Ok so I've posted this elsewhere and there was no resolution, but I have new info.
So I know there are several others experiencing this problem. Today I flashed every one of the current 4.2.2 roms. I figured I'd install my apps a few at a time to try to determine which app was causing the internal SD to stop mounting. Strangely though when I would boot into the ROM for the first time it did not mount. So I went back farther and only installed the ROM zip and no gapps. Bam it mounted. Rebooted several times, both into recovery and into the system. Each time it mounted. Then I would get excited and install the gapps. Bam no internal SD mount. I tried all the current gapps on here and the insomnia forums. I even downloaded the 03/01 gapps from goo .im.
So obviously this is not all conclusive, but I flashed and wiped and flashed about 25-30 times today trying to figure this out. I confirmed this in my mind for the latest BBB and linereo 10.1.
Those of you wondering I used cwm and tw(the awesome custom insomnia version). My procedure is factory reset/ROM wipe, then I wipe, system, cache, dav cache, internal SD, and Android secure...2 times each. Then I reinstall the ROM.
After install fixing permissions does not fix the problem either.
Also I'm not really expecting any developers to work on solving this since it is also caused by the goo gapps, just thought it might help some of us struggling with this problem.
Swap sd is currently solving my problem, it mounts the internal as external, but it mounts.

I don't think it's really a ROM issue as much as a compatibility issue with some hardware variance with some exhibits. I know we've had other issues that only some people have and no amount of wiping and flashing fixed those.
I bet if we opened up my exhibit, which is only 4 months old, and say camcory or tims we could find differences when we compared them component by component...I believe this is the reason for the varied compatibility.

This solution is easier than replacing the system board...
Connect the phone to your computer via USB (this works on tablets with USB as well). When it pops up to ask you to turn on USB storage for the computer, do it. Give it a minute or two to mount the SD and one internal user partition to the computer desktop, then unmount/eject the drives from the computer and turn off USB storage. The other internal partition will be mounted, and will remain so until the next phone reboot.
I've hooked my T679 to both my Hackintosh laptop and my TF101 in this manner, and it works every time.
Thank you red_five_1976 so much for your help. I followed your instruction and voila, it worked !! I like many others, am scratching my head on how exactly to get my external micro SD card to work after installing Android 4.2.2, you method worked for me and I'm a happy man !!

I've also found that if it didn't mount on a reboot, if I reboot to recovery and wipe davlic and cache, then reboot back into ROM that solves the problem.
Btw I'm using the 6/3 AIOP at the moment, best this phone has ever been in every way.

Note: This is a long one, theres a tl;dr at the bottom of the page.
So, yesterday I bought the XDA Premium app from the market. I installed it, used it for a few hours, rebooted my phone, tried to open it back up, "Failed to load activity" error or something along those lines.
So then I looked it up on the market, it wasn't recognizing that it was installed, so I reinstalled it again. Used for a few hours. Rebooted my phone, same issue. This has happened a few times.
So I figured maybe some data was corrupt. So I installed it from the market, uninstalled it, tried to reinstall it again and it said not enough storage space on my phone. I looked and I have 2.77GB free space on internal storage.
So I figured I'd move some apps to the SD card anyways. Started moving all apps from my phone to SD using the App2SD app (Which I think just redirects to the application's page in settings). Everything seemed to work fine.
Rebooted my phone, and some icons on my screens in LauncherPro didn't load. Tried opening several of them.. all of them "failed to load activity". I'm talking about at least a dozen different programs here that had somehow gotten corrupted? Rebooted again, they didn't come back. Reinstalled a few of them and rebooted and one still wouldn't open.
So I'm restoring from a CWM backup I did just before I went to bed last night. Any opinions on what might have gone wrong or what I could do differently? Is 2.77gb reserved space for texts/cache/etc or was the "not enough storage" error incorrect?
tl;dr: xda premium was corrupting/wouldn't install because I *only* had 2.77gb free internal storage? Tried moving things with app2sd and many, but not all, corrupted after a reboot. Rebooted a second time, didn't solve any issues.
Don't use apps2sd with this phone. Go to settings, applications, media area, click on everything with a check mark, move to phone. This will solve your problem.

So, as I mentioned above, I performed a recovery using an image I had created in CWM late last night.
Just for kicks I tried moving a single app to SD to see what happens.. Google+ went from 28mb on internal memory, to 8mb on SD. Restarted, it was corrupted and not even showing up in application list.
So whats the deal with our phones? They can't move anything to SD card without corrupting? Hopefully this is patched as I appear to be hitting the artificial 2.77gb cap on apps?
So.. my experiment with JUST moving Google+ to SD failed more miserably than I originally thought. Not only did G+ corrupt as I mentioned above, but my phone also started freezing again and VNC Viewer also corrupted itself.
So I had to perform a second recovery on my phone this morning. Ugh. Having way too many issues with this phone considering I only bought it maybe 2 weeks ago.
One more update.. hope someone can help me with this.
So I performed a recovery with CWM. Since then I have not done ANY App2SD attempts or anything. This is a recovery from before I ever did any of that.
Now, whenever I uninstall any app, when I reboot my phone several apps will corrupt. These are apps that I had used App2SD on before I had done the recovery. Its like, when I uninstall an app it finds some data from prior to the recovery and that corrupts the app...
Any way to fix this? I looked at my SD card and the only data on there is Titanium Backup data, CWM backup data, and a few zip files that I had put on there. I told ES File Manager to show hidden files and there is nothing on my SD that I see that could be making Android corrupt these files...
Any help would be greatly appreciated.. I'm starting to thing I'm going to have to do a fastboot recovery to completely stock and start over again..
If you restored a recovery, wasn't that recovery with apps2sd as well? Does that make sense?

Here, I'll lay it out a bit more clearly:
1) Midnight last night I performed a backup with CWM. Everything was working fine, I hadn't done any changes or App2SD things yet.
2) Woke up at 10AM today and this was when all of the issues covered in this thread started. After 10AM today was when I first attempted to use App2SD Pro to move apps around.
3) I recovered using the recovery from part 1 of this message
4) I'm still having issues with programs corrupting even though I'm using an image from Midnight yesterday.
How can I completely wipe my phone then restore with Fastboot? Wipe SD, internal, everything.

So it appears I fixed my problem.. even though I have run two complete restores using a backup from prior to my issues, some apps were still set to go on my SD card. So I guess a CWM backup doesn't store all file system data.

Also some apps try to load on SD card watch out amazon apps. I've put 300plus sized various apps on my phone and didn't crack 2gb. And really 200apps or more your not going to have time for them.

Yea, the XDA Developers app tries to load on the SD card by default. That was the entire root problem that caused all the above posts...
Hopefully Motorola fixes this SD card issue in the next patch.
Hey guys, is there a way to supreme wipe internal SD card, so it is like I just bought the phone.. After dozens of different ROM's my internal SD is full of empry folders, some pieces of files and God knows what else.. There was a tool for G2x called "G2X Cleanup Batch" - http://www.xda-developers.com/android/remove-old-clutter-with-batch-cleanup-for-g2x/ Is there anything like that for O2x? Thanks in advance!

I just plug my phone into my computer, do select all, then delete, then empty recycle bin to wipe everything from the sd card.
You want go back to stock rom, cleaning everything? (deleting cwm too?)
I had to do it yesterday, I did something that my cellphone couldn't mount E: and stuck on CWM.

Well not external SD, but internal - phone memory.. Kinda risky to format phone without knowledge, that this wont brick my phone..

the fastest way is to unmount internal from menu and just format it but you will not have preinstalled test apps that lg provides(not a big lose they were all demos)

Do you mean you want to format the internal 8gb SD card, or do you want to wipe the 1gb internal storage?
If it's just the internal 8gb SD you can do as I posted above, i've done it a few times. If you want to want to wipe the internal storage, then probably I would go into CWM, do a factory reset, then reinstall a fresh ROM of your chopice that you have on your SD card etc...

Or SmartFlash can do pure stock, all internatl Enrico/OSP stuff, if your phone works (or even in bootloop)... tested few days ago.
Well, I'll explain how to fully wipe your cellphone and install 2.2.2 version.
Warning: I'm not responsible for any damage caused into your phone
- Necessary files:
nvflash+v10b rom MD5: B17CB7FD3542B5D02BAF49879A83ACFF
nvflash driver MD5: B630C36DEF01E6912407840BBB033BCA
- How to:
Turn off your phone
Take out your battery
Press volume up + volume down and connect to your computer usb
Your screen will keep black, don't worry
Check in device manager if there's any APX with error
Right click it to install driver manually
Browser to driver folder
Install driver (ignore any warning)
Now there's "NVIDIA USB Boot-recovery driver for mobile devices"
It's time to flash the rom
Execute "flash.bat"
If the install is stuck on "Nvflash started", unplug and try again
For me only worked in the 2nd try
After that I rooted using SuperOneClick, installed CWM and see my signature :silly:

Hello everyone,
I am quite desperate with the P1000. I am not a noob and I have tried everything I know.
After reboot, when I copy a file/folder to the internal storage, it will disappear. When I delete a file/folder it will reappear after.
This happens using the USB connection, I tried formatting it, i tried to securely wipe it, factory reset, flash other roms, re-partition on odin with pit files, connecting with adb and root wipe files... manually format the partitions on recovery.
I cannot start any application once the system has booted. Permanent FC's so no... I cannot use root explorer or any other apps.
Nothing of the above helped.
This problem occurred after updating the device to the latest firmware using KIES. There was no such problem before.
Please, If someone could help here.

When the internal memory turning bad, that is the symptom.
You can read many cases here.
They wipes, old data still there.
They install new stuff, it is missing
As if it is read only.
You may have to remove the chip and use external only.
There are two threads dealing with it.

This from a real noob, but I thought I would solict some additional input if there is some trick I have missed on my problem.
Since installing CM 10 on my Vivid, everything has worked well except the fact my internal SD has gone away. Based on stuff I have read here I have tried the following:
1. Factory Reset - no help
2. Mount drive using installed recovery, CWM - No Help
3. Install TWRP recover and try that, no help
4, Try various apps from Play Store, No Help
At this point it seems best to me to return the phone to factory state and see if the internal card is there, if it is, unlock the boat loader, install TWRP recover, then flash the rom, then boot imate (s on) and hope it works.
Any app that requires root and access to all the drives errors out.
A large group thanks to all of you who post so much info on the phone, and how to do things.
I assume you've tried mounting Internal SD in the Settings/Storage menu?

Thanks for reminding me about this, but yes, I have tried it numerous times but it doesn't work. A bit frustrating since it keeps me from storing certain stuff or using some rooted apps. Unless I find some other way, I will give the restore option a try in a few days when I get home.

My guess... Is you did a reset from fastboot...
If that is the case... I recommend getting twrp... Installing that... Mounting both storages in recovery... Then hook to your windows PC... And let windows format your internal SD...
After a reboot... Your phone will either mount your internal... Or offer to format it again... At which point it will work...
Unless we are talking about apps2sd in CM11...

As far as I know I did not reset from fastboot, but I have been in that mode a good number of times, so it is very possible.
I replaced CWM recover with TWRP and it would not allow me to mount the internal card, but the phone booted up OK, just no internal card. So I tried another factory reset and while in TWRP I noticed I was also getting a "can't mount /emmc/" message. Phone still boots up ok and works except for apps requiring access to the internal card.
I ran across some reference to a recovery called aroma something that is supposed to help with this problem, but am not sure it will work with my phone, still researching that.
So maybe this is pretty good evidence I did screw up and reset from fastboot?
I figure I am going to have to restore the phone to factory status, and start over, but I will wait to see if there any other suggestions.

I noticed that my internal sd card has two 0 folders, they are like so
Sdcard/0/0/
Everything from internet saves on the first /0/Download for ex., i am now on arhd 51, but previously on archidroid everything saved inside /0/0/Download, is there any way to fix this, or repartition or something without need to wipe all the data in sd? Because everything is messed up and i don't know any more wich uses what folder and so on.

can you post some screenshots of the two folders and their content, perhaps i can tell you what to do?
Edit: you can also copy all the files in one of the 0 folders to the other 0 folder and delete the empty one, i asked for screenshots just to make sure if there is any important file in one of those folders that maybe will cause problems with functioning of other apps or system settings if you replace it.
Here is the /0/0/ and i cannot delete archidroid folder or clockworkmod folder (write protected), also if i copy pictures from 0/0/DCIM/Camera to 0/DCIM/Camera, they are not shown, even after reboot when mediascanner is running, when i transfer them back they are imediately shown.
Edit: i can delete archidroid and clockworkmod folder, but via phone, not via pc, but the probem with transfering pictures to now used DCIM still persists.

I transfered all the pics inside a new folder in sdcard/0/DCIM but they just don't wanna show, they pop up only in sdcard/0/0/DCIM
What can cause this problem? (I can normally view them in file managers.
I noticed that when i force run media scanner it scans storage/emulated/0/0/
It doesnt scan the first /0/ that is used now, can this be fixed?

oke bro, here is what you should do everything personal and important to you like pics./music/documents copy to your external sd card or to your pc if possible both should be safe. format your internal sd card of the phone in recovery mode after everything is restored back put your files back into proper folders and all should be fine again. you are rooted and have custom recovery installed right?

Yes of course i'm rooted with twrp, i tought there was some other way than getting in all that copy paste, but i'll do that and then it must, just must work.
I think i messed something up in archidroid installation when it asked for EXT4 repartitioning or something like that.

yes that must be it, as i had read somewhere on these threads a similar problem like yours its better to avoid changing or setting options from which you dont understand what they do like EXT4 or F2FS partitions unless you know what you are doing. at least thats what i do to avoid any problems with my device

Yes, i know now for future, but, one more question..
It's 'calculating time for copy...' now, i see it'll take ages to 'calculate', so, is there any option to just mount the internal sd card, i remember i could mount it via CWM on my old S2 and everything went quick, but i cannot mount just SD anymore, only as a 'Media device', it seems quite slower..
Do you know an option to do that?

I do recommend using philz touch CWM advanced edition or just the CWM touch version as a recovery choice (especially philz) rather than Twrp, but its all up to you..
I was on philz quite a while, now testing twrp, i did the wipe, everything's perfect now, but i accidentaly deleted my pics coz' they were in the 0/0/ and i deleted the second 0 thinking i don't need it
