , but this time I had it with pixel .
I purchased a new S21 ultra .
Still testing it .
It's heavier and bigger , I don't like it so far .
But I am going to wait until I get to be more familiar with it.
But I am going to order a new battery and battery connector oboard and replace it.
In the meantime I am having S21 ultra in my hands , I still didn't have time to test it's camera image , I hope i like it similarly to pixel

But instantly the rear camera issue returned.
This happened last time I replaced the battery too.

I think it has to do with timing. I was able to turn the flashlight on and off from the QS menu before unlocking my phone. Luckily this solved the problem for me and it hasn't come back.
