Does your recovery work or does it boot into recovery safe mode?
If yes then, your system partition may be corrupted. If you can use fastboot then flash the recovery.
Try it. Recently I had a similar issue.

Hi,
I have taken nandroid backup using TWRP Recovery.
I have installed paranoid android after unlocking the bootloader.
So i have CWM now.
But now i want to go back to my Backup which i took earlier. But the CWM is not recognizing my backup.
Now i want to install the TWRP recovery back. Please Help.
Flash a kernel with twrp in it and then boot to recovery and restore the backup

Hello. I had an issue in TWRP when I tried to flash a rom I got the error that system was set to read only. Recovery allowed me to wipe system. I tried a few times and decided to restore my nandroid. I got the same error. I panicked, tried 4 more times, and it finally restored. I know the rom I was good (Resurrection,) as I checked the MD5. I also have flashed supersu and xposed. I am on the stock rom now. When I searched for a root cause, I only found it for other phones. Any ideas? Thanks.

Use TWRP Manager to refresh the recovery, and try again

So TWRP app said it could not flash due to error or already having the current recovery. I'll try flashify.

Did you flash the correct version of TWRP via fastboot?

I did. I reset defaults and it asked me again. I am at a loss. While I am no developer, (you all are amazing, ) I have rooted every device I have had. I am at a loss. You are all amazing at what they u do.

I am certainly not a developer either, I just like tweaking my phone and I try to be super careful about following directions when it comes to things like modifying boot, recovery, system and so forth.
I wonder if flashing the proper stock recovery, then flashing TWRP again would work? Please get feedback on this as I am not sure if it is safe. Also maybe check the md5 on the TWRP download.

You are seeing the slider bar at the bottom of the screen when TWRP asks to allow system write, correct?

Hi there.
I'm having trouble getting USB-OTG to mount in TWRP 2.8.7.1. I'm on mikeoswego's stock xt1096 ROM (Verizon).
I formatted my 16GB flash drive FAT32. It shows up and mounts fine within the OS. I even backed up my apps with Titanium Backup to the flash drive.
My issues start when I reboot into recovery. I'm unable to mount USB-OTG and it shows up as 0 MB.
Has anyone run into this issue before? Any solutions or tips?

You can try with twrp 2.8.7.0 version... I think it will help you...

Thanks. I didn't try 2.8.7.0, but did upgrade to TWRP 3.0.2. It's working on that version
