Nexus 6 Boot Failed - Nexus 6 Q&A, Help & Troubleshooting

So I recently decided to flash a rom based on the newest android m preview. Everything flashed fine, no errors, no 'failed' messages. However, when I try to reboot my phone it immediately sends me to the boot loader with a Boot Failed message. Furthermore, at the top of the log it says "fastboot fall through from normal boot mode". I have never experienced this code before and could not find an answer on the forums. I was really hoping I could get a solution as this is my primary phone.
Thanks guys.

Did you flash the M bootloader?

Yep. Boot loader and radio. My guess is something wonky happened with the damn toolkit. Probably what I get for being lazy. I'm gonna try manually reflashing all of it via adb and see how it goes.

Not ADB. Fastboot flash while in bootloader.

Yeah, fastboot. My bad. I actually just got done with attempt # god knows how many and so far no luck. I still get the same messages on reboot.

[Q] Can't boot to recovery...

Hi guys, hoping one you can help.
I recently had my unlocked/rooted HTC One replaced after it completely died on me. Almost the exact same thing as what happen to this guy http://forum.xda-developers.com/showthread.php?t=2295374
Anyway, I went to unlock/install custom recovery on my new One. Done the unlock process fine, I've always used ClockWorkMod but I decided I'd flash TWRP to try. Files were sent/written ok and everythig seemed fine. But when I try to boot into recovery it shows the "Entering Recovery" screen (with the red warning text at the bottom) for a split second, then it just reboots the phone... So I tried Flashing CWM instead, again process went as normal but still can't boot to recovery. Even flashed the stock recovery and still the same. Tried flashing CWM through Rom Manager, just gives me "An error occurred while flashing your recovery".
It's a 3uk phone and their is no RUU for it yet that I've seen, so I thought if I can get S-Off I can change CID and use a generic European RUU. This when I realised it's not rooted and as I can't get to recovery I can't root it that way. Tried using the "Perm Root" option in the All-In-One kit but that just throws errors back.
Also the TAMPERED doesn't show above UNLOCKED for me in bootloader.
Any ideas? I stumped
Thanks.
Anyone?

Yeah I had done that, before and after flashing.
Funny thing is this morning after a full charge (wasn't like it was flat last night, had at least 50%) it worked fine, fired up the bootloader and the TAMPERED had appeared and now it loads the recovery perfect... :/ Just rooted it and now I'm off to S-Off and super CID.
Very strange but it's working now so I'm not complaining.
Thanks anyway guys.
Hi I have the Developer edition 64Gb and I have this problem too...
Aparently the TWRP flash went well (I tried both ways, AIl in one kit and fastbbot ways) but I cannot get into recovery.
I erased cache and reflashed again but nope.
Since this phone comes unlocked I assumed that I can just skip those steps of unlocking it (am I correct there?) and went straight into flashing recovery but it does not seem to have worked.
Anybody has a different suggestion?
Thanks,
EDIT:
Never mind, I just waited few hours and tried again and it works
???

[q] please help!

Okay, so I decided to screw around with my HTC One again, and ended up trying the new Cyanogenmod installer. Needless to say, I have screwed my phone up worse than ever and I have no idea how to get it back. It is stuck in bootloader, I can't go into recovery or reboot or even power off. It just keeps rebooting to bootloader. I tried the RUU, but it comes back with an error that the image version isn't correct, and I tried flashing another recovery, but i still can't get into recovery. Right now, I am sitting in my bootloader with nothing to boot into. I can use ADB however, just please help me.

Try, "fastboot erase cache" and then try to go into recovery.

[RESOLVED] Unlocked XT1058; Bootloop & Failed System Flash

Long story short: Phone won't boot past "Unlocked Bootloader" warning; Recoveries won't stick; Unknown Error & Freezing when trying to Flash System.img
I own an ATT Locked Moto X, but went ahead and unlocked it by purchasing the oem code. No issues there.
I decided to go ahead and try rooting it too, and was able to flash TWRP as my recovery. Still, everything seemed to be looking good, until I tried hard rebooting (to see if root would last). Upon rebooting, I could not get past the warning sign, notifying me that the bootloader was unlocked. I waited, and waited, and restarted again, but all progress stopped there. I would be the first to admit that whatever happened was probably my fault; perhaps I flashed some files in the wrong order, or entered a wrong command into fastboot. I don't know, and frankly don't care. My only question is how to get out of this mess and back to rooting, or at the very least, back to stock ROM now that I'm unlocked.
Booting into recovery only leads to the red triangle over an open android mascot, with the "Android Recovery" title. So clearly TWRP didn't stick; I tried reflashing, but nothing lasts past reboot. Then I tried flashing Philz Touch instead, which, upon exit, notified me that root was not enabled. So I rooted, rebooted, and was finally able to see Philz Tough again, even though I still can't boot normally. That time, both Philz Touch as well as root seemed to have stuck.
With that minor victory I downloaded the latest firmware for my model and tried to flash the components over, thinking I had somehow corrupted the existing OS files, but here's where it gets particularly frustrating. I am able to flash every file except for system.img, even though I am using moto-fastboot, which I understand is a common mistake. I get no specific error code, simply "Unknown Error," upon which the phone is frozen in fastboot. The phone remains unresponsive until I hard reboot [back into fastboot], and see that, once again, Recovery is back to stock. So now I'm baffled. I can't seem to find a way to flash the system file, factory resetting doesn't work, and my recoveries are constantly reverted back to stock. Does anyone have any advice, or suggestions for what I could try next?
You try RSD lite?

I was already downloading it just as I saw your post, as my last-ditch effort. It also failed, but it gave me "Unknown USB Error," which is all I needed, it seems. I tried a different USB slot on another side of the computer, and lo-and-behold, the flash finished.
I'm not sure what saddens me more, the fact that I pushed off making this post for hours, only to have the solution less than an hour after I do, or the fact that I never thought to try a different USB port... In any case, sorry for the uselessness of this post. I think I'll save rooting for another day though...

Moto X Verizon Dev Edition TWRP, boot loop

I have Moto X Verizon Dev eition on 4.4. unlocked bootloader, Installed TWRP few weeks back with SuperSU for root. Worked perfect.
4.4.2 upgrade messages kept coming up, but I just ignored them by hitting back button.
This morning, while I was typing something and I was about to hit send, that message popped up and instead of hitting send, i hit INSTALL NOW. no warning, no confirmation, it said "power off" and started process of updating.
But the problem is that I still have twrp and is rooted, so everytime it reboots it goes to Team Win Recovery page and if I reboot from there, it goes back to normal boot up but then reboots automatically in 15 secs and goes on in an infinite loop.
I cannot start/stop anything when it boots up normally because I have literally 15 secs before it reboots automatically.
Any idea? On teamwin reocovery page, should I goto wipe>> factory reset? and then upgrade and so the whole TWRP and SuperSu again?
and of course, i am to stupid to have no backups made

turn phone off. boot into bootloader/fastboot mode. run "fastboot erase cache". your reboots will stop. then use tibu to freeze motorola ota and you wont get ota things pop up anymore.

Awesome. gorked like a charm.
just a note for someone else trying this.... first time i booted up after doing a "fastboot clear cache", i got a warning saying failed to load info. I clicked okay and I would get no service. Rebooted normally again, and everything worked fine.
Thanks again fro the response.
So are you now on 4.4.2?

Nope. I rolled back. seems like I will have to roll back the root and then install 4.2.2 and then root again or something. Too much mess to go to 4.2.2. There are no specific and detailed directions for vzw dev edition rooted phone that I could find, so would leave it alone for now.

If you're stock rooted, you just need to flash the stock recovery (available from the moto dev site) and make sure you don't have any missing or modified system files. You can then just take the OTA like normal, then reflash TWRP after.

There are a lot of threads discussing this.
If you have a Dev Edition X with an unlocked bootloader, as long as you have stock recovery on the phone, and are close enough to stock, you can just take the 4.4.2 OTA and re-root using the same TWRP and SuperSU process. (more details -> http://mark.cdmaforums.com/MotoX-OTA.html which is based on many threads here on XDA)
Downgrading to 4.2.2, etc.. while rooting was for locked bootloaders only.

I'm fairly positive this will fix my problem since I'm experiencing the exact same thing as the OP.
However, I'm still new to bootloaders and rooting, etc...
Can someone breakdown what jayboyyyy is saying here? I can get into my TWRP bootloader. How do I run "fastboot erase cache"? Do I use the built in terminal command? Do I need to plug into my computer? Do I need to point my cmd to the TWRP I originally downloaded and installed on my phone?
I really do not want to mess this up. Like I said, I'm sure this is the fix for my phone. Thanks in advance!
TWRP is recovery, not bootloader / fastboot
You will need to plug into your computer, you should probably read more before you break things worse tho.

TWRP is a RECOVERY, not bootloader.
Do you remember how you flashed TWRP on your phone? You booted the phone into Fastboot/Bootloader mode. Connected the phone to PC via USB cable, then from command prompt on the PC used the Fastboot or mFastboot command... Same thing. However in this case the command to clear your phone's cache is... mfastboot erase cache
The other option is, you can see if there is an "erase cache" menu option while in TWRP as that would work too.
Then to take an OTA, you need to flash stock recovery back on your phone. If you try to accept an OTA while TWRP or other custom recovery is on your phone, you will get stuck in a boot loop like this.

ONE PLUS 3T - OOS 4.1.3 stucked in bootloop after flashing SuperSu

I have update my rooted OP 3T by flashing the complete rom through TWRP.. But the problem is whenever I flash the Supersu version : SuperSU-v2.79-201612051815 or SR3-SuperSU-v2.79-SR3-20170114223742, I am getting bootloop.
Please anyone advise me.
I checked that without flashing SuperSU phone is able to Boot safely.
Thank you in advance.
It's normal if it bootloops once or twice
But if it's more than that then it's problematic
So which one is it
Another thing you can try is try magisk root instead of supersu and see if gets the same result
Phone was stucked in bootloop for an hour without any reboot.
I am not fond of magisk yet... So dont have any plan for it.
Just want to know if this happens to only me or there are many others with same issue.
Edit:
Finally installed Magisk for root... Issue closed.

That is not the proper way to unroot (go back to stock). Relocking the bootloader while rooted was asking for trouble. There is a guide on how to return to stock in the Guides section.
Also, fastboot commands will only work while in fastboot mode, if the phone is looping, or otherwise stuck on any other screen/mode, than fastboot commands will not work. It's also a bit irrelevant, as your ability to flash anything with a locked bootloader is very limited. That's the whole purpose of a locked bootloader.
I'd start by making sure you are in fastboot, get fastboot working, and unlock the bootloader again. Then follow the proper process to return to stock.

can u tell me the command that i can try on an phone with a locked bootloader
I forgot this is an edit the first time Gentlemen, I am absolutely at wit's end now I am no geek with a capital G however I have been modifying phones for some 3 years now and I can't even tell you how many times I flashed ROMs my beloved 3T 3T has had been acting glitchy lately like when I would reboot the circle that turns - instead of just turning evenly it would twitch, it was such a small thing I ignored it but I think now that that's must be have been indication something was not right few other things like that. I flashed the Freedom ROM cuz I'm so sick a Google and Google Assistant when I came out I was in a bootloop so I wiped everything except OTG everything and formatted data did the ROM Flash again the a boot Loop still abides and I've done it a dozen times now sometimes a couple of times it it just it kept recycling the bootloop another time and got hung up on the OnePlus it just sat there on the logo and if I miss something obvious I do apologize I'm tired and I'm at wits end because I'm not used to not being able to solve my own problems I guess so any help at all or just even a heading in the right direction would be so much appreciated that I don't even know how to call it quantify it thank you
By the way I seldom post or even post questions but when I do I always pretty much say the same thing you guys are the best this is the one place I can come for Andrew Android advice or or guidance and I just I absolutely trust what I read so thank you thank you thank you thanks for being there
Edit - I'm so tired I forgot the first time I went to reflash the ROM it told me that my 3T was in fact not a 3T that it was a three and I read someplace so I'm XDA that that's just Corruption of the install script so I downloaded it again I ran 7-Zip it said it was coherent so I flash it again if that's important but thanks again
