After having a great upset about the hangs that occurs when managing ringtones or notificacion, and reading lot of posts with no help I decided to post the way I fix my issue with the android.process.media error.
It seems that it's caused by an error when this process tries to access to the media stored in the device just for a ringtone, a sound of the camera shutter or the sound of a whatsapp notification.
My S4 is in Spanish, hope to make myself understand.
What I did was, going to Settings/More/App Administration/Swipe to ALL/ and search for "Media Storage". Entered and push button "Erase". This way you erase all data about ringtones, notification tones, etc. REBOOT.
Then, you must download from Internet the Default S4 Ringtones (Google knows) and place them in /sdcard/media/audio and Reboot again.
At least it worked for me. Thought I had to restore to default and it really pissed me off.

I successfully rooted my NC and other than two issues, things seem to be fine. One of the issues is that the default notification sound has reverted to what sounds like an old, landline telephone ring. When ever I get an email, content the NC to my computer, or anything else that triggers a notification, it "rings". I've looked in the system/media/audio/notifications folder, and all of the notifications are in there and have the proper permissions. I've also tried recopying the whole notifications folder from the B&N 1.0.1 manual update zip and clearing the cache, but still the problem persists. Furthermore, I've played all the OGG files in the notifications folder, and I can't even find one that sounds like what I am hearing, so where is this sound coming from, and how can I restore the default one?
I wish I could at least figure out where this notification sound file is, so I could overwrite it...

Thanks for the reply. I saw that already, but the OnTheHunt.ogg file is not the sound my Nook is making. The sound my Nook makes is literally like an early model cell phone ring. I notice it most when connecting to my computer to transfer files (so presumably the USB Mode notification sound). It is also plays as the "Default Ringtone" sound available under GMail notification sound settings.

I think I have the same problem, and it is annoying. The default sound resembles some kind of digital alarm clock sound (or as you put it, an old cell phone). It happens most prominently when connecting or disconnecting the USB cable, but also if you leave the default notification in Gmail.
I've tried pulling all the sound files via ADB and none of them match, and also tried programmatically setting the default to a specific sound via RingtoneManager.setActualDefaultRingtoneUri() in a stupid test app, which changes nothing. Very perplexing.
Take a listen to the attached recording and see if it is the same sound.

I was trying to enable ADB on my nook color and in the process I also seemed to have 'lost' my usual notification sound and ended up with this annoying ring! Anybody found a solution?
Having a slight issue which is a little frustrating. I've a search through the forum and can't find an answer.
I have a couple of custom ringtones on my SDCard in .ogg format. Every time I reboot my phone I have to reset the ringtone and notification sounds back to my custom versions. Otherwise it states 'Unknown Sound' and remains silent when a call or message comes in.
I realise this might be a consequence of running a custom ROM. But if anyone could help it would be greatly appreciated.
Would using RootExplorer with root permission be able to move the problematic sound files to /System/Media/Audio/ without issue? Or will cause further problems?
Okay, so I moved them anyway to the path above. Assuming that these would be refreshed to the cache before the Sdcard has been scanned. Initially I had the same problem 'Unknown Sound' and none of my ringtones to be found.
Then after about five minutes or so I checked the sound settings again and my custom ringtones appeared. Although I still have to reselect after reboot.
I'm not sure how but on the third reboot having selected my ringtone and having moved to the System folder they seem to have been cached and they appear along with CyanTone etc like they've always been there.
Is this a workaround I will always have to use when creating ringtones? Or am I being a complete noob?
Bought a Mini 2 yesterday. Rooted, used Titanium Backup to restore my apps and settings from my One XL, and used HTC Transfer to get text messages, contacts, etc. All seemed good...until I tried to set up ringtones. The phone ringtone was fine, as it would allow me to browse for any audio file I wanted, but when selecting the ringtone for notifications, Facebook, etc I just got a blank list. There was absolutely nothing there. After fiddling for a bit, I managed to get some custom audio to show up, but still none of the default stuff and even then files were wandering in and out of visibility.
No big deal, as I eventually got everything set up right. I installed AdFree, which recommended rebooting. So I did. And it reset a bunch of my settings, including the ringtones I'd spent all last night setting up, and deleted the text I got this morning (all other texts, which were originally from my previous handset, were untouched). So I went to set up the ringtones again, only to find that all the default ringtones were readily available, as were my custom ones, all in the one list. But I still had ads, so AdFree was apparently not doing anything.
I don't recall exactly what I did after that, but I was reading through the manual and fiddling with a few things, and when it got to the bit about sliding right or left from the lock screen to get to certain home pages, I went to try it...and it demanded I swipe up, refusing to work if I did anything else. And I had no lock screen shortcuts.
I've also lost the default HTC wallpapers; when selecting that option I just get a blank screen.
What on earth is going on?

I've also lost the ability to add lock screen widgets. The only thing I can do from the lock screen is swipe up, either from the padlock icon to unlock the phone at the screen at which it was locked, or from just below the padlock to open a Google search.
Does anyone have any ideas?

Yeah...hard reset, and dont install AdFree app. it seems its causing all the problems. If you don't like ads, use a firewall instead.
I personally recommend and use Data Defender. But there are others.
PS:Also if the app requires internet, live with the adds and support the developer.

How will the hard reset affect the root?
What does data defender do, just prevent apps connecting to the internet? I find most of the in-app ads less intrusive than the web browser ones. Snopes in particular throws up a full-screen pop-up on every page, which is just obnoxious.
ETA: Actually, it's not AdFree that's causing issues (or at least it's not just AdFree), as the default ringtones/notifications/alarms were inaccessible before I installed it. It was only after installing it that they came back.
Hard reset shouldn't unroot your phone
If you prevent apps from accessing internet, there won't be any ads.

Seems to mostly be working, but I've lost the ability to use custom notifications for Messages, Facebook, Facebook Messenger, and twicca. If I use ES File Explorer to choose a file, I get a 'wrong filetype' error, despite the fact that the audio is in the same format as the default notification audio (FLAC). If I choose Media Storage, it just shows the default sounds with none of my custom sounds. Previously, I'd somehow managed to get my custom sounds to show up in that list.
EDIT: Figured it out. Had to move/copy the files into the 'notifications' folder on the SD card. Don't recall having done that previously, so I have no idea how I'd got it working before, but it seems like it's all good now.
I am using X-Rom 2.3.1 and have issues trying to do anything in the Gallery or Google Photos app. Trying to copy, move, and sometimes view pictures throws an error saying it has stopped responding. This happens about 3 or 4 times for every picture moved, and it is frustrating. I've tried clearing Media Storage and Download Manager's , restarting the phone, and even disabling Media Storage. Disabling Media Storage worked, but I could no longer access the Gallery (which defeats the point of even trying to solve the error to begin with.) What should I do about this issue?
Edit: So I've gotten the error to stop appearing by using Simple Gallery instead of the Samsung Gallery. Simple Gallery is a better app anyway, so I don't mind using it. However, the thought that I am putting a band-aid on the problem doesn't sit well with me. If anyone has any suggestions, please let me know. I've tried basically everything a Google Search will find and no amount of cache clearing, app preference resetting has been able to fix it.
Edit 2: I apparently had some file in my downloads folder that was refusing to be read. I backed up the folder and deleted it and it works now.
I recently did a factory reset on my android one model, and since then, I've been noticing that some apps and system sounds are defaulting to the default notification sound. For example, the low battery alert. I had to go in and manually change it to another sound (the original one is unavailable in the list). There are some apps that have their own notification sounds that I can select in the app, but even after doing that, still produce the default notification sound for an alert. Anyone with ideas on what might be going on?
Yes. The problem is it can't find it as it apparently did before. I get this if I change an SD card. I find if I go to sounds and check what things are set to I will find one or more of them are now numbers (to some previously unknown system sound. I think the solution is simply to, in Sounds, set each one (ring, notification, alarm) as if you are doing it anew. Hopefully that will fix it. If this doesn't fix things it may be that you are using a special sound on Sd card and a particular app may not have permission to access storage. Hope that helps.
