Last night I went to open Titanium Backup and was notified that I didn't have root. I've been rooted with ioroot for over a year now without issues. Root Checker app confirmes root was lost. SuperSu gave binaries error.
HOWEVER, in About Phone > System Status it was still showing as 'Rooted'. I'm still on 4.2.2 / VS98011A. All I've done is update the same ol' apps I've had for months.
What gives? Anybody have any ideas why this would happen?
The only thing I can think of is when SuperSu updated recently it asked to use TWRP (instead of the 'normal' method) to run a script to update binaries which I obliged. It seemed to struggle with that.
Anyway, I did use TowelRoot this time since I was too lazy to refresh my memory on the ioroot process. It worked fine however I've had a few random locks and a random reboot. I never had those issues before.
Any insight would be appreciated.

Do you have TWRP installed? If so try to get it to reflash the binaries again.

After rerooting with towelroot, i uninstalled SuperSu. Then reinstalled SuperSu and choose install binaries via 'normal' method. That appears to have worked fine.
My question relates to why did I lose su/root functionality in the first place? Is this known to happen sometimes with app updates, (not just system/os/ota updates)? That has me concerned.

I would assume not installing the SuperSU binary updates correctly broke SuperSU. You didn't lose root per say, you broke SuperSU which apps need to be able to run as root.

I've seen this topic mentioned in the forum but I have yet to find an answer that works. Maybe there isn't one, but here goes...
I have a Droid Maxx that shipped with 4.4.2. I rooted it with the PIE Exploit and everything went as expected. I set SuperSU to survival mode, and took the 4.4.4 OTA. This did not seem to kill root access, so again, all was well. Then, I installed Xposed and rebooted...fine. Installed some modules, configured them, rebooted..."Xposed is not active." So, I updated the framework and soft rebooted...everything works. However, if I hard reboot at any time, I get the "latest version is not active" error in Xposed. The only thing that fixes it is to update and soft reboot.
Mind you, root access is surviving these hard reboots. I check with SuperSU, Terminal and Root Checker each time just to be sure, and root is not failing. So what the heck is the issue with Xposed? Every time I see this topic brought up, the only advice is to update Xposed or to make sure you have root. Check and Check. Unless I'm missing something, it seems like root is permanent since it survives reboot. I've tried uninstalling/reinstalling everything one at a time, denying/granting root access to Xposed, etc. but nothing fixes the problem.
Is there a solution to this that I'm not seeing, or is this actually an unfixable bug?

And achieved root. All was well, but about 24 hours later, my tablet decided to restart, and when it rebooted, I no longer seemed to have root access. I then confirmed this with a root checker app. I still had the Google Play store, and the SuperSU app however. I checked using a file explorer and I still have an su file under /system/xbin.
I tried a reboot using the same method as above, but it seems to be failing because the files already exist. I tried uninstalling the SuperSU app using the full unroot option inside the app, but it simply says 'uninstalling, please wait' for a few seconds and then goes back to the home screen, SuperSU still there. I then attempted to restore to factory defaults, but alas the app remains and I am still unable to reboot (as well as losing all my data sadly). I am getting a drop down notification telling me that 'the SU binary needs to be updated' but that installation fails as well.
I'm unsure how to proceed trying to reroot my device, and feel stuck. Any help would be greatly appreciated. Also, I'm not sure what caused the device to restart and me to lose my root in the first place. From what I've read trying to figure this out, an OTA update may of caused me to lose my root, so I'm also wondering if there's anyway to prevent this from happening again if I can regain root.

So after getting my replacement N910T, I rooted it using the method here, and it seemed to go well, but during the process of reinstalling apps, I noticed some erratic behavior like the device not allowing root access when root checker says it is rooted. Or SuperSu not asking for SU access on apps that normally do, thereby denying root access, like Root Explorer. When I go back and check root checker again, sometimes it shows that it has access and other times it says that it doesn't. For a device with a supposedly unlocked bootloader, I've never encountered so many issues. My AT&T S4 which has a locked BL never gave me any issues once it was rooted. With all the devices I've rooted over the years, I've never had this much trouble. So I'm wondering if it's corrupted somehow? Or is there something about this systemless root that is the issue?

I've read threads until I'm crosseyed and googled but couldn't really come up with anything, so I could use some help here.
I had the same problem with a Note 4 Tmo I am working with. What SuperSU version are you using?
I had root issue problems (root chk says rooted, issues in ES File Expl, etc) when I used anything higher than 2.65.
Try flashing SuperSU 2.65 stable through recovery (I use TWRP 2.8.?.?) and when exiting out of recovery DO NOT allow SuperSU to be installed as SuperSU installation says...

Hi have you figured out your issue yet. If not, Try using supersu 2.52. It's the last full system root I believe. I have had much better luck with full system than systemless.

I haven't tried anything else yet, I giving my head some time to clear. I've been at this for hours now, so I need a rest. Can I find that version of SuperSu as a flash able zip?

Okay, the installation of supersu 2.52 via TWRP has definitely made a difference! The device is noticeably stabler now. One last thing, though: root checker shows it's rooted, but says that a superuser app isn't installed, which is what I thought supersu was. So now what?
Did you flash over 2.71. And do you have a su app icon on your scrren.

Yes, I just flashed it from TWRP without doing anything else. And no, there isn't a Su app icon on the screen. I have a nandroid backup that'll put it back to the way it was before I did that, just in case it crashed and burned.
Download the app from playstore and then go to settings and do a full unroot. Then reboot to recovery and flash 2.52 then wipe cache and dalviche and reboot
MUCH BETTER!!! I'm in the process of reinstalling all of my apps; I don't want to take a chance of restoring my backup from Titanium, just in case there's something that won't play right.
I'll report back in a day or so.

Yes, it's been a few days and my device is working the way it is supposed to. Interestingly enough, my SuperSU has automatically updated to version 2.65 but it's still working fine.
I've since turned off automatic updates in Google Play for fear of getting a version that begins the cycle all over again.
I still have the issue with the device telling me that the recovery is not seandroid enforcing even though I've updated to TWRP 3.0, and the set warranty bit: kernel issue. In spite of these, my device seems to work just fine with no issues.
Thanks again to everyone who offered help. It's nice when people offer help instead of posting just to tell you to find the answer yourself.

My research on this has taken me all over the place, but none of the suggestions or explanations seem to apply. The device isn't ostensibly crippled, so I don't know.

I got a note 5 SM-N920W8 Telus last week. First thing i did was to upgrade to marshmallow, and then root it. I rooted it fine, wiping the phone in the process of installing the custom recovery. Now i've used plenty of apps that require root, and all have worked fine. About 15 minutes ago I try to go in "Ad away" to disable it temporarily to check if firefox was gonna to react faster on loading pages (it seemed so fast last week, but sluggish lately) only to find out Ad away is complaining I dont have root access anymore. So I go and check in SU , and sure enough, he also complains I don't have root. Marshmallow 6.0.1

Im having similar issues. 6.0.1 on my tmo note 5 rooted perfectly using Max Lee's method. However as soon as I rooted and transfered all my data back I did a nandroid backup. Happen to wipe my phone the other night and and restored the nandroid backup and as soon as it restored I lost root. Happens everytime I restore even a newer nandroid in twrp. Never had this happen on any root method on any note.

It's happened again this morning.. Root dissapeared by itself. The only recent thing I can think I did , was buying a game (The cave) from play store last night.

I have a Samsung Galaxy Note 5 on the Sprint Network. I just now noticed that my applications that require root access, including Titanium Backup are reporting that my device is no longer rooted. Weird. This has never happened in the past when I had rooted Samsung phones. I will re-flash the Super User file as instructed in this thread and see if that resolves the issue.

Per your insstuctions, I reflashed SuperSU 2.74 using TWRP. Then once my Note 5 rebooted, I launched Titanium Backup and found "SecurityLogAgent" and uninstalled it. However, in order for me to access TWRP I have to reinstall it using ODIN and immediately after I get the green "PASS" light in ODIN I have to quickly do the 'three finger' button press to launch TWRP. Weird....
Anyway, so far so good.

Thanks that worked to a certain extent. Titanium requesting root privileges for backup would still pop up as failed. Checking root would show as unrooted. The Superuser app was updated on the 5th of July 2016. Since I applied that update, i have not lost root on any of my devices.
I recently used the "New Root Method For 5.1.1 w/o custom kernel" to root my otherwise stock Note4 SM-N910T.
Everything was fine, but after a few weeks I lost root. I was able to get it back by re-flashing SuperSU 2.65 from recovery.
After further research, I found the blog post by Chainfire on SuperSU 2.74, and understand that 2.74 addresses the Samsung security update.
I've been using "SuperSU 2.65 Stable" because of root access compatibility issues with some programs.
How do I know if the Security Policy update was the cause of losing root? My phone's About Device has "Android Security Patch level" as 2015-11-01. So it doesn't look like something that was updated in the last week.
If it was not the security policy, then what caused the loss of root?
I have automatic updates off, and did not update anything at the time I lost root. Is there any other way that an update or change can be pushed? My /data/security/spota folder is empty. Should I delete the empty folder just to be sure?

3) 1 to 3 weeks later - something disables SuperSU, and root is lost

I want to understand what is happening in 3) so I can exit this annoying loop.

This has only happened once? Interestingly, I had the same exact issue happen to me on my S5 with MM, I rooted using chainfire and just out of the blue, all root privelages were dismissed. So I flashed SuperSU v2.65. It happened multiple occasions until I was able to switch to an AOSP ROM. After going completely custom it seemed to have stop doing this. Maybe because of a contrast of coding that differ in security.

It has happened twice. I'm working on a Tasker recipe that will check for root regularly and report when it is lost.
Now it has happened three times. Still trying to figure out what causes it. I have not found a tasker function that checks for root.
Unless I can find a solution, I guess I'll have to go back to the traditional root with beastmode kernel.

Nobody seems to have any idea what's going on. I'm guessing the loss of root was specific to SuperSU 2.65.
Here what I've tried today:
1) Recovery / Flash with SuperSU 2.65 to get root back.
2) Update SuperSU Pro to 2.76 from Google Store. Download and copy ZIP to phone storage just in case.
3) reboot and verify root still available from Titanium Backup
4) Uninstall ES File Explorer Pro. Install Solid Explorer
5) Test root access of Solid Explorer. It works. Buy Solid Explorer.
6) reboot again just to be sure root remains
7) Backup with TB.
So far, everything is working. Hopefully it will remain rooted.
Almost 3 weeks, and no loss of root. Super SU 2.76 seems to fix the problem.
J500G lost root minutes after connected to internet. I have to root every single day.
