With the aid of some very fine research by JVDE, I put this together from a handful of parts and a 8x2cm board. Functions from left to right are (skip-volUp-voice-volDwn-prev). Works great with Android Auto!

The schematics are available at the link I included in my OP. As for the physical layout, I'd have to rip it apart to show it; I'll be the first to admit it ain't all that pretty.
I would have preferred to use a laddered approach, but there just wasn't enough room on the 2x8 board that I used, especially considering the area the switches occupy. Plus I didn't have discreet resistors for each value so had to use a couple in series for a number of them.

Rubber screw caps and a better usb attachment method and you have a winner, I would also powder coat the piece black and then highlight the diamonds with red. But thats just me, oh, and maybe round the corners a bit so you dont end up in the hospital reaching for your phone
LMFAO.....oh man.....I needed that considering the type of morning I am having. I deffinitely give you an "A" for effort. After seeing this, I want to now make my own custom dock. Seeing that Verizon will not be selling one we really don't have much of a choice. GREAT JOB!!

Custom Gelaskin

Has anyone done a custom skin for their incredible from Gelaskins.com. I recently got a groupon deal for $30 in Gift cards for $5 so I'm going to go the custom route and upload images to try to make a really nice skin image that matches the contours of the phone. I was just wondering if anyone else has tried this.

I did, it was a nice little (somewhat) protective addition.
Likewise, though a fair bit of warning: they cover the light/proximity sensor. You'll have to carefully cut out a small square with an exacto knife to regain that bit of functionality.

I have a real similar item I used to use on my iPhone, the Streak is too heavy and it gets wobbly. The way the handle looks bent on the photo of this one makes it look like it might have similar issues.
Need something like a rear mirror hanger with a slide-in that contain the streak on both sides and the bottom

A great tool that I don't see a lot of people use is the magnetic placemat. If you're unfamiliar with what this is, it's a magnetic placemat. Uh... I guess that's pretty self explanatory. You can pick one up for only $20 from ifixit and you'll never have to worry about screws going missing again.
I've used one of these in the past and it really does make disassembly much easier. You don't realize how much time you spend fumbling around with screws until you use one of these.

I think the metal bar is the speaker cone and it's mounted on something rubber/plastic that flexes. There could be a voice coil type thing in there or it could be piezo- electric like those annoying little beepers.
Those look like blocked. Are you sure are not fake?
I don't think so. Most cell phones seem to have them, some have tiny holes in them.
I suppose I could try a stethoscope on them, or make something like it with a microphone and headphones. By a weird fluke I have a real medical stethoscope in the other room. Mechanics use them too.
