A little more information would be very helpful.
First is your phone completely S-OFF or are you running an UNLOCKED bootloader?
If you are running an UNLOCKED bootloader, did you manually flash the boot.img file contained in the PACmon ROM's zip file in FASTBOOT USB mode using the command:
fastboot flash boot boot.img
Did you check the Original POSTs in the PACman ROM thread to see if there are specific instructions on flashing the ROM for S-OFF versus S-ON Unlocked bootloader?

If you took a minute to read what's inside the PACman thread instead of just flashing right away, you will notice that blindndumb advised you to flash troj80's fix. The fix isn't provided in the ROM so find it somewhere closer to the end.

Was running icecoldsandwich but decided to go back to stock
Downloaded ruu vivo .exe and flashed but now stuck on white htc screen
Cant get into recovery anymore as when i click on it just takes me to a picture of a phone with red triangle and exclamation mark
I put the rom.zip file and renamed to PG32IMG and flashed via fastboot but still stuck on white screen.
Been searching for fix for 5 hours but dont know what to do. Need phone for work tomorrow what a mess !!
Any help really really appreciated
If you unlocked using htcdev.com you still have to flash the boot.img manually, otherways it will be stuck at white screen with HTC in green letters.

Rerun the ruu.exe, but while you're still in the setup, find the rom.zip somewhere in your Temp folder on your computer.
Rename to PG32IMG.zip and put at the root of your SD card.
Now reboot in bootloader.
Maybe this works but I'm not sure, and don't blame me if something goes wrong, because I'm still new here.

OK, since you are S-OFF using Revolutionary, you will not be able to install the latest Sense 3.0 RUU because Revolutionary is preventing the RUU from overwriting its HBOOT with the locked HBOOT 2.00. The reason you are getting stuck on the white screen is because the Sense 3.0 RUU requires the locked HBOOT 2.00 be present.
Look in the Development section This is a stock, rooted ROM based on the Sense 3.0 RUU that you can flash since you are S-OFF. First you will need to reinstall ClockworkMod recovery and then you can flash the stocked, rooted ROM or any custom ROM.
If you really want to return to stock using the Sense 3.0, then you need to search this forum for the way to return to S-ON and then you can flash the RUU.
Hope this helps.

Please read the OP again. He has attempted to flash an official HTC RUU, not a custom ROM. The boot.img is not the issue, it is a HBOOT issue!

OK, if you can't connect with USB then FASTBOOT USB mode isn't an option. You need to create a PG32IMG.zip with the CWM recovery.img file in it, copy it to your SD card and flash it from HBOOT mode.

This will not work with the official Sense 3.0 RUU/ROM that he is using!!!! You can flash that boot.img all you want, but if you do not have HBOOT 2.00 installed on the phone, this ROM and boot.img will not work! It will bootloop on the white screen.
That boot.img requires that HBOOT 2.00 be installed because it uses a new /LIB partition to be mounted and HBOOT 2.00 is the only one that supports it.

Bootload Loop

So I just got this HTC Vivid and some how I managed to put it in a bootload loop. I've flashed many ROMs in the past without any problems at all, but failed to do so on this one. The ROM I tried flashing was "[ROM] Dark Raider 1.0 [4.0.3][Aroma Installer][4-13-2012]" (http://forum.xda-developers.com/showthread.php?t=1586613) Everytime I turn the phone on it would just keep rebooting over and over, I tried reflashing a different ROM and now every time I turn on my phone it automatically goes into fastboot. I've tried looking around but I haven't found a solution to this problem. Anyone help?
Try this...............

I think Dark Raider is not for the Holiday or Vivid. Not sure. I do believe its developed by pirateghost, and he could be your "savior". Go to goo.im (Goo inside me). http://goo.im/devs/pirateghost/vivid and look in the rum_float folder and download beta4 NOT BETA.5. Then flash with that and you should be good. Flashing instructions can be found in the Vivid Super Guide thread..
You can also try the rooted OTA ICS firmware in the root of pg's directory to get back to a base ICS working ROM. I know that both of those work as I've been where you are now 3 times in the last 5 days due to playing with my phone and flashing ROMS. I tried pg's beta 5 last night and soft-bricked my Vivid twice, then recovered it it with Beta 4 and didn't loose a thing. For some reason I thought flashing the ROM (zip) over system was a good idea! D'0H!!!!
All you have to do is use the third method in the flashing ROMs section close to the bottom of post 1 from the Super Guide link above. Just copy the boot.img out of the zip and flash.
fastboot flash boot boot.img
fastboot reboot
Don't forget to flash the kernel too. I am using faux123s beta 4 kernel for now. works great for a beta!
If you need more help ask and I will do my best to get you back up and running!

That was the first thing I did... I grabbed the boot.img from the zip and flashed it with adb.
Dark Raider is almost specifically for the Vivid as it was built from an AT&T Vivid base. You don't flash the boot.img through ADB, you flash through fastboot.

This is exactly what I'm doing: I grab the boot.img from the zip file and place it in the same directory on my PC with my fastboot/ADB. I put my phone in fastboot and plug it into my PC. I then open up a command prompt and change the directory to my folder with fastboot/ADB. I then type the command: fastboot flash boot boot.img and am prompted with:
C:\ADB>fastboot flash boot boot.img
sending 'boot' (3860 KB)...
OKAY [ 0.820s]
writing 'boot'...
OKAY [ 1.810s]
finished. total time: 2.630s
After that I enter the command: fastboot reboot-bootloader and my phone reboots and goes back into bootloader. When I try to open up Recovery my phone opens it for a second and then just tries to reboot normally, and bootloops.
Try doing "fastboot reboot" without the bootloader part.

fastboot reboot just reboots into your rom. If your having to take your battery out then it looks like there is still a problem.
I returned my phone to stock so now it works. Is there a certain thing I need to do before flashing a ROM just so I can be sure I don't mess up again?

That you are flashing a ROM for the Vivid or Holiday device.
I think the Dark_Raider file might be since it's in the Vivid development Forum. But I haven't used it.
Look in the developmental thread and read some more.

What HBOOT version are you running and are you S-OFF or S-ON. You didn't give enough information to give you a straight answer.
Off hand, I would say that you are using an unlocked HBOOT, so you probably just need to manually flash the boot.img file from the last ROM you tried to install.

[Help] White screen

Hi there.
So I wanted to install cyanogenmod on my htc incredible s. So i rooted my phone without big problems. Then I tried to install cyanogenmod through 4ext recovery via zip file. And now all I have is a static white screen. I tried over 10 ruu's that supposed to be installed from pc and phone. I tried another recovery, custom OS, hard reset through recovery and bootloader, goldcard and some other things that I cant even remember. I also tried to lock bootloader, unlock it, reset root etc.
android version before brick was 4.0.4, sense 3.6.
vivo s-on
hboot 2.02.0002
radio 3831.18.00.11_M
Also when i install cm or some other custom OS through 4ext zip install it says OK but when i reload nothing changes.

With an unlocked bootloader 2.02.0002, you need to extract the boot.img from the CM ROM's zip.file and manually flash it in FASTBOOT USB mode.
You cannot simply flash a custom ROM from 4EXT recovery when you only have an unlocked bootloader. There are plenty of sticky threads in this forum that document the procedure you need to follow. You need to read, study and understand before you try modding your phone.

If I am flashing from exe file through fastboot I get ERROR [131]: CUSTOMER ID ERROR.
If I use fastboot flash boot boot.img I get no errors but it still stucks on HTC screen.
If I flash using 4ext install zip mode I still get no errors but it stucks.
I also got error that I dont have boot.prop in my system.
I cant remember other errors that I had.

The ERROR 131 means that you are trying to flash an official RUU for a different carrier/region. You need to create a goldcard in order to successfully flash a RUU.
Why don't you try flashing a custom ROM from recovery and then flash the boot.img file.

OK, after re-reading your posts, here are some additional tips:
When flashing a custom ROM from 4EXT, please make sure you do a full system wipe and then clear both Cache and Dalvik cache before you flash the ROM.
After you flash the ROM, then you need to immediately flash the ROM's boot.img file contained in the zip file using FASTBOOT USB mode.
As for trying to flash 10 different RUU's there ERROR 131 indicates that you have a bad goldcard.

How do I fix my phone?

I successfully had Cyanogenmod installed, but now when I turn my phone on it stays stuck at the loading screen and goes back and forth to the HTC start up (the screen before the Cyanogen loading screen). I tried reverting back to stock using the RUU but it fails every. single. time. and there's nothing I can find that will help me at least get my phone back working.
Any help? My carrier is T-Mobile

Sounds like possibly you need to flash the boot.img on it. I think , a senior member will correct me, but if your S-On and rooted, you should do that , S-Off don't require the boot.img.

I don't know what S-On or S-Off means, but it's been like 20 minutes and it's now stuck at the HTC quietly brilliant screen.
I did do a back-up before I rooted it. I tried re-installing Cyanogenmod but the installer says my phone is unrecognizable.
edit: still stuck at Cyanogen loading screen =(

S-ON or off means security is on or off, If you haven't done the mod to S-Off , your still stock S-On. To see for sure though , in the bootloader mode at the top it will say Tampered first line , then the second should say something like M7 ??? PVT ship. S-On
What recovery are you using? CWM or TWRP . Do you know or use ADB?
I just reread your OP and you said you RUU'd. why? I think too with S-On you have to relock the bootloader to RUU then unlock after boot. You can use the same unlock.bin code that you used to unlock the first time if you still have it too . Best if you done the root and everything else , do some reading on S-Off procedure and do that .

Just checked, it's Tampered, Unlocked, M7, and S-On.
I used CWM for recovery and I have ADB.

IF you can get to recovery, do you have a backup that is working ? If not you may have to unzip the CM rom, look for the boot.img , put that into the adb folder then run adb with fastboot flash boot.img , then reboot and it should go. If you have a back up of the Cm rom you might be able to go to recovery and do a restore from the backup there. And another thing you might do. Not sure if it will mess it up but/ fastboot erase cache then boot

I just fixed it. I downloaded the stock T-Mobile recovery.img for HTC One, then opened the command line used fastboot uploaded the recovery.img then used that to recover and it just magically reinstalled the Cyanogenmod.

Good glad to know it works again.. Now do some reading on S-Off sounds like you have some idea of ADB so it really isn't that hard and it will make flashing easier in the future. You might even consider a full GPE conversion. I did it and like the Kit Kat but running Elegancia and once in a while revert back to the GPE with Xposed and Gravity Box KK for a mod. Super good battery.

boot to recovery
clear cache and delvik
reboot to bootloader
fastboot USB and type
fastboot erase cache
now see if your phone will bootup
If no, find a different rom to flash
and here is a recovery flashable version of TWRP, flash it the same you do a rom and reboot to recovery to see it
