Icons stuck in status bar (Android L)

Hello all,
I'm running the latest 5.0, LRX21O, and I've noticed that icons tend to get stuck in the status bar. For example I still have the phone icon even though I hung about about an hour and a half ago.

(picture 1) Same thing happens work lockscreen notifications but with my Jawbone UP24, even the battery is low out shows a low battery icon and when i charge it and reconnect it still shows on the lockscreen as low battery (sorry no picture yet) where unlocked in the status bar it just shows this (Picture 2)

The eye move based on inclination of the watch and then center itself again.
Dinamic info:
Wifi: only appears if wifi is enabled on the phone.
Battery indicator: Its only visible when is lower than 25% and it will increment its visibility based on power left. (appears left of the date)
Buttons:
Pressing the hour open google keep.
Pressing wifi icon position opens google fit.
Pressing Battery icon position open alarm.
KNOW BUG:
Date format is day/month/years (for some reason cannot find how to display month with leading zero) to fix that I added the name of the month at the end.

Lock Screen Pattern Attempt Alert Problem.

Whenever I just turn on the phone to unlock using pattern, in the upper left corner of the status bar I get the alert (or you can say notification I don't know) saying that "you have incorrectly attempted to unlock the screen %d times." (yes, I get the ampersand d operator symbol) I don't know how to fix this problem. It looks weird when I turn on the screen. Somebody please help me with this.
PS. I use lineage os 14.1 & screenshot is added.

Strange Battery icon after unplugging charger?

I have a Note 9 exynos variant and I usually charge my phone after turning it off. However after charging has completed and the red led turns green, I remove the charger I get this strange battery icon with exclamation mark and the white circle around the battery slightly green towards the top. Anyone has any idea what is this? (Pic attached)

i thinks it is normal. i got the same icon after full charge, it started to happen after upated to pie. On orio the animation was nice.

Screen Problem

A week we got to display this blue band every time I slide somewhere with black background, and i didn't know where this come from if is a hardware or software problem

I also have that effect. One way to make it less visible is using Screen Balance (the app is in the Play Store).
My config: I have it set in Tint (it is in Color Mode that is in the options). Then, on the main page, it is in Fluorescent and in Strenght 5%. I recommend removing battery optimization and blocking it in multitasking, to avoid closing (sometimes it happens anyway).
