Long time ago, my phone had the usb host mode problem when plug in into pc, but can charge at OS. So i didn't bother it because it still can transfer files to PC via USB OTG.
Last month, itself random went into usb host mode when charging and the phone totally cant charge, need to restart then its ok. When i go into fastboot it shows FASTBOOT AC and then FASTBOOT randomly (like gotten shock) and after awhile the problem gone so i didnt bother about it.
Last week, I unplug my phone after i wake up and then itself shows USB HOST MODE, so i tried connect USB OTG and then it didn't works. I thought it was kernel problem so I downloaded ElementalX 18.2 and then i flashed it. Now, after boot, the phone wont charge. It only charge when powered off and RECOVERY mode. Fastboot shows no sign, no FASTBOOT AC or USB, even OTG power. This is strange and weird.
Also, i flashed ARHD 81.0 at older version of TWRP Recovery, and then after i got that problem, i reflash my phone with TWRP 2.8.0.2, at the end it says : "set_metadata_recursive some changes failed" , and the phone can't boot. Luckily i got a nandroid of it so i flash it immediately. It can boot, but the no charging problems still exist.
I later upgrade my TWRP to 2.8.3.0 via TWRP Manager, still get "set_metadata_recursive some changes failed" problem. But, I found out that after i flash the rom, then flash nandroid, when i boot into OS with charging, it can charge in the OS, but not anymore after i went into fastboot or recovery.
And something interesting is, when the phone boot, haven't load the drivers, itself began OTG mode and powered up my powerbank, PLEASE HELP!! Im totally stuck on this!
Does this is a hardware or software problem? Its this the USB board broken or the motherboard? Please respond..

Phone Information : HTC Incredible S, S-ON, 4EXT Recovery
I was using SVHD 2.1 ROM from quit some time, since last week, phone was lagging too much and when I connect phone to PC it was not showing me storage options. Phone was charging but I did not see any increase in battery percentage. I wanted to try VIVOKAT by szezso so went for flashing the ROM. This is where the problem started.
I did wipe data, wipe cache, wipe dalvic and wipe boot. After that I flashed VIVOKAT and when I try to flash boot.img, I found that it was not showing me "fastboot usb" mode. I tired different cable with no luck. And as expected phone is stuck in boot animation.
I thought there might be driver issue, so I re-installed HTC Sync but that also did not help.
Later I did restore of backup taken using CWMR before rooting, Now, restore appears to be successful, but phone is not booting, when I try to start it, it restarts into recovery.
can you guys please help me in resolving this issue
I tried with other PC and other HTC phone combination, USB Cable is fine and other phone is detected so I believe my phone's USB is not working.
When I restarted my phone with USB connected windows indicates that device is connected/disconnected and gives USB error. If I click that message USB devices window opens and it says - Windows has stopped this device because it has reported problems. (Code 43).
What options I have now?
I reflashed SVHD 2.1 and kangaroo kernal v19 on which I was earlier and phone booted
will check USB again after full recharge and will let you guys know.
anyone knows if replacing USB port (phone hardware) will help? asking as phone is charging normally when connected to charger but not when connected to PC
So today i unplug my phone from my charger, then i wanted to transfer files via USB OTG. So i immediately plug in my usb otg and my pendrive(cuz pc cant detect my phone due to usb problem), sadly it wont work (got usb host mode). then i download latest elementalx kernel (18.0) and flashed it. after that, i immediately go charge my phone, nothing happened. no usb host mode bug, no landscape mode.
then, i entered fastboot while charging my phone, it only display fastboot instead of fastboot usb and its not charging. Tried reinstall rom (ARHD81.0) three time, one of them it will charge after boot, but NOT anymore after i entered fastboot. Tried wipe system and cache, still not charging. Only will charge when powered off and in recovery mode. Help! Some of my friend told me that my battery is dying, isit true?
Just now tried to use usb otg, the phone powered up the device that connect to usb otg. Guess it straight went into usb host mode when boot. Any fix?

I had an HTC rooted and after installing busy box it couldn't anymore charge/detect when usb to computer was connected.
If the OS was not powered it would work to charge or give commands to the smartphone via ADB/fastboot.
I have factory reset and relocked the smartphone with fatsboot flash ruu.zip
The problem is that the problem still persists when the phone is booted in Android (no charge or detect usb).
I have currently tried the following actions with no success:
1) boot into safe mode, reboot
2) disable daydream, fastboot
3) charge for 10 min while phone is off, disconnect charger and continuously hold power+volume up+volume down for 2 minutes
Does anyone have an idea what could be the issue ?

Maybe the USB port. You can replace the usb port by replacing the little board (USB port + main mic).

After also cleaning the usb port from the smartphone the issue still appears.
It is very possible to be the micro usb port like you mention. I will send the mobile to warranty as this issues take a lot more time then I am willing to invest in.

I have a major issue since the update to CM13.0-20160524-NIGHTLY-huashan. I litteraly spent the whole week trying everything I found on internet in order to solve it without success.
My problem is the following one: I uninstalled the gapps to manually update it for the latest version of Android (Android 6.0.1), when I tried to boot to the recovery nothing happened the phone just restart, same when trying to boot to the bootloader. I tried everything adb reboot recovery adb reboot bootloader, I tried flashtool, I used the developper options to reboot to recovery, I tried manually by holding the volume button while restarting, I tried the flashify application. Nothing, I am not able in anyway to access neither the recovery nor the bootloader. The phone is just restarting, the led is first blue then goes red then green then red and restart. However, I have a root shell on the phone with adb shell.
If anyone have a solution, can help me please do it. An Android phone without the gapps is quite useless.

Hello, since the LED bar lights up this means your device is soft bricked, in which, mostly are recoverable.
What interests me is that you tried to, quote, "uninstall gapps".. If you previously have gapps, you don't need to "uninstall" gapps in order to update to the next build. Just flash the new build zip and gapps will be preserved. Also I am quite skeptical about what you did, as I never heard of a way to actually uninstall gapps. Basically if someone wants to remove gapps, he needs to format the System partition and then flash the ROM zip again..
Are you sure you failed to boot into flashmode? the correct procedure to do so is
Make sure the device is turned off
Make sure the USB cable is connected to your PC
Hold the volume DOWN button
Still holding the button, insert the USB cable to the XSP
LED will light up green, indicating flashmode
But even so, since you are on an unlocked bootloader, recovering from this brick is rather easy.
Get yourself a TWRP img file from the TWRP basketbuild folder
Open flashtool
Make sure the device is turned off
Make sure the USB cable is connected to your PC
Hold the volume UP button
Still holding the button, insert the USB cable to the XSP
LED will light up blue, indicating fastboot mode
On flashtool, click the lightning icon and select fastboot mode
On the right of the new window that appears, select "Select kernel to flash"
Navigate to your TWRP img file and select it
When flash completes, unplug your XSP from the USB cable
This will flash TWRP into /boot partition. So when you start your device, it will go straight to recovery.
Now, flash the ROM zip

And when I said I delete the gapps, I meant that I deleted the google play store from my phone (this app was anyway not working after update). I tried to use flashtool but it is the same, can't get into fastboot mode, the phone just normally starts or put itself in charge. Isn't a manual solution on how to flash a recovery only using adb shell?

That seems to be related to your USB cable (since that is the famous issue). Can you try to charge your phone WITHOUT it stop charging (i mean suddenly the LED disappears and when you tap the screen the charging icon doesnt show)?
If it so does happens that your device doesnt want to charge, it seems to me that you are having a loose USB port which possibly from your XSP, or, your USB cable is no longer up for the challenge
You will need to find another reliable USB cable for you to try. Try to charge your XSP to see if that particular USB cable is reliable for you. Try to wiggle the wire a little bit, moving the XSP from different directions and see if it would still charge. If it still charge, then this is the USB cable for you to use. Do the mentioned steps above again.
If you insist and want to use the same USB cable, you will need to find a "sweet spot" so that the connection between your XSP and the USB cable will remain reliable.
I was texting, using xiaomi.eu rom stable lastest, stock kernel, no root.
Gallery stopped, then camera, whatsapp closed, instagram gave black screen...
I turned the phone off and then turned it on, bootlop, so I went to TWRP and wiped all partitions and also formatted data. I also did a selinux fix contents or something like that, I flashed another recovery, a custom one via zip and then phone died, mi logo and rebooted, only fastboot available.
When I use fastboot and connect it to my pc it gets detected as kedacom adb automatically, I tried lots of drivers, uninstalling any driver and installing google drivers and lots of things, got ADB and fastboot 15s installed, I also tried different cable, other pc, 2.0 ports, 3.0 ports, other Windows, virtualbox, doing fastboot flash recovery twrp.img with waiting device and then connecting...everything I've seen and know. I bricked my phone and other phones more than one time, and managed with almost no issue to fix it, but now i'm just lost.
I get Press any key to shutdown, on the left top corner of the phone, I do the following:
fastboot devices(gets detected by fastboot)
fastboot flash recovery twrp.img(instantly I get press any key to shutdown)
I can't do anything, it's dead, any tips or something? Thanks guys.

THE FIX
It was very hard and tricky, but possible, you'll need a computer, usb data transfer cable, type c to usb, and a usb mouse, you'll need your device to be unlocked first, if it's not unlocked, I can't help you.
I used Tool All In One by Mauronofrio, installed drivers and with phone in fastboot connected it, if I press check, it will reboot and fail, you have to go to recovery flasher, if no recoveries are available, then download and put it on the same folder where the tool is. Go to recovery flasher(not zip one) select it, I recommend mauronofrio one, the lastest, and then select when going to flash flash and boot, it should be black screen with waiting devices and the phone with the press any key thing, then just disconnect the phone from the cable, restart it to fastboot and connect it, it should boot, you'll have mi logo or fastboot for a minute or 2 and then it will boot to twrp, but you may not have your touch working, just connect the type c to usb adapter and use a mouse, you should be able to use it, go to wipe, wipe all partitions, then format data and connect the phone back to computer, if it does get detected, you'll see the phone in file explorer, so you'll be able to move any files, move any rom to internal storage, use then the usb adapter and with the mouse, flash the rom, reboot and everything will work.
IF IT DOES NOT GET DETECTED, Go to recovery flasher(not zip one) select it, I recommend mauronofrio one, the lastest, and then select when going to flash flash and boot, it should be black screen with waiting devices and the phone with the press any key thing, then just disconnect the phone from the cable, restart it to fastboot and connect it, it should boot, you'll have mi logo or fastboot for a minute or 2 and then it will boot to twrp, but you may not have your touch working, just connect the type c to usb adapter and use a mouse, you should be able to use it, partitions allready wiped, so just put the rom file if it was deleted and flashbe sure that under partitions is MTP enabled, if it is and it still does not work, just go to reboot and select recovery, wait for it to boot to recovery. If it goes to fastboot and does not work, When booted, your twrp touch will be working again, if not, just flash another one.
There are other options, trying usb hub, 2.0 ports, a non amd pc, uninstall drivers and install google ones with internet disabled all time when flashing... But this is the one that worked, if anyone needs help, I can help you with Discord or pm.
