Hi,
My phone wasn't rooted prior to JB upgrade. I'm currently on 91.2.26001.XT890.Retail.en.EU
I've successfully unlocked bootloader but now I'm facing a problem when I'm actually trying to flash custom recovery -> rom:
a) I don't have CWM \ TWRP recovery
b) Clearly I'm not rooted
c) While in phones fastboot stock recovery option just restarts my phone (as well as factory - I assume it's tied to factory reset).
Phone itself works completely fine.
I'm kinda stuck and in need of help.

Requires SU (as mentioned in RAZRi_Recovery_Flasher.bat)
And I'm not rooted. I've tried to do it manually step-by-step acoording to bat file, but in the end I get an error on this stage:

Why don't you flash CWM in fastboot mode? It's that easy... Download here:

Then simply reboot to CWM and flash RazriRoot2.zip
After that boot up normally, check root, boot into fastboot and flash CWM again -> done.
The last step is neccessary as CWM will be one time overwritten after rooting.

I have no idea how to flash CWM in fastboot mode. I've done following procedure:
got cwm.img from http://forum.xda-developers.com/showthread.php?p=41479852
Sent phone to fastboot, making sure device is connected done that in cmd:

And no result. I mean it gives out info that copying and stuff is OKAY, but there are no changes after reboot.
Or I'm terribly missing something.

You must not boot normally!
Switch directly from fastboot to recovery and then you are in.
Flash RazriRoot2 from CWM, boot up normally, then again boot into fastboot and flash CWM a second time.

You did it right, but you must not boot into normal system after 1st flashing
So again the steps:
- put RazriRoot2 onto SD card
- boot to fastboot
- flash recovery
- directly after that enter CWM recovery
- flash RazriRoot2 from CWM
- boot normally into system, check root
- boot into fastboot again
- flash CWM 2nd time
- reboot, done.

I fastbooted image, disconnected my usb. Then I choose recovery - aaaaaaaaaand it just goes black (shutdowns apparently). When I turn it on - I get stock loading, etc.
I'm confused.
Same thing happens when I do fastboot reboot-bootloader.
So my main issue here is how to get into CWM after actually flashing it, I guess.

Read my last description, step by step!
NO REBOOT, NO DISCONNECT, JUST ENTER CWM AFTER FLASHING!!
and IF phone shuts down, then go DIRECTLY to fastboot again with Power/VOL- and release Power immediately during(!) vibration with keeping VOL- pressed.

Alright, I'll sound completely dumb, but how do I do I enter CWM recovery after flashing? Recovery option in phones fastboot doesn't do anything for me. Or is there any magic combo?

Well... VOL- navigates and VOL+ selects.
Are you sure your device is connected? I have the same ROM base as you. When I enter fastboot mode, there is a black screen with some blue options. Do nothing, cable Phone to USB and check with fastboot devices if you are connected.
Then flash the CWM (try my link posted above).
After flashing just press VOL- one time to select "Recovery" then enter CWM with VOL+
That's it?!

As I wrote the last post I thought of that you maybe tried to enter with Power...
Then alright, glad it works now!
But don't forget to flash CWM a second time after rooting, otherwise RAZRi will "forget" CWM and install its own recovery again when you booted up. This must be only done once.

Just spent 6 hours doing exactly the same thing!
I've had 6 phones and thought this would be easy. Just missed the + instead of power about 10 times and couldn't work out why !

I need some help here.
Power button broke on my n1 sometime ago. Recently I tried to flash a new kernel but forgot to wipe and it's basically stuck at the logo.
I'm able to get into the hboot screen via bqttry removal technique but cannot select any option because of bad power button.
Any ideas how to flash a new room? I tried connecting USB to pc but phone is not recognized while in hboot mode.
Any help would appreciated as it's my only phone

Is your phone unlocked (ie do you have a lock symbol at the bottom of the 'X' screen)?
If so, I believe you can hold the trackball while booting up and that will take you into fastboot.
From there, I think you just issue the command

fastboot reboot recovery
If that doesn't work, you can just boot off a recovery file that is stored on your computer. Download either clockworkmod (I would recommend any v2.x as v3.x has some problems) or Amon_RA to a easy to type location on your computer. Then issue the command

Is there any more into that error? Did you verify the md5sum to make sure the recovery isn't corrupted?
Also, if you have a recent nandroid backup saved on your sdcard, you can pull that off and flash the imgs directly

./fastboot-mac reboot
Where these files will be located depends on your recovery. I know with clockworkmod they are located in the backup folder under the clockworkmod folder on the sdcard. I don't know where they are stored for Amon_RA, but I would assume it would be similar.

Finally got my phone working!

I have the same problem but whenever I try "fastboot flash" or "fastboot recovery" I end up with "FAILED (remote: signature verifiy fail)". I think this is due to that my bootloader is not unlocked, so I tried:
fastboot oem unlock
I got the option to unlock bootloader but I had to use my power button to select "unlock". My power button is broken so I don't know what to do.
Is there anything else I can do? Otherwise I think I have to open the phone and try to repair the button.
PASSIMG back to stock and root. Use "unroot/restore" section in Wiki.
If you're skilled enough with soldering tiny connectors - you can repair the button. There is a guide on this forum, showing the exact location to attach to.

I have an HTC One international. Today i try to get root access and install custom recovery, whent i flash recovery i missed that chose a boot partition to install it. Now phone boots only into recovery, and from my PC, adb or fastboot don see it, but in usb devices its present.
Im on linux Ubuntu 13.04.
In phone dont have any backup.
The phone can't boot in bootloader?

When i chose from CWM recovery menu "Reboot recovery", phone boot into download mode, and thats all. Fastboot or adb dont see it.
But. In usb device list:
In download mode:
Bus 002 Device 061: ID 0bb4:0ff9 HTC (High Tech Computer Corp.) Desire / Desire HD / Hero / Thunderbolt (Charge Mode)
In recovery mode:
Bus 002 Device 063: ID 0bb4:0f91 HTC (High Tech Computer Corp.)
Assuming you didn't corrupt your boot loader you should be able to get at the boot loader and use fastboot to flash a new recovery.
Power the device off
Hold power and volume down, the phone should turn on and load into the boot loader.
From there you can select fastboot (navigate menu with volumn buttons, power button to select fastboot).
You can use this command to flash a recovery:

fastboot flash recovery recovery.img
If you can enter bootloader,you may try formatting partitions from fastboot,and flashing them afterwards,that is if bootloader is unlocked.
You can also lock your bootloader and try installing RUU,it will put your phone back to stock,

fastboot shouldn't be affected.
when you connect your phone to the PC, does it the text in bootloader change from "FASTBOOT" to "FASTBOOT USB"

adb or fastboot is not recognize my phone. In cwm recovery i have only reboot option or reboot into recovery. I have the same situation as tha developer in other post.

Before, when you are in bootloader (image attached), are you able to "fastboot devices" and it shows up? because the bootloader is not affected by the recovery.
basically "fastboot flash boot recovery.img" put the custom recovery where the kernel is supposed to be.
First thing you need to get/confirm working is bootloader/fastboot.

In cwm adb will work.
So i think you have some issue with drivers.
And, however, if you turn off the phone, you can't access in bootloader with a longpress of power and vol-?
Hey, so I'm the one who had this same situation. What I had to do from what I recall is go to CWM screen, cleared the cache, even in advanced the davlik cache, then I believe (and you might have to try different combinations of this, because I don't 100% remember as I had before tried so many different ways) I unmounted the system mount, then went to reboot from the first screen to get to the bootloader (hold power button and volume down). After this, I clicked on Fastboot and it then went to Fastboot USB. This is where you should be able to connect. Don't leave this screen until you flash the recovery in the right place and then properly install the ROM. Let me know if you get to at least connecting and I can even send over an image for your boot, so you can at least get your phone back.
It can be done, I had the same situation so your phone is not bricked. Just keep working at it.

Hey guys, I need some major help!
I was trying to update my phone earlier today, when I flashed Android Revolution HD 40.3 something very bad happened!
For some reason the ROM did not install properly, and I can't get back into recovery. I was using TWRP and the loading screen comes up for it, but it never fully launches.
I can get into the bootloader, and I tried using the Toolkit under fastboot. It seems like my phone is detected, but when I try and flash anything it doesnt work. Ive flashed both TWRP and clockword. They seem to flash because it will show the loading screen of either one, but it never actually enters the recovery. What should I do?
My phone is S-ON, I am not sure if that is what caused the problem/is what is keeping this an issue.

Does the phone just reboot before it enters recovery? I would try manually flashing a custom recovery image. Here is a link to the exact image I used.

My phone was S-ON at the time, I recently gained S-OFF. You should still be able to flash a new recovery with S-ON. S-OFF is usually only needed for flashing new radios or firmwares.

How did you flash the recovery? Like I've said, I tried flashing a new recovery via the Toolkit. They do seem to flash, but I cant access the recovery. I'm not sure if my bootloader was relocked or what, but it doesn't go past the intro screen for TWRP or Clockwork

It could be possible that the toolkit is installing an incorrect version maybe? Switch off your phone. Once off hold vol - and power the phone on. This should load up HBOOT and tell you the boot loader status.
This is how I flashed recovery. I entered fastboot and used the commands from that link to flash a new recovery image. Have you used fastboot commands before?

To be honest, I'm pretty new to the fastboot commands. I usually follow guides to root, and flashing ROMs is pretty straightforward.

If you have fastboot setup and working, just boot the phone into fastboot mode and use the commands in that link I gave you.
From fastboot. Use command "fastboot erase cache"
From fastboot. Use command "fastboot flash recovery recovery.img"
Then simply highlight "recovery", tap the power button and the phone should load up clockwordmod recovery.

It doesn't make you sound incompetent at all! Yes its just the bundle, then the drivers should be installed with that too. If you're not 100% sure there are plenty of guides online on how to setup fastboot.

Ah, no dice. I followed all the steps, everything seemed to work (thanks for the guide). It flashed the recovery okay. When I restarted the phone, it goes into recovery, shows clockwork mod recovery, version XX and then proceeds to try and boot into my regular system

Hm, that's really strange. So recovery tried to boot, but the phone restarts and tried to boot normally?
Did you try running the command "fastboot erase cache" in fastboot mode?

Yeah, basically it goes back and forth between trying to get into recovery and try to boot normally but can't boot into either

OK so new recovery flashed, I would try running "fastboot erase cache". This commonly fixed issues with recoveries failing to boot.

So im stuck with my nexus with no OS... after flashing pure shamu it booted to just a password box... I input my password and it said i had corrupt data. I hit reset... wiped phone... all files GONE... Now what do i do. I cant mount to put recovery on.... I cant do anyting with this phoen right now... anyone had this happen... If so what steps to remedy?
Sounds like you need to try and fastboot the original google files, nuke and repave....

after a failed root, i accidentally deleted all data. Now i'm without OS, boot locked, and the TWRP recovery has been deleted too (when i select recovery mode, it come back to the fastboot menu). i cannot use "fastboot oem unlock"
I don't know what i have to do . I really need help, i just want to restart from the beggining like official rom ...

you cant unlock it without booting up and enabling allow oem unlocking in the developer options

If you can boot to TWRP recovery, then you can ADB push a ROM to recovery and boot it.
Edit: Sorry, I missed the fact that TWRP had been deleted too.

No, if you can boot to TWRP, as @rootSU said, you can move a ROM to internal and get up and running. Then you can go about unlocking.

and how can i boot twrp ? i'm like john snow, i know nothing
i only have a console, and a nexus - which cannot exit the "bios"

Power off the device. Once completely shut down, hold power and volume down until bootloader screen appears. Then use the volume keys to select recovery. Then push power to select.
from your original post, it says your not running twrp anymore. have you tried just letting the phone try to boot up? have you tried reflashing twrp to your phone via fastboot. id try, and if its successful, id push a rom to my phone then. if not, try flashing a factory img.

Or, if you are still in fastboot, issue this command > fastboot reboot recovery

as i said before, when twrp has been deleted, the Recovery mode is useless : when i hit it, it instantly return to bootloader screen

Then I'm afraid you may be stuck. A RMA may be in your future if no one else can offer a solution.

flashing twrp..
fastboot flash recovery recoveryname.img
and flashing a factory img is done while the phone is in its bootloader. there are instruction how here on xda. but to be honest, i dont know if that will work. where did you buy your n6 from btw?
I have traied a few different recoveryies and fastboot and mfastboot, but I can not get any recovery to ACTUALLY flash to the phone.
It is a XT1053 running 4.4.4. Even when everything says it flashed fine, the recovery is still not there when i try to boot into it.
Any ideas?
Use the Vol+ button to select recovery, not power.

What @nhizzat has said, is the most common mistake.
Beyond that, you don't mention if your bootloader is unlocked or not. I'm guessing it is, otherwise you should get some kind of error from fastboot/mfastboot on your PC.
One other thing that some have run into is... after flashing a recovery, having to reboot into bootloader before trying to enter recovery.... For example...
Start the phone in Fastboot/BootLoader mode and connect it to your PC.
open command prompt file and change to the folder your recovery IMG file resides in.
type the following, pressing enter after each command, and wait for it to finish before going onto the next...
mfastboot flash recovery recovery.img
mfastboot reboot-bootloader
Note: the recovery could be twrp.img, cwm.img, etc.
The device will reboot then display different BOOT OPTIONS
Use the VOL DOWN Key to SCROLL to Recovery and VOL UP Key to select it.

My thread title says its Unlocked, I will try rebooting to the bootloader first then trying recovery. I did not try that. I have been rebooting to the OS then rebooting to Recovery,

that is your issue right there after you flash a custom recovery you need to boot into the recovery before letting your phones boot into the OS.
if your phone boots into the OS then the recovery will not stick and you will be back on the stock recovery.
so
1.boot into bootloader
2.flash Custom recovery of your choice
3. make sure the phone does not boot into OS it can reboot back into bootloader and it can reboot into recovery with no issues but if it boots into the Phone OS then you will be back on stock recovery.
