I'll need your help on this one as I've tried all solutions that came to my mind.
My XT1092 is stuck at the bootloader screen. I can access bootloader and execute fastboot commands but none of the bootloader command work (no recovery, nor reboot, whatever I choose the phone juste power off).
Before the brick I was running 5.0.0 and had just applied the small update (5.0.2 I think). The phone never woke up from this one.
I've tried to find stock images that I can flash via fastboot but I can't find any. (the only links I found lead to an inaccessible website).
I rely on you guys now...

I think I was in a similar position this morning. I think mine failed since I was rooted. Were you also rooted?
I got mine back up and running by flashing all the stock files (XT1092 5.0) via fastboot.
I then decided to switch my device to XT1095 and run 5.1, which seems to be working so far.
Yeah it was rooted.
The thing is I can't find stock 5.0.2 files for europe.
I managed to push a CM zip trough ADB, then flash it, then flash a stock brazilian ATT firmware via ADB, but I can't come back to my original version.
I also cannot start recovery unless I boot in fastboot mode, then launch "fastboot boot recovery.img" with a TWRP Recovery img.
I flashed back to 5.0, not 5.0.2. I'll find the link now (I'm on mobile).
As I said though, I decided to convert to XT1095 and try 5.1. Looks good so far, need to test 4G when I get into the city tomorrow.

I used a fastboot that was lying around from a while ago. It was fastboot-v2 I think. If you need it I can upload it tomorrow.

I had a mostly-stock Nexus 7 running 4.1.1 - unlocked, rooted, TWRP installed, but no major /system changes. When the update came in, I figured "what the heck, I'll just see if it applies - worst case I lose root and my recovery console, but it's a nexus, it's easy to get those back". I applied the update, and it worked! It applied and rebooted, worked fine - but it did lose root and recovery as I expected.
So I tried to get recovery back. It won't come back. Here are the ways I tried:

Booting into AndroidOS reflashes the recovery console with the stock console (I tried to race it running adb reboot recovery, but lost).
I tore my hair out for hours. Nothing I tried would let me boot into a custom recovery. I eventually backed everything up as best as I could using adb (Since luckily Android OS still worked), and reflashed to stock everything. PROBLEM IS STILL HERE! Then I decide to revert the bootloader from 3.41 to 3.34. SUCCESS. 3.34 booted to recovery without any problems at all.
Any idea what the heck is going on here? Anybody else seen this? All the threads I've seen with people having problems booting into recovery end with somebody suggesting "Just get app xyz that will reboot into recovery" - but that is NOT an option if A). you cannot get it to boot or B). you are not rooted/are running stock, because you can't be in AndroidOS and still have the custom recovery flashed UNTIL you are able to get into recovery to actually fix it.

I brought up the same concerns when I did a manual flash to 4.1.2 and went through a mess eventually reverting to 4.1.1, using OTA rootkeeper than updating again because on bootloader 3.41 *NO* custom recoveries successfully boot.
how do you flash the old bootloader? if you don't mind me asking.
I have been on boot loader 3.41 for a few days and no issues with twrp. I have flashed a few things since and all is good? Sorry not sure why its not working for some.

Huh? I have 3.41 and can go from bootloader to recovery with no problems. Even got a video of it if you don't believe me

I update to 4.1.2 using CWM, didn't lose root. Then changed back to TWRP 2.2.2.0 with bootloader 3.41, now I just updated to TWRP 2.2.3. Everything went smooth even though I'm android noobie.

I think the issue comes from going back to stock recovery without a rooted AndroidOS. Either one by itself seems to be fine. If you still have the CWR or TWRP recovery already flashed, 3.41 can boot to that. If you have root in AndroidOS, you can always re-flash recovery from ROM Manager or something, and that will work fine. And you can boot to recovery from Android OS (so long as you have root).
If you have stock recovery and no root, your only option is fastboot. And that fastboot flash and boot commands do not work. They look like they work - but they don't. So if you already HAVE a custom recovery, it probably won't even break it.
If you really want to do a proper test / prove this, go back to stock recovery with 3.41. Turn the tablet off completely. Boot up holding the three buttons and go into recovery to see that it can go into the stock recovery console. Then reboot with the three buttons again (don't boot AndroidOS), and run the fastboot boot command to run a custom recovery. If that works, I'll eat my hat.
(well, really all that means is that the problem isn't occurring on all systems. Doesn't mean it's not happening to me and many others)
The problem might not be that 3.41 can't boot to recovery -it might just be that 3.41 can't FLASH or BOOT a recovery console through fastboot. Many people will never notice that because:
A). They already have a recovery console flashed, so it still works
B). They flash their recovery from ROM Manager, GooManager, or whatever
If you lose root, you no longer have B as an option. If you also lose your recovery console, you can't get root access back without fastboot, so you have to use fastboot.

Not sure you've got it right. At least on my N7. My first try was flashing the update.zip in another thread without updating my ROM at all. Running Paranoid Android (2.1799 then) and TWRP. Not sure when in the mix I updated TWRP though.
Thought it might have been a problem with the update.zip, so downloaded the full update.zip from google and tried installing bootloader*.img from that via fastboot.
No change. Reverted to 3.34 without any playing around with deleting files and all is good.

Found the solution to the the issue (sorta), I too went through all this trouble as you did in your OP.
I did not have to do anything other then straight up flash this version "recovery-clockwork-touch-6.0.1.5.b8147-grouper.img" via fastboot and bam! recovery works. Link - http://forum.xda-developers.com/showpost.php?p=32810167&postcount=228
Problem is with previous versions of 6.0.1.0 (Official) and TWRP 2.3.1
I still prefer TWRP, but hey This is better then the alternative (No recovery at all)
Now we need to figure out what changed between CWM 6.0.1.0 and 6.0.1.5 and get the other guys (TWRP) to fix their **** for us poor saps that seem to be a minority in the Nexus 7 world having this issue.
I would love to know what actually the fix is and why only some have these issues with older versions of CWM and TWRP.

First, specifications:
I have an HTC One M7 (AT&T) that had been updated to 4.3 OTA, then unlocked via htcdev, flashed with CWM 6.0.4.3 recovery, and then rooted with the Superuser zip on the CWM site. The device remains S-On.
Now, for the detailed inquiry:
I've been in the Stone Ages (see, Jelly Bean) for a while and have been planning on updating my phone for some time. However, the easy way, i.e. OTA, requires that I return to stock recovery. Now, as far as I know, this would require the use of an RUU (I'm not quite sure which one - perhaps "RUU_M7_UL_JB_50_Cingular_US_1.26.502.12_Radio_4A.14.3250.13_10.33.1150.01_release_318450_signed_2.exe" from AndroidRUU?) which would require that I lock my bootloader (correct?) which would also happen to wipe my device. I was hoping that I might be able to flash the stock recovery image (which I have been unsuccessful thus far in finding on my friend Google, since I assume it was deleted from my phone when I flashed CWM?) without having to go through an RUU. Is such a thing feasible? Where would I find the appropriate stock recovery image file to do so? And is there a way, once I (hopefully) receive the OTA update, to copy that new stock recovery image to my PC before I switch back to custom recovery to avoid bothering all you fine folks about this again?

You can install stock recovery using fastboot command.
However to apply OTA, if your phone is S-On, you must lock the bootloader.
You don't have to lock bootloader if your phone is Soff, for OTA, just stock recovery is good enough.

Just flashing the stock recovery will probably not allow you to successfully install OTAs. You have to have stock rom, stock recovery and possibly some /preload/ files that were deleted when you unlocked your bootloader. The quickest way to stock for OTA would be to flash a RUU. I would propose this route:
Option A
Lock your bootloader
Flash RUU 3.17.502.3
Receive OTA updates if they are still available
Option B
Lock your bootloader
Flash in bootloader: firmware 4.18.502.7
Flash RUU 4.18.502.7
Flash in bootloader: firmware 5.12.502.2
Flash TWRP 2.6.3.3
Download Guru Reset 5.12.502.2 and place on your sdcard
Boot into TWRP and install GURU Reset
For complete stock Say no to installing Root when exiting TWRP and Flash the Stock Recovery
Thanks for the feedback. I was a bit surprised to find out about needing to re-lock the bootloader even with stock recovery since a bunch of other places I looked said that I could use an unlocked bootloader as long as everything else was stock (just couldn't find a way to return to stock recovery without an RUU - which would require re-locking the bootloader - at the time). Used this (http://forum.xda-developers.com/showthread.php?t=2545227) from Majmoz's signature and got the relevant recovery.img from it. Since I had it, I figured I might as well try it before going through the process of re-locking the bootloader since being bounced out of the OTA and then going through the full process of relock-RUU-OTA anyhow was the worst that could happen. Turns out it worked.
So, for posterity's sake in case anyone else should come across a similar issue, everything I did (most of it probably unnecessary, but who knows):
-Made CWM backup (in case I happened to need to re-lock the bootloader which would wipe the device data to factory)
-Pulled that backup to my PC in case flashing to stock recovery wiped it (adb pull [backup directory, e.g. /mnt/shell/emulated/clockworkmod/backup/[folder]])
-Went into recovery from the bootloader, fastboot, plugged in the usb, and flashed the stock recovery (fastboot flash recovery [the downloaded stock recovery .img file in the platform-tools directory])
-Went back to the bootloader menu and entered the stock recovery and rebooted the device from there (I recall having to do that when I flashed CWM for the recovery to "stick", not sure if it was necessary here)
-Went into AT&T Software Update, let it download off the Wi-Fi, and let it apply. Did its thing with the image of the green downward arrow within the circulating arrows followed by reboot a few times, the "Android Update" screen where it runs through your applications, and voila, I am now on 4.4.2.
Updates:
-Have downloaded the updated stock recovery image.
-Flashed CWM Recovery back onto device
-Re-rooted via CWM (confirmed root)
-Made a new backup through CWM.
Now, if only I could figure out what the issue with the LTE signal is since I've never fiddled with that - guess I can heckle AT&T tech support for that.

Flashing a recovery will not wipe anything but its a good call to backup on your computer :good:

no but again its a good practice to check if the recovery is working correctly.

Have you checked you preferred network setting (GSM/CMDA/LTE AUTO)?
Is your apn correctly configured?

Good to know: the backup directory actually showed as empty while I still had stock, and then each backup's folder reappeared when I put CWM back on - was patting myself on the back for a few minutes there.

It worked! Used *#*#4636#*# to pull that up (didn't know one could do that!) and, sure enough, it was on "WCDMA Preferred" for some reason. Switched it back to "GSM/CDMA/LTE AUTO" and my long-lost signal has returned.

I just got my motorola yesterday. I did quite a few things like unlocking it and rooting it..After my first initial root it worked fine but earlier today i decided to install lollipop. I was using the twrp version 2.7 to install the rom but i realised it wasnt working till i tried a few more time and it finally it. I tried back up my stock 4.4.4 room before i installed my new rom but it failed several times because i think the recovery was faulty to say the least. Anyways my lollipop worked very fine but i notice anytime i restarted the phone it took a long freaking time to restart but it was working normal as usual until i restarted my phone another time and realised i couldn't go past the bootanimation.It takes forever but i never get past it. I then decided to flash the twrp 2.8.0.1. After flashing i realised I could do a backup but it was too late. I tried a few tricks I've always used to get past the bootanimation like mounting the storage but it never shows up on my pc. I also tried side loading the rom using the adb sideload command but it never workedd.it always shows up saying "error: device not found" but when i go back to the bootloader and use the fastbootdevices command it shows up my device. IS there any recovery that will allow me to sucessfully mount my storage or sideload my rom without all this hussle?

Tried rooting my phone with no prior experience. Worked, but eventually I got to where the phone wouldn't boot past splash screen (logo came up, then screen went black while backlight is on and didn't respond to stimuli until battery was removed). Possibly resulted from a build.prop file problem. Did get into boot though, so I fixed that problem by installing a cyanogen rom from a zip file using twrp recovery. I didn't want a custom mod though since I'm used to the look and feel of the 4.0.3 ICS I had, and the cyanogen doesn't seem to have google play so I can't even download anything (original purpose of rooting was to get incompatible apps by spoofing via build.prop file). Currently trying to get back to stock. Unfortunately I didn't make a nandroid backup of the system before the problem occurred, hence I have no backup to flash.
What I tried:
1. Use the official HTC 4.0.3 RUU .exe to upload to the phone. Unfortunately HTC Sync doesn't see this phone, even though I have the correct drivers installed.
2. Attempted to use twrp recovery to install the rom.zip extracted from the RUU directly, the same way I successfully installed the cyanogenmod rom. Failed, claiming it's unable to open zip. The .zip file was checked for integrity and returned by an archiver app.
3. Tried to use these commands from the cmd with phone connected to install the .zip from fastboot:
fastboot erase cache
fastboot oem rebootRuu
fastboot flash zip rom.zip (rom.zip being the functional file of the stock rom)
Failed, saying it's not allowed. Tried it with both locked and unlocked bootloader, no difference.
4. Unzipped the file and tried to flash the system.img file directly, but that failed with a "data length is too large" error.
5. My recovery being twrp 2.1.1, tried looking for altenatives. Can't find any links to download the WCX recovery, but I found several for newer twrp versions, including 2.3.1.0 and 2.6.3.0 which I tried downloading flashing. After flashing the phone got stuck at the cyanogenmod loading logo and wouldn't boot further, and the recovery itself didn't work, just showed the HTC splash, turned black, then booted again normally, getting stuck at the cyanogen logo. Same thing for the 2.6.3.0 version, so I reflashed the original 2.1.1, at which point I can get into recovery but the phone is still stuck at the cyanogenmod during normal boot. At this point I installed the original cyanogenmod .zip from recovery and now at least the phone boots up normally. Still need to get stock back somehow.
Out of ideas and exhausted from this. Any help would be appreciated. Perhaps anyone has a working stock 4.0.3 rom for the Vivid, nandroid backup, etc., or any other ideas. Thanks for reading.

OK... First... Stock rom can be found in development... Most of the roms there are stock... Read the OP... It'll tell you...
Second... You should use TWRP 2.5.3.0 or something like that I forget...
Third... When you tried the ruu... Did you relock your bootloader...
Side note... Could you get your phone to show up with 'fastboot devices'

I was on 6.0 on XT1095. Tried to root it. Screwed up by flashing something that didn't work. Now i am stuck on 'warning bootloader unlocked' screen. Previously when I was on 5.1 I restored my phone from bootloop by using official images from Moto and using their instructions. Now there are no 6.0 images. I guess trying to reflash the whole 5.1 ijmages would brick my phone. I tried flashing just the system image of 5.1. It didn't work.
I found this 6.0 stock ROM here on XDA http://forum.xda-developers.com/moto-x-2014/development/rom-xt1095-flashable-stock-rom-t3277874 . How do I flash this?
Please help. I'm on stock recovery. Previously I ran commands like 'fastboot flash system x.img' and so on when phone was in fastboot mode. How to flash this zip now.

A bootloop is simple to get out of. Just press and hold the power and volume down keys to boot into recovery and go from there. If your phone isn't booting past the unlocked bootloader warning splash then you need to elaborate on WTF you flashed. Just saying "something that didn't work" isn't sufficient to make an accurate diagnosis.
To flash that stock ROM you posted a link to above would require you to fastboot flash a custom recovery first.
As for restoring a 5.1 image I don't understand why you'd think that would brick your device. You're formatting the entire device by going back to ANY stock image. Just because you are on 6.0 doesn't mean you have to restore a 6.0 stock image (that probably won't be available for some time).
