I have a Nokia Lumia 1020 cell phone which uses Windows 8 and is running on Consumer Cellular.
I can access the internet via Wi-Fi but not otherwise.

What are my replies to the questions that the 1020 is asking?.
APN (Access Point Name) is att.mvno for both Internet and MMS.
There is no username or password required.
The MMS (but not the general Internet data) requires a proxy server: proxy.mvno.ccmobileweb.com, port 80.
You really should just use Nokia's network setup app if you can, though.

if you have the t-mobile web plan, your APN is wap.voicestream.com
if you have total internet plan, your APN is internet2.voicestream.com
the t-mobile web is a lot cheaper, you can add the following proxy setting that allow you to get to most of the web, email, and sync with your company exchange.
Proxy setting: getmorespeed.voicestream.com port 8080.
no proxy if you pay for the total internet.

I spent a long time with CYTA's customer service trying to get the correct mms settings. So, to save other Kaiser and P3600 owners in Cyprus from all the trouble, here they are:

also need to activate the mms option either by calling customer services, or by sending an sms to 8000 number writing 'on mms', or by logging on to cytamobile-vodafone.

I have Hyperdragon IV IOLite on my at&t tilt with radio 1.65.21.18. I am having troubling getting a solid internet connection though. My current settings:
Name: Media Net
Modem: Cellular Line (GPRS, 3G)
Access point: wap.cingular
Username/password/domain - blank
Advanced: user server assigned IP, use IP header compression, use specific server addresses - 4.2.2.1, 4.2.2.2
When I turn on my data connection it looks like it makes a successfully connection, but when I load up PIE and try to get to google it just sits on 'Locating...' and eventually times out with "The page cannot be found. Check the name and try again."
Did I miss a setting somewhere? I don't think I've got a proxy set anywhere. Skyfire also fails to load and minimo isn't working either.
ETA: I can make/receive calls, and send/recieve SMS and MMS messages. Just the internets are failing me!
look for the att settings cab file and run that. This will insure that you have all the right settings, also you might want to uncheck the proxy, where ever that is.
Lordy, I finally found that old AT&T internet settings thread. Someone should really add this info to the AT&T Wiki:

Hey guys, I posted this in general as well. I couldn't find anything for the life of me on it so I apologize if it's in here but I searched for about an hour. I got MMS working with manila 2.5 for Tilt 2. Here is the post.

First use that link and download the OTHER networks fix by Valkyrie/Ace10134.
This will create a SampleMMSC MMS server in addition to the ATT one. Once that is done, go to all messages in you text messaging, then menu, then options. Once in this menu, hit menu again and hit MMS server. Here is where you need to put your settings and I think where the issue on the other fix has been. ATT's send limit is 600KB while Manila 2.5 defaults to 1024kb.
Server name: Whatever the hell you want it to be.
IP address: wireless.cingular.com
Port number: 80
Server address: http://mmsc.cingular.com
Connect via: MEdia Net
Send limit: 600 KB
WAP version: WAP 2.0
Let me know how this works for you guys. Make sure you actually have the phone set to use the new MMS server too =)
If this does the trick for it some one cab it up for me please. I can't right now. Good luck guys. Let me know if this does the trick for you. I have sent many of MMS fine.

For those who can't receive at&t mms:
I used the PR MMS MediaNet settings tool available on XDA
Go to Settings>Connections>Connections
Delete all data connections before running the tool
Use the device Network Setup Tool to make your primary connection.
Run the PR MediaNet MMS tool to set the size limites
After setting up the connection create a new connection in com manager
Use Work Connection
Use these settings
Connection Cellular GRPS 3G
Name I used MediaNet No Proxy
APN wap.cingular
Username [email protected]
Passwork CINGULAR1
Do not use the proxy settings for this
Have all internet applications go to this connection do that in your com manager
Go into your mms options in messaging click servers click at&t mms make sure MediaNet is still the connection for send/receive mms
now it will send receive mms using the at&t proxy and the internet for other apps without the proxy
