Okay, I've tried everything I can think of, and I've managed to really confuse my phone and myself.
I took the 4.4 OTA update, and about halfway through it failed. The phone would not boot currently, upon booting the colors would go all jacked up with bars and lines. So..okay thats not good. Knowing some of the security features by hitting them head on in Odin, I finally settled for flashing only the AP image. Which gave me a booting 4.3 system with no wifi.
I tried to manually run the OTA in recovery at this point, and it fails with "error 7 /csc/contents.db has unexpected contents" I think that it what it said because this phone runs through the update pretty fast. However, it fails to update the system, but formats my data and cache.
So now I have what I assume is the 4.4 BL and 4.4 Baseband, running a 4.3 system with no wifi.
Is there any way to fix this csc issue so that it will update to a full working 4.4 system.
Phone: SGH-I537 (AT&T S4 Active)
BL: Assuming 4.4 BL since the 4.3 one will not flash due to "fuse..." error of sorts
BB: xxxxxNC9
AV: Android 4.3

Obviously you cannot reflash the boot loader due to samsung's security mess; but you can still flash CSC, Phone, and PDA back down to the 4.3 versions and it will boot up and run. This has worked once so far, but after I applied the 4.4 update again, back to square one. I'm thinking AT&T has sent me the incorrect update file via their OTA updater.
Edit: I have no audio now...
Edit 2: Took the AT&T OTA again to see if it would fix...forget it. I'm stuck with 4.4 BaseBand (with audio thankfully), Android 4.3 system, and no wifi again.

If you have not messed up the phone already, do not try this. It will get you stuck between a rock and a hard place.
Use odin to flash the 4.3 BaseBand. (I did this while flashing the 4.3 system also...) After that is done, take the 4.4 update. Then flash the 4.3 system ONLY from odin. You'll wind up with a 4.3 phone using the 4.4 baseband. But it fixed some issues for me. You'll get your audio back, but wifi will be broke again. I no longer have a modded s4a to mess with, my sustained water damage. So I was swapped for a new one. You should have an ML2 build, and an NC9 BaseBand.
It seems that the 4.4 update completely breaks the graphics subsystem and also updates you to a half baked BaseBand. And going to AT&T wont help you, even though it is their update that did this. Be very careful when updating to 4.4, it could leave you with a heavily confused phone. Also, the 4.4 update did not contain a boot loader far I could see by manually running the update. I'm assuming that is how I am able to still boot 4.3 after the failed 4.4 update. If you need any more information from my rough road with their broken OTA, just ask.

If you are unhappy/unsatisfied with the 4.4 factory image, you might have to reflash the 4.3 OTA or the 4.3 bootloader/radio to get WiFi working correctly when restoring an old backup.
Without doing so, you might experience long boot times at the "Google" logo and WiFi being disabled (which you can't turn on) when the OS finally loads.
It seems like the updated radio and bootloader in the new 4.4 factory image needs 4.4 now to correctly work.
I never tried the ota. But I go thee or four times some days back and fourth testing. But makes sense if hour stating the actual image and binaries etc.

Yea, I noticed this recently as well. Flashed the factory 4.4 image, didn't care much for it and restored a Nandroid only to have broken WiFi. All I had to do was flash the 4.3 radio, not the bootloader, and all was good.

Download the 4.3 images then do the same stuff what you did upgrading to 4.4. As easy as that, conflict might arise from 4.4 going to 4.3 using nandroid back up only
Hi,
I have a rooted I9295, after the last ota update (2 weeks ago) i noticed that my wifi stopped working and i get an annoying message "Prevention Information". I think the update failed on the radio part which is causing this.
I'm currently on android 4.2.2 with baseband version MF5 and ML3 buildnumber.
can anyone point to a fix ?

I'm not sure if this is the right modem, but it could be. Hopefully somebody else will reply with a proper solution.
The latest OTA update was for 4.3. Looks like you didn't update at all, but the process borked something up on your phone. Maybe a factory reset will fix your phone. Yeah, major pain, but had you gotten the 4.3 update you'd really be screwed since going back would not be an option and you would have lost root.

Unfortunately we're not so lucky in Europe, still no 4.3 update.
The latest OTA update we had here added some more Samsung bloatware and a nice message warning you to close the flappy charge port after charging.
I had a rooted 4.2.2, and i think that caused the problem.
Going to try to update the radio with Odin.

So i ended up doing this:
First i flashed the entire phone (since i couldn't find just the modem)

After that i rooted the phone again

And that fixed the issues.
Everything works fine again, and i didn't loose any data in the process.
I unexpectedly got a nice surprise this morning with a hefty 400mb update via ATT Update Server OTA. It was 4.4.4, not 5 as hoped but at least it's something.

The strange thing is after the update, I got an notice that it updated successfully, it said a software update was available again. Downloaded it, and it was almost 500MB but then said there wasn't room to install it. I cleared a bunch of photos and file and then the cache and when I went back, continue update was grayed out. I'm thinking I managed to delete it when I cleared the cache. Wonder if that was Lollipop. Did the date trick and checked update again and said up to date. So who knows, but there was nothing in the FOTA folder.
I called ATT and they were not even aware that it 4.4.4 was being pushed out OTA yet. Said that only 4.4.2 was the most recent, and of course knew nothing about when 5 would be out.
Not worried about it til I can have root on it.
Mine said software is up to date and i'm on 4.4.2. Can someone grab and post update file please so i can sideload by ADB..

when i checked update it showed getting update, but i was on Data so cancelled it.
Will try in Wifi at home and see how this goes...

The kernel swap root method continues to work in OA1 4.4.4 although the lag after flashing the ND3 kernel is extreme. Fortunately Safestrap also continues to work.

If I used this method last update, will ota fail? I mean do i have to flash back to get this ota?
My update wont install. Says install interupted. I think its because i had rooted this phone and even after unroot it shows custom on boot.
Do i need to odin wipe and if so what to use? Im on ng3 4.4.2....cant find the odin stock

I manually change date already 4 times and still get "software is up to date" i need .cfg file of last update.
EDIT: Never mind i got it..

Really buggy update and a visual POS!
- The Gallery app has text overflowing if the album has long characters.
- The Emergency Icon was poorly implemented you can see theres an ugly bar next to it and the AT&T logo if you have a passcode.
- When you press the recently opened apps icon the background turns a solid blue instead of showing your background makes it look like Windows 95 or something.
- App drawer lags every time you scroll a page.
So i had to Odin back to 4.4.2 nc build and then Ota to ng then 2nd Ota to this 4.4.4. Whew

I agree with you on the recently opened apps but everything else seems to be working fine for me. Maybe your phone didn't update correctly. Mine actually seems less buggy and quicker. Try a factory reset.

I was rooted on NG originally, but the OTA failed to installed. I Odin flashed to nc build (my phone shipped with NG) so I could install the OTA, but it failed. I redownloaded and tried 3 times. I even tried sideloading the NG update on SD card, but it fails every time too.
Did you have to do anything special to get NG to install? I can't for the life of me get it to work. Or were you originally on NC and in which case I'm screwed?

nope i was on NG - it was rooted like you but unrooting still left the custom logo at boot time. So I downloaded to stock NC - you must not flash the bootloader (BL) file in the 4 files in this rar at http://forum.xda-developers.com/showthread.php?t=2714455 - use the google drive download. You can't downgrade the bootloader. But as long as you don't flash the BL file in odin, flash the other 3 (AP, etc) and u should be good. (if you do flash the BL, no harm, odin will fail, pull battery and restart download mode remove BL from odin).
from there let NC load up and then OTA x 2 (advance date if needed) to the 4.4.4 build.

Hmm... that's exactly what I did, but the OTA update failed. I tried several times... wiped between installs, and it stopped at 32% or whatever it is.

now I'm trying the OTA again... crossing my fingers!

I searched and found nothing exactly like what I was experiencing and nothing I hadn't already tried. Hopefully someone has seen this before. I HAD a nice rooted stock SM-G900A 4.4.4 and now I have a flashy gold brick (soft). I rooted by downgrading the kernel to use towelroot then flashed stock kernel back. Everything worked fine for about a week or so. I'm not sure at what point I started having issues exactly so I'm not sure if an app or mild mod could have caused my issues. The only thing I had done was use SuperSU to block Knox and change the terrible TW stock sounds to standard ogg clicks and what not. The trouble started with the camera. I opened my camera and the screen would flash green then I would have red ghost images in a pattern on the screen. The pictures I took had the same effect when viewed in the gallery. If I switched modes the problem would go away with the camera until I closed then opened the camera app again so I assumed it was a software rather than hardware problem. I couldn't find any info on a fix so I did a factory reset. The problem persisted. I decided finally to flash back to stock. That's when it really got bad. After flashing to stock the phone will boot to the Samsung animation with the swoosh, freeze there for a few seconds without completing the boot animation then the screen shuts off and the phone becomes unresponsive. The only signs of life is the blue LED is constant lit. I have to do a battery pull to start over. I have flashed just the PDA and I have flashed the whole shootin match (except for BL) and I get the same results. I have tried stock tar files from a couple different sources. It will go into download mode as well as stock recovery where I tried a factory reset after flashing. PLEASE HELP!
So I flashed OA1 stock kernel and got it to boot. Of course now I get the Update interrupted message when I try for the OTA to anything newer than the 4.4.2 it's running. And after 8 hours of messing with this gem the camera issue persists.

That is what I ended up doing just an hour before your reply and all is well again. I suppose my first attempt to go back to NCE didn't work because I didn't use the downgrade flash, just a regular 4.4.2 stock. There must be a difference between the two. The OTAs still wouldn't work so I ended up installing the updates from external sd. The lollipop OTA is downloading fine though. Thanks for confirming I FINALLY went the right route. This root screw up wasn't as bad as my last (efs S3 debacle) but it still took too many hours to fix.

If you take the 5.0 ota you won't be able to root. Just FYI. There is no root for it yet. There is a pay however in the general section on how to upgrade to 5.0 and keep root.
My last post got no attention, so here is my second attempt.
A software update rolled out for my 5.1 Lollipop S5 Active, and since I wasn't really utilizing my root extensively, I decided I'd get rid of it to receive this update. Unfortunately, I didn't do a whole lot of research and decided I should start by uninstalling my SuperSU. After all, it seemed harmless. I then rebooted, and the thing will not surpass the AT&T boot animation.
The symptoms pointed towards a simple soft brick (i.e. gets to animation, recovery and bootloader not locked). However, I have not been able to successfully flash a fresh firmware onto the device with Odin. SamMobile only has one firmware release for this device, and it is the 4.4.2 version. Is this why the file is failing to flash? If so, how can I get around this issue? Thanks in advance!

Now if I remember correctly (if i am wrong someone please correct me) if your phone was on 5.0+ then you can not downgrade. Same thing with the build number where the old is NF4 i think, and the newest is OF3 with 5.0. my advice is to scour the internet and find the newest flash file. i know there is a 5.0 out there somewhere.
