I am not sure what the issue is, but I unlocked the bootloader, rooted my Dev Edition XT1053 and now I can't get signal on T-Mobile. I installed TWRP 2.8.1.0 and then Dirty Unicorns 8.0 (4.4.4). I see in settings->about phone that "Baseband Version" is "Unknown". Did I somehow erase my radio?
I tried searching around for reverting back to stock but seems that there is a risk of bricking in case of downgrading. Not sure if this applies to downgrading from a 4.4.4 ROM or not. But in general, I couldn't find a radio image for this phone.
Any ideas on how to proceed?
FYI-- wifi works just fine
Download the factory images for your device and android version and flash the radios with these instructions. I see you are not on Verizon so do not use the radios in the post I linked. But the post I linked will give instructions on how to flash your stock radios.

switched to pacman rom, now baseband version was listed as
2323.020.90.00R
but still did not get signal
flashed the radio from 4.4.4 TMO retail and baseband version says
2323.038.123.00R
after that, worked perfectly fine. thanks for the help!

Need expert advice to fix the phone. Friend of mine managed to break his device and now he turned to me for help. I don't have any history of the device but here are all details I can gather from about screen.
Phone is rooted and running custom recovery TWRP 2.5.X
Phone make model - LG G2 VS890 4G Verizon variant
Software - Simple ROM v1.1, kernal 3.4.0,
Build - LRX21Y
Android 5.0.1
Baseband - Unknown
Valid entries for ESN, MEID, IMEI
MIN, PRL VERSIN, ICCID is unknown
We wish to use this phone on any GSM network assuming Verizon version is GSM UNLOCKED.
If you need any more details please let me know.
So far I have tried flashing 12B and 24A radio using TWRP but no luck. Baseband is still unknown.
Any advice?
To answer my own post, I tried flashing Cloudy G2 2.2 ROM along with KitKat baseband.

I updated by boot loader from 4807 to 4882 so I could install CM 12.1 official ROM. After installing cm12, I realized that I got no service. I then switched back to my previous rom which was the Titan Prime with my CWM backup. I still got no service. Then I used *#*#4636#*#* to check for any problems. I realized that radio was off so I pressed "Turn Radio On". The radio won't turn on. I believe that the radio is the issue. How do I get my service back?
Carrier: Bell Mobility
Model: XT1064 (unlocked, Canadian).
PS: The SIM works on my other device so the problem is the phone.
Edit: I fixed the issue by flashing the modem and fsg from the old 4.4.4 stock image.

What ROM did you flash the modem/fsg on top of? Experiencing an identical issue intermittently (sometimes the radio will shut down entirely and pressing "Turn Radio On" doesn't do anything.
So I have a Chilean XT1097, which I converted to Pure. To update it to 5.0 I needed an EFS backup; if I didn't restore it I'd get a "no signal" icon on the status bar, along with no IMEI and an unknown baseband version. If I restored the EFS backup everything was fine. The thing is that I just updated to 5.1 using the OTA, and whether I restore my EFS or not does nothing; I get something even worse; status bar has a "no SIM icon", and along with the unknown baseband if I hit the IMEI combination on the Dialer instead of showing me a simple "IMEI" screen with no number, as it did on no-EFS 5.0, the app now crashes.
I've tried the following:
Updating from 5.0 with EFS restored
Updating from 5.0 with EFS restored, restoring the backup again on 5.1
Updating from 5.0 without EFS restored, restoring EFS on 5.1
On 5.0 with EFS restored, backed it up again (since previous backup was in 4.4.4), updated to 5.1, restored backup.
None of these work. Any ideas?

I have an XT1097 from Telus in Canada. with 5.0 I could use the modem from my providers 4.4.4 or 5.0.1 build and it worked fine. When I tried the original XT1095 5.1 soak the modem worked without any issue. I assume maybe that was simply because it was a soak test. In the final version you're using it sounds like they went back to a model specific modem again. It probably isn't compatible with a 5.0.x modem which is why it doesn't work for you. You may have no choice but to wait for 5.1 from your provider to use it. If I get time and feel like playing I may give it a shot myself this weekend.

Yup, this seems to be the case. I don't have the original ROM my phone came with, so I guess I'll flash the Brazilian 5.0. I hope that when it updates to 5.1 I have no modem issues (for what it's worth, updating to 5.0 didn't require me to restore my EFS backup!).
Which phone model and provider do you have specifically, I can look for the 5.0.x update for it assuming there is one.

XT1097 on Movistar Chile, latest there is 5.0.2. Shouldn't I need a 5.1-specific modem, though?

Most likely but I figured it would be nice to have the original 5.0.2 for your specific phone but I don't see it in my list of firmware.

Yeah, I couldn't find it either. The closer I got was the Claro Chile (the only other carrier that offers the phone here) 4.4.4 which, due to a falling out they had with Motorola up there in Mexico, stopped selling and updating Motorola phones.

Alright, the solution is to never restore EFS. I suppose it can also be accomplished by flash stock modem and fsg on 5.0.
I flashed
TMO_Retail_XT1053_5.1_LPA23.12-15.222.21.15.ghost_row.Retail.en.US
and XT1058_GHOST_RCICA_5.1_LPA23.12-15_cid14_CFC.xml
today, both get no signal and IMEI is "0"
It happened before when I was flashing RETAIL-XT1058_BR_5.1-222.21.7-BR and I fixed it by flashing (NON-HLOS, fsg, motoboot) from ATT_XT1058_4.4.4-KXA21.12-L1.26_CFC_1FF.xml
When I tried to do the same thing today, I get the error
downgraded security version
update sbl3 failed
Do anyone know how to fix this?
I use ATT in US.
I am not sure about my phone model. I believe I picked US Celluar Factory Unlocked phone, but it writes Model: xt1053 at back
You should've only flashed what's for your model only. If it says xt1053 in the back of the phone, only I repeat ONLY flash official firmware that is only for your phone. If you flash a different models firmware, then it causes hard brick. Since you flashed 5.1, there is no possible way to downgrade at the moment. And if it was uscc, it would say xt1055. I know because I have one.

flash manually tmo retail xt1053 rom thats for you phone.
days ago i flashed brazil retail radio on top of my xt1053 with official stock US OTA 5.1 and got imei 0 as well. fixed it downloading tmo retail rom and flashing that radio only (NON-HLOS.bin and fsg)

I was about to update to computerfreak's .49 rom (version 1.37 that is).
Prior to that I wanted to update to the latest modem, So i flashed the modem_cleaner by baybuther27, and then the .49 radio from firstencounter's thread
Since then my baseband is unknown. I can't connect to wifi and my sim isn't being read
I was BL unlocked and on computerfreaks 1.35 till now with no issues (except GPS wouldn't lock).
Would greatly appreciate help on this. Thanks!

If you made TWRP backup, restore the modem backup category.
or you can try re-flashing the previous radio that worked for you.
