I have had several roms, over several devices, over several years. I have never had issues before.
I have Note 4 T-Mobile running 4.4.4. I rooted, installed TWRP and am trying to install cm-12.1-20150509-UNOFFICIAL-temasek-trltetmo
I tried installing a different ROM last night and the same things happen.
When I wipe Cache it removes the TWRP
I install TWRP app, then install TWRP 1.8.6 or whatever using the TWRP app
Reboot into recovery
Flash the ROM and Gapps
Wipe, wipe again just to be safe
Reboot
It goes through the CM logo to where it initializes apps, goes through them all, reboots and does it all again. 2 different ROMs did exactly the same thing. I dont know what im doing wrong. I tried installing TWRP With ADB but it hangs on "waiting for device" When I adb devices my hone shows and I have USB debug turned on. After I finally quit and pulled the battery It said no OS available so I had to DL and reflash stock image.
I am beyond frustrated now,
Any ideas? What info do you need from me?

You've got a nightmare. Head back to stock with Odin. Start fresh something with your base is off. Either wrong baseband orkernel but something off

I just flashed stock, used kies to update to latest and ill try re-rooting and re installing TWRP and doing it all again. this blows.

Which rom u trying
few thoughts.
First verify the MD5 of your download of the rom. make sure it wasn't corrupt. Second. Flash just the rom only and boot, then flash gapps and boot.

I think flashing just the ROM and then gapps did the trick. Seems to be up and stable. Man Ive never had so many issues before blah.

I have not messed with CM since I had the Note 2, which was until the Note 3 came out. Was a different carrier too. Generally the rule was flash CM then boot then reboot and flash GAPPS. I haven't looked but I would assume it is still the same.

I know there are a million posts about this, but I have been through them all trying to follow the guidance because the problems seemed to be the same as others have faced, but I feel differently now....
I unlocked my HTC one, following the htc dev site.
I installed TWRP (I cannot for the life of me get CWM to load into reovery, it only reboots the phone...)
loaded the roms/Super SU onto the SD card.
I know my phone is rooted (or i should assume?) because SuperSU is requiring me to grant access to apps and such.
I flash the custom ROM to the phone, install the package following Revolutions steps. Complete installation, return to fastboot menu. Flash boot.img, clear cache and reboot phone....Phone goes to the HTC Quietly Brilliant screen, with the development message at the bottom and continues to reboot over and over and over from there.
What did I do wrong?
I know pretty much have a phone that wont work because even after restoring it, it is ina boot loop after it fully boots up and nothing is loading correctly. Is there any way to start this from scratch?
Notes:
I tried doing an ADB sideload of Revolution HD.
I do a factory wipe before i load any rom....

I have tried multiple ROMS...all the same effect....
I also get the message after installing the ROM from TWRP that "your device is not rooted, swipe to install superSU" so sometimes i swipe it, sometimes i dont...either way same result.

I flashed a ROM called 42ONE....it ended up successfully booting past the HTC screen and is currently updating the android system...However, it will operate for about 2 minutes then the phone reboots like it was before when i tried to restore....what the heck?!?
There is no need to flash the boot.img in fastboot. Just flash the rom from twrp.

right, i only did it to ensure that wasnt the problem....however my new problem...I Have a ROM finally installed...but my phone is rebooting a minute after it starts. I am able to use the screen and everything but it just shuts down and starts back up on its own...

Download the rom again (might be a bad download), transfer to your sdcard, do a full wipe, and flash again. Something you flashed has created a conflict.
Edit: That's a Sprint ROM. Do you have a Sprint phone? If you've been flashing GSM roms, that's your problem there.

I do have a sprint phone. Trying to reflash the rom now.

Installed Cyanogen Mod, looks great boots up but stil reboots my phone. Not only that but I have no service either.

problem resolved so far. REflashed stock rom, relocked device. its working for now.
I am trying to flash the newest TWRP. First, I am using Slimbean 4.2.2 and trying to install using TWRP 2.5. First, turning on USB storage was an issue, as when I try to start USB storage, it forces a reboot of the phone. After the reboot, USB works fine. When I try to flash the new TWRP file, though, it just stops and restarts TWRP. I get the first couple of command lines and then the screen goes blank. Sometimes, it says install failed and sometimes it just restarts TWRP. Any ideas? Maybe I should wipe the phone and reinstall everything? Can I wipe and flash the TWRP first, reboot in 2.6 and then install the new rom? Thanks.

heres what i would do, first odin to stock with no root. then i would odin flash the latest CWM from CM11 thread then flash the latest TWRP from CM thread also and then i would recomend flashing newer roms like AOIP or Vanir or even CM11. Its my option . Feel free to PM if you need help

Ok. Now I'm really getting annoyed. I checked the md5 sum files on both the OP and the phone. They matched up, but install still failed. The error says twrp can't read the zip file. I am also noticing the every time I try to open USB connection the phone reboots. Once it's rebooted, it will go into USB connected mode, no problem. It's just the first time. Plus, when I reboot the phone after the failed install, it automatically updates android. I'm not really sure what that's all about, either.

Chronologically-speaking, here's what I did.
1. Rooted with appropriate Auto-Root file with Odin 3.09
2. Used Flashify to flash the latest TWRP files (2.8.7)
Most of the time, I just reboot onto "dm-verity verification failed." I've tried everything I can think of.
Sometimes, I get lucky and boot into TWRP. When I flash using TEKXodus 3.6 ROM, it proceeds to do everything but then, the screen goes black and I can never boot. I've read it can take 15 min for it to boot. I've waited 30 min with nothing. Only thing I could do is boot back into Download mode to reflash it with a stock ROM through Odin.
I don't understand why this doesn't work and I've flashed a bunch of phones before without any issue! Help? Anybody? UGH!

So i see your posting about your flashing but what about your wipes..
Nandroid backup ext sd.
Then reboot twrp.
Wipe sys/dal/cache/data..
Include the internal if after what i posted above doesnt work.
Check the md5 sum of your downloaded rom..
Nav to your ext sd were you should have the rom stored..
And flash the rom..
Curious as to why you used flashify to get twrp.. it comes in a tar and flashes easy in odin..
You might wanna try that as well twrp may not be installed 100% corectlly.

Last but not least you need to use bishock kern in your flash after the Rom flash, in the same session before reboot.
Or it will never work.

So I was running a custom rom for months now for my N910T without issue but yesterday the phone suddenly rebooted by itself while I was using it and would keep rebooting roughly every 3min.
I backed up everything in TWRP and wiped everything to reboot into a clean rom like factory reset but that didn't work as everything was still there and nothing was wiped. So I tried to flash a new rom but it failed and gave an error E: cannot execute upgrade binary.
I'm on TWRP 2.8.1.1 so I can't flash a TWRP update (have to be on 2.8.4 or above) and thought my recovery was too old. I then plugged phone in download mode and used ODIN to flash a new TWRP. It would write fail every time, no matter what I changed. I tried to install different versions of recovery and used different versions of ODIN with no success.
I also changed different cables and ports. I then tried to flash a stock rom through ODIN but it also says write fail. Basically I tried everything and searched on XDA and google and tried every method I came across. Anyone have this issue or suggestions?
tldr: Phone N910T reboots from home screen, write fail flash from ODIN and fail flash from TWRP recovery. Can't use phone because it would boot to home screen for a few min then just reboot again. Help!

Yes I tried every wipe option possible. Wipe system, boots right into rom. Wipe internal, everything still there. Wipe data, factory reset, everything still there.

Then something I'm your phones been corrupted probably, your going to have to rma for a new device
has anybody come up with a solution to this problem im having serious boot and loading issues. i have flashed many roms and have never had a problem. i odin flashed the cog2 firmware then twrp flashed the tekxodus rom. something happened changing permissions with the built in setting with the rom and twrp was lost and i cant get back to a usable phone. odin flashed everything i can find for my phone and its not working everything either falis or goes to loop.
You probably corrupted your partition doing something wrong
So I tried installing Xposed Framework by flashing it's ROM, but after I did it, I rebooted my phone, but every time I do it, it always boots into TWRP recovery. I tried fixing it by looking in the setting and I used a backup I made before doing all this crap and I restored, but my phone still boots only into TWRP recovery mode. I would really appreciate it if anyone could help me. Thanks.
PS. My phone is rooted and it has TWRP

Is it possible that you used the wrong Xposed for your device or SDK version? I can't imagine that restoring a backup didn't fix it. Did you try wiping system in TWRP first?
Things you can try:
- Flash the "Xposed Uninstaller" zip file in TWRP, which should always be on your device when you are flashing new versions of Xposed.
- Reflash TWRP in Odin. This should fix a recovery loop.
- Wipe system, data, dalvik, cache in TWRP, then try restoring your backup.
- As a last resort, you can download the latest stock firmware for your device from the sammobile site, flash it in Odin, let it boot once, reflash TWRP, wipe in TWRP and restore your backup (if you were running a custom rom you liked or something), then flash the correct Xposed for your device & android version.

Hello. I already tried zipping the uninstaller for xposed but it did nothing, I also tried wiping dalvik cache and all, but no luck. I also think that my backups are just not functional. Now I'm left to just reflashing TWRP and if that doesn't work I'll have to flash the original stock firmware for my phone. I have spent already 2 days with constant searching for ways to fix my phone and not being able to do other things because this has been taking up my time and my patience, so I really appreciate every bit of fixing my phone that I get. Since I am new to all this I would be very grateful if you could send me the links to the correct files for my device and also give me a tutorial on what to do. As I appreciate any support, so I'm thankful for you helping me But until then my device is still not able to boot regulary but rather boots to TWRP

Update:
I fixed my phone, and here's what I did, so if anyone is new here with the same issue I had, I hope this can help you. So first of all as I said I kept booting to TWRP, later I tried reflashing TWRP, but I remembered that I missed something in ODIN, so I just pulled the cord and my screen after rebooting just said something along the lines of "An unexpected error occurred while updating software, please recover using Samsung Smart Switch." after that, I tried Samsung Smart Switch, but it was no luck for me, so then I used the last resort. I went on sammobile.com, I searched stock firmware for the exact model I had and then I checked the stock firmware that was available. Since I'm from Latvia, I picked the Baltic region, then I downloaded the latest stock firmware. After a long time, I downloaded the file, and I checked the instructions on how to flash it, I flashed the stock firmware, and then my problem was fixed, my phone was working again and now I have Android 7.1.1 as I used to have Android 6.0.1 and I had ZERO file loss However as you flash the stock firmware, TWRP get "flashed" out and same with root if you have it, and in that case if you want TWRP and root again, I highly suggest Meziani's YoutTube channel, because he gives simple and working tutorials on how to do it I'm glad if this helped anyone.
