Using 2012 Fire HD 7"
I rooted it and installed the second bootloader and TWRP 2.7.0.0 just fine, and it even booted back into the stock OS just fine
But then when I went into recovery to flash Paranoid Android and its gapps, I started running into problems.
I rebooted after that and it stayed on the blue boot logo for over 10 minutes, so I rebooted and did a factory reset wipe and tried again.
When I tried the wipe then, though, it then said that the wipe failed and that all of the partitions (data, system, cache, dalvik, etc) couldn't be mounted...
Also, I can't use USB mounting because it says that it cannot find USB volume information.
I can get into TWRP just fine, but I can't seem to flash the ROM because it always just bootloops instead after completion.
I'm not really sure what to do now. Any help?

Sounds like your recovery somehow got corrupted. At this point I would just suggest to get a fastboot cord and factory reset. You can always tried to ADB sideload a ROM onto TWRP, but if you can't get it to mount now I doubt it will mount to flash the ROM fastboot.

I followed the guide for installing TWRP on my KFHD7 to the letter, but now every time my Kindle reboots, it goes into recovery mode.
I've tried using idme to change the bootmode, but under TWRP I get idme:command not found.
I can boot to fastboot using my cable, and I can restore everything to stock and it works fine, but if I install TWRP it keeps booting into recovery mode.
Also, I have a problem where the volume on my kindle starts going up on its own, and the volume slider stays on the screen until I manually turn it back down. The button doesn't appear to be stuck, what else could it be?

Use this tutorial to push a ROM onto TWRP http://forum.xda-developers.com/showthread.php?t=2459498
I tried that, still no luck, it keeps booting in TWRP.
I am able to return it to stock by using SR Tool and everything appears to be right, but I have tried installing both CM11 and Pac-Man, both end up rebooting in TWRP over and over.
The steps I did:
1) Root device using Bin4ry's Root_With_Restore, option 1
2) Backup bootloader, system, and recovery roms
3) use fastboot to flash boot, bootloader, and TWRP
4) reboot into TWRP
5) Go to wipe -> Advanced -> Check system, cache, dalvik cache, and swipe to wipe.
6) Once it's finished, hit back and do a factory wipe.
7) click install and install rom and gapps
8) reboot system
it just keeps rebooting in TWRP.

Didn't you wipe cache and dalvik cache after ROM install?

I was trying to install CM12 on my VS980 LG G2, and in order to do that I installed TWRP 2.8 because that was what they recommended to use with that version. It worked, and I installed it by using the flashable zip from http://blastagator.ddns.net/twrp/2-8-3-1.php
I used this recovery to install the latest build of CM12 from here: http://forum.xda-developers.com/lg-g2/development/rom-cyanogenmod-12-0-lg-g2-t2944269 after wiping my data using advanced wipe as the TWRP thread says to. I wiped Dalvik, system, and cache, and then flashed the ROM, GApps, and SU.
When I rebooted, I was bootlooping. Not too abnormal for flashing things on this phone, so I went back into recovery to restore the backup I had made, to find that the backups file list is completely blank. No (Up a folder) or anything. So I panic, and think I wiped more than I have to, so in order to reset everything and start over, I did a format. It says it completes, so I reboot and get back into recovery, and push a new rom to /data/media/0.
Now, when I try to install that zip, it's not there. Nothing gets placed in there. Installing a zip from anywhere else results in bootloops because it can't boot anything, somehow. It's like there's two partitions and it's trying to boot from the one with nothing on it, and that's the same one I'm shown when I go into the restore menu. There are folders like /data/, /sdcard/, etc still visible if I try the install option, but they do not show up when I try to use the file manager in TWRP.
I'm not sure what to do, can I fix the partitions that TWRP is looking at somehow, or is that not even the issue here? I would like my phone to be usable and I feel that having the recovery it shouldn't be this hard to get it back to booting order.

I have close to the same issue on my vs980. Except I have twrp 2.7 and I can't flash it 2.8.4 it always fails. But cm12 and gapps were successful on 2.7 version of twrp except it out me in a bootloop. Which my backup and everything was still there for me to restore. Do you have an issues with random reboots after you put twrp on your phone too?

so I was about to flash a new ROM to my Z3C, because I was having some trouble connecting the phone to pc (drivers etc are ALL installed and have beend reinstalled too).
Since my phone was already running on .291, I skipped flashing stock rom, went to recovery, did a wipe, flashed 3.1.1.0 TWRP FOTA Kernel, flashed ROM, booted ROM (fine), forgot to flash GApps, went back to reco... Wait? No more recovery? Hmm.. Let's try booting system. OK! Went to settings and did a wipe. It took me into an older version of TWRP (I don't know exactly, something like 3.0.2.8?!), where I did the wipe myself (Data, Cache, System). I did system wipe because I wanted to flash the 3.1.1.0 FOTA TWRP once again (since it seemed to be incorrectly installed) and then install the ROM freshly. So, after installing TWRP (which was "successful") I tried to restart recovery to install the OS. WHAT? Again no recovery? Damn.. Now there's no more system to boot from.
Then I tried to put the phone into Flash-/Fastboot mode, to install the stock rom and start over again. Trying to Hold Vol+/- while plugging in USB doesn't do sh*t. The green/blue LED pops up for a sec and then disappears instantly. Entering recovery with hardbutton method won't work either.
Now I'm stuck with a phone, that can't do anything.. Is there any way to recover the phone?

I had an issue where my tablet would just reboot over and over. So I tried to reset it using factory reset. This has me now in a recovery reboot loop.
Hardware:
Galaxy Tab S2 (SM-T713) - wifi only
Software:
TWRP 3.1.0.1
Lineage OS 16
What I've tried:
Wiping cache and davalik cache several times
Formatting data
adb reboot fastboot
Wiping cache, davalik cache, and system (which made TWRP report no system of course)
adb sideload of lineage OS 16 - reported success but TWRP still said no system
re-flashing the lineage OS 16 ROM from SD card (this resolved the no system issue)
Reboot to system from TWRP and adb fastboot after the system re-flash
Odin won't respond and says it can't contact the tablet (multiple usb cords tried , etc) so no luck re-flashing OEM ROM
No matter what, it goes into recovery reboot and won't try to start the system. I can't find anything else to try through searches on XDA, google, etc. Any suggestions?
Not exactly the same, but I thought I had hard bricked my T713 about 9 months ago. I was on stock at the time (not even sure what build), and a bunch of wipes and formats later I was in a TWRP loop. I couldn't even power the machine down. Even that would loop me back to TWRP. I managed to back door my way into download mode by installing the boot.img from the external SD card. That didn't fix anything, but after that I was able to hold down power and volume down and spam the home button until I got to the download screen. After that I was able to get back into odin3_v3.12.5 and load the factory image. I am on LineageOS 16 official now, so I have no idea if this is of any help. I'm pretty sure the boot img came after drilling all the way into the XAR-T713XXU2BRB2_T713XAR2BRB2-20180206 file, since that is the one I eventually got to load.
Yes, I forgot to mention that symptom - it won't power down at all. Just goes back to TWRP.

Flashing the boot.img file did it! I was able to get in to download and use Odin to reflaah the stock ROM. Now to switch to LOS.

As the title suggest, I fail to boot into TWRP.
I have unlocked the phone and flashed TWRP (https://dl.twrp.me/raphael/twrp-3.3.1-0-raphael.img).
I could boot into TWRP using Power + Volume Up , wiped everything (except vendor) and pushed a custom rom to the internal storage.
I restarted the phone for some reason and cannot boot into TWRP anymore. Whatever I do, I end up in fastboot. Tried to flash various versions of TWRP with the same result.
Can you guys help me out?
Edit: I flashed the corresponding Xiaomi fastboot image and repeated the process, same problem.
Edit: Solved by repeating again.

In future, don't wipe everything. You should only need to do the factory reset in TWRP, which will automatically wipe data, cache and dakvik. If you are flashing over MIUI then you will also need to format data.
There is almost never any need to wipe anything else in TWRP when doing normal custom ROM flashing.
