You must do it from a clean system without root (in theory) and the recovery of stock
so you must prepares a clean install with RSD lite and from there you have the zip ota, you must place it on the external SD and reboot into bootloader mode and choose to enter recovery, once in the recovery you must install zip from external sd
ota file has specific restrictions in origin (compilation of your operating system) to destination are upgrading so ota files will verify that you are in the point of origin of the operating system for which it was designed that update.
see here one example

[Solutions] OTA update not insatalling stuck at Red triangle

Many user at XDA everyday asking this same Question again and again.
Our phone not updating after getting OTA update.
Symptoms
Phone reboots to recovery but stuck at red triangle
Requirements
>>Stock recovery ( custom recovery TWRP/CWM can't flash OTA )
>>Stock system unmodified (custom kernel modifies system files so if you flashed custom kernel you need original files to restore)
>> Data/preload Apps ***Most Important*** according to phone software version (different CID contaions diffrent apps) when we unlock bootloader it delets data partition so data/preload apps also when OTA check OS integrity it also check data/preload apps with their md5/sha-1 match.
{How to Do}
1. 1st we need flash stock recovery (if you already have, skip this point ) you can flash it via Fastboot or Flash Image GUI App
fastboot flash recovery recovery_name.zip
Link for stock recovery
you can extract stock recovery from firmware zip here
2. Stuck at Red Triangle
means stuck at OS verification stage. You have to do 2 things
verify data/preload apps
verify system
extract OTA updater-script (open with Notepad++) from OTA (Meta-inf/com/google/android/updater-script) match apk file to OS.
Getting missing files - find these apks according to your system software version Like This or fastboot getvar all
Where to find apps
Previous OTA
Guru ROM Reset
Nandriod Backup
Anyone uploaded over internet
Find a HTC One m7 same version backup apps with ES File Explorer
Restore files using Root Explorer
rename apps according to updater-script copy to phone storage copy to data/preload REBOOT.
Most apps
calculator, flashlight, Htcringtonetrimmer, HtcSoundrecorder, 7digital, soundhound, Twitter_client.apk + more
Tools
Notepad++
MD5 & SHA-1 check
If this process is boring Lengthy to you, use RUU, Guru reset rom to get stock & then flash OTA.
Don't want download OTA again or use PC downloaded OTA
Don't want download OTA again or use PC downloaded OTA
copy ota file to PC.
start downloading OTA at phone over wifi again
stop wifi at 1% ( don't cancel OTA dialog )
match file name phone storage/download/ota file to PC file rename it.
copy PC file to phone/download overwrite existing file.
resume wifi it will show download complete Start Installing.....
The correct way to flash recovery is
fastboot flash recovery nameOFrecovery.img
fastboot erase cache
fastboot reboot-bootloader
Don't skip the last 2 steps !!

How to flash stock firmware from stock recovery - no sdcard, no apply update from ADB

Hi all, i've downloaded recoveries and firmware and tried to follow this guide - http://forum.xda-developers.com/one-mini-2/general/stock-ruu-rom-recovry-htc-one-mini-2-t3250020
But, i can't figure out how to install the stock firmware from the stock recovery.
The only options i get in recovery mode are
- reboot system now
- apply from sd card
- apply from phone storage
- apply from cache
- wipe data/factory reset
- wipe cache partition
i don't have an sdcard, and i've copied the stock ROM (zip file) to /sdcard/ - which is just an emulated sdcard.
However if i try of the "apply" functions then it fails.
Any advice?
Many thanks.

You may find instructions into post #3 of the thread above
Let me make it more 1•2•3
Rename RUU image (1.2 GB) to 0P8BIMG.zip and place into /sdcard folder.
Reboot device, entering into hboot.
HBOOT checks for such packages in root folders and installs them if they parsed check. All by itself.
So, basically, you don't need to enter Recovery if you want install the RUU. HBOOT does it.
Still, I may not be totally correct, cause I didn't check it with current device and when I used hboot I stored *.zip on /ext_sd.

You need to have S-Off to install just about any update/downgrade other wise with S-On you can only install update of a higher version.
You need to check your OS version in bootloader

i did try this, but it didn't do anything, I did see it flash up on screen that it was looking for 0P8BIMG (from what I recall) but then just entered the bootloader as normal.
Maybe it's because I have S-On as suggested by @TryllZ
The only way I managed to get the stock ROM installed was by using TWRP to restore the snapshot version.
Thanks anyway.

What I do?

I was on Oreo then I go to twrp make clean wipe
Then I flashed stock recovery but I forgot rename rom to update.zip ...how I can install ROM on my device pls help me.
How I can access my phone's internal storage .
1st Alternative : You can just reflash TWRP from fastboot and rename using TWRP's file manager. Then you can re-flash stock recovery from twrp itself.
2nd Alternative: If you have access to ADB in recovery, try this ADB file manager utility:
http://motyczko.pl/qtadb/QtADB_0.8.1_windows.zip

Help? I installed the wrong zip file.

I was on 8.9.13 and tried to install 8.9.20 with a zip file trough TWRP. I wan't to get back to 8.9.13, but even when i start my device it is stuck on region which my only option is China, but after that i get into a loop it wont get further.
I downloaded 8.9.13 zip file and want to put it in internal storage but i get many different maps with letters and numbers. When i want to copy the zip file 8.9.13, it wont get on internal storage.
I have my bootloader unlocked.
Please, what am I doing wrong? And what can i do?
Go to the Mitools, download the right rom, and flash it
Or install TWRP and install using the TWRP
