I plugged in my phone for 20 seconds and I went to check for email notifications and to my amazement it's fully charged.
This is now another possibility in battery issues. I know I'm using a faulty device (that's why I ordered a RMA) but this is kinda odd so I thought I'd share it with you guys.
Let's hope this instant charge sticks lol. I went from 7% to 100% in 20 seconds.

Had that also one or two times. If you leave it plugged in it goes back to the correct percentage after some minutes.
Likewise a minor software glitch that will get fixed soon, nothing to worry.

Good for example you need to go somewhere and you forget to charge it. Just plug it in for 20 sec and bam it's fully charged and problem solved.

This happens when you plug in the phone and the wall charger has no voltage, e.g. because you use a switch that is off. If you switch it on, the glitch occurs and the phone is instantly charged. Of course that's not really the case, though. Rebooting solves this most of the time.

The Galaxy Nexus had none of these issues. I just hope that Google fixes this soon.

Nope. It doesn't let me continue to charge until I restart the phone (for me at least).
My phone did this last night. I noticed it right away, so I just unplugged the cable and plugged it back in and bam it went back to charging its merry little heart out

I am also experiencing this issue with a Nexus 4 (few weeks old) running Andriod 4.2.2.
Anyone know if this issue is fixed in upcoming release?
Just happened on my Nexus with Nokia 1.2mAh charger and latest CM10.1 nightly, reboot solved it though

I think this is correct. I've had this happen 3 times in 6 months and it's only happened when I plug the phone into the charger right after plugging the charger into the wall.
I've since tried to leave the charger plugged in about 10-20 seconds before attaching the phone and it hasn't happened since (~3 months)

There is already a thread about this (http://forum.xda-developers.com/showthread.php?t=1783173).
It seems to be happening randomly, and for me only on overnight charges.
I'm not sure if it's by design or a bug, but it happens.
The weird thing is, if I leave it plugged in during the day, it will stay on. At night, it usually turns off after reaching 100%.
I had turned phone off when it was fully charged yesterday and put my Sim in Galaxy
SII and when I replaced back today and turned Nexus 4 on today, the battery was showing 73%. I can not believe it lost 27% in totally power off mode.
My phone is not rooted yet.

How long have you had your phone for? If you just got it recently, keep testing it like that for a few days. It could be just the battery sensor on the phone being new and all.
After I posted the first post, I was checking the battery in settings and it showed that my phone was on battery for over 22 hours. The weird thing is that I literally turned phone on from power off state and saw nexus boot animation came on and phone powering on.
I now think the phone actually do not power off in power off state.
Is something wrong with this phone? Or it is suppose to do that.?

Exactly the same thing happened to me. I just updated to 4.3 ROM, firmware, radio, plugged the charger and went to sleep. After I woke up (9 hours) phone was at 87%. Let it another hour, still the same. Than rebooted and 100%. IDK maybe the new firmware messed smth? I'll try again later and report back

Mine has improved as well, actually. Before, it would take a very long time to charge fully. Especially that 99-100% gap. It would literally take 20+ minutes to go from 99 to 100. Made no sense. Battery charging and life seem to have been much improved since the update.
The one thing that hasn't changed as that as soon as it fully charges, I can still unplug it and plug it back immediately and see the battery percent drop. Usually goes from 100 back down to 96-99%. I have never once, since I've had the phone, had it stay at 100 after unplugging and replugging immediately. Weird.
Mine actually charges faster and I get better battery life. I would do a clean install. Running 4.3 stock with goodies.

Check for corrosion and gunk in your charging port and connector. If none found try a different charger. The picture you submitted shows a quite odd report. Also could try wiping battery stats... but I think you might need an unlocked phone for that.

This solved my problem regarding the phone turning of at 50% battery or 37% etc. You will see after applying the settings the battery percentage will change and show the real battery life.
I got this solution from a Google tech.
1) Power off your phone.
2) Press and hold the Power and Volume Down buttons. This will put the phone in fastboot.
3) Use the Volume buttons to scroll through the menu until you see "Bootloader logs"
4) Press and hold the power key for a long time (7+ seconds) until the phone reboots.

I returned my phone a few weeks ago because of this issue and got a replacement from Motorola . I do notice certain kernels will create this situation moreso than the stock rom or stock kernel will not create this issue so that's also something to think about.
However, thanks for the insight should my replacement phone display the same symptoms.
Edit: interesting, I just tried your suggestion and my battery went from 91% to 66%.
I am on the validator kernel using the suggested settings.

On occasion at work but not at home. Sounds like another cause. Why would it if I may ask?
I actually find them to be silly as you must plug the charger base via USB which I can do with phone! Just another step IMHO including finding the sweet spot when resting on charger.

I will be using the phone for 6-7 days without the wireless charger to see if the problem comes back. If it doesn't I will then use the wireless charger and see if problem shows up. This is an issue with a lot of people and many of them have RMA'd the phone just to realize that the problem came back and it's not a hardware issue.

By using a regular 5v charger instead of the idiotic 5/9/12v charger that came with it.
I am having the below issue.
1. If phone is around the 30% mark, and I receive a call, it would just switch off.
2. If i reboot the phone with it being around 30% mark, it will just switch off after displaying the lockscreen.
Is this behavior that is same as the one in the thread?
I tried the mentioned steps and my battery percentage stayed the same. It also shows the same battery percentage in TWRP.

Yes exactly the same problem. Keep pushing the power button a few seconds longer after phone switches off. My problem was fixed but a week later it came back. For others it was fixed permanently. There's a lot of people with this problem and they keep increasing. I fear that certain wireless chargers are causing it. Are you using a wireless charger?

No I am not using any wireless charger. I only use the turbo charger at home and a 2A N7 charger at work.
I am waiting for 5.1 and will run it for a few days to test . Hope that 5.1 will fix this issue.
Also Google suppport asked me to reboot into safe mode and see if the issue is still there.
Just tried this and it went from 82 to 85%.
I have an issue where the phone turns off at 5%
I have this problem currently with my N6 and it is beyond madding.
First it would do it only when the phone was getting to 30~35% tried the fix.
Now the phone crashes it all the time. 80% crash, charge for a little bit use again 85% crash, charge to 100% use streaming a movie from google play 65% crash. now im going to try a custom rom to see if that will fix it.
tried the fix several time only seems to make it worse.
even with a blank install after reinstalling the stock img the phone will crash.
