I'm not sure what you are trying to do. Do you want to get your bootloader "relocked"? Or do you need it to say "locked" with no sign of tampering?

You should remove your serial number of your phone.
Your request is not clear. You want to un-root your phone and relock it correct? Is your phone operating normally? If so, then you will need to find the stock recovery for your phone and install it like you installed the custom recovery. Then after the stock recovery is installed you can use the command fastboot oem lock to relock your phone.
If your phone is not operating normally then you need to explain what it is doing and possible what you were doing to the phone when it stopped operating properly.

I apologize very much,I have opened my title wrong sory

mate, remove IMEI and s/n from your post. and if you want to remove TAMPERED and set it to LOCKED (not relocked) then you will have to do S-Off.

I am currently S-OFF and rooted. If i make it S-ON then the phone is stuck in bootloader. Any ideas on what I can do to un-root it (for warranty purposes)?

Which RUU to use?

Can anyone tell me where I can find the right RUU for my device? I accidentally whiped my OS this morning . I can get into my bootloader and TWRP, so I'm thinking installing the appropriate RUU will do the trick, right?
Do I have to gain root first, or am I good to go?
The getvar all result is this:
C:\adb>fastboot getvar all
(bootloader) version: 0.5

There is an exact match in the ruu section
http://forum.xda-developers.com/htc-one/general/lollipop-ruu-zip-small-collection-t3053944
Look down the list for 7.19.401.22
Note, installing the ruu will wipe the device (all data, twrprecovery etc)
I think you're good to go, but perhaps others will offer suggestions
Or just flash a ROM. If you have TWRP 2.8.6 or newer, you can easily transfer your ROM.zip to your phone, and just flash away

You can use the same one that you already have on your phone - 7.19.401.2, and take updates to the latest
