As many of you, I too have autofocus issue. In the first moment after staring the camera app everything is fine and then the autofocus kicks in and puts everything out of focus. Close captures work out well but infinity is always blurry ..
So... lots of users share the opinion that the optical parts (the multiple lenses) are not moving accordingly and can never achieve focus due to a hardware problem. Therefore the camera unit needs to be replaced which is nearly impossible if you don't have a warranty. (If you do have warranty you get a brand new phone lol)
BUT... everything is in focus in the first half a second after starting the app right? So I thought switching off the autofocus will help and I think it does! I am running latest Venom ROM 4.4.3 with Sense. The latest sense camera doesnt have AF switch but I found this nifty app called "A Better Camera" and it does offer AF lock. So - after starting the app, first thing press the AF round button on the left and choose AF lock. In this was the phone never gets to try to focus and the focal lenght stays on infinity (I assume...) which gives you much more detail.

Well bottomline - its FAR from good but somehow better (?)
what do you think? - try with your faulty units and maybe upload your examples here?

My Nexus 6 seems to have an issue from day one and is still present with the 5.0.1 update and the most recent camera update. I'm unlocked and unencrypted.
When using the camera, any camera app, the viewfinder and ultimately the photo/video comes out blurry when shooting in landscape. If i shoot in portrait, the viewfinder and the photos are sharp.
In landscape is starts clear and sharp and then quickly changes to being blurry. Even resting the phone on a still surface it is always blurry. Lighting conditions or shooting mode don't make any difference.
I attached two photos showing the difference, both handheld.
It's almost like something is wrong with the OIS in landscape. Hardware issue?

touch to focus... it actually begins to focus and then it looks like the sensor is shaking on the screen and shows up in the photos. I have no issues at all when in portrait mode.
This even happens when the camera is sitting on a still surface so its not my hand shaking.
This is with the camera sitting stationary on my desk and a 10/sec timer
mine did the exact same thing. the OIS never fully settles due to a hardware defect. i got a new one from T-Mo and returned my defective one and the problem has gone away.

Thanks for the info and helping verify its a hardware issue. guess I will be calling T-MO then. Where they hard to convince in replacing it?

i was lucky to call and complain about it within the 14 day buyers remorse period so they could send me a brand new one instead of a possible refurbished one, but it was like pulling teeth with them. i had to explain that it was a hardware issue several times before they agreed to send me another one. after several phone calls later in the next few days, i finally got someone who was willing to do it then and there while on the phone.

I have about the same problem with my more then 1 year old Nexus 6. After my nexus 6 fell of my bed (with a tpu case and no visible damage) it seems that i can only get the camera to properly focus while holding it at 90 degrees (so with the camera sensor towards my right hand) and while holding it at 180 degrees (so upside down) 0 and 270 degrees (camera towards left hand) the camera focusses, during the focussing it's focused for like a fraction of second and then the blurriness/vibration starts, in both camera and video mode.
So thanks for this topic, i guess i'm ordering a replacement camera module.
Replacing the camera module has fixed my focusing problem. The module itself was around 13 dollars on aliexpress. For anyone that's facing focussing/ois problems with the camera, you can easily replace the module yourself.
So I dropped my htc one m7 and after that I noticed that the camera doesn't focus in on objects, even if i tap the screen there is no change so all pictures look out of focus.
I went to get a new camera fitted today although the guy working in the store ordered the wrong camera yet blamed it on the CPU and said even a new camer may not fix the problem...
Any help is very much appreciated!
I am not much interested in camera when i bought Mi A2, it works fine. but recently when i was taking picture, it was half side blur and i was not able to make focus on upper side of camera. i tried all modes but no use. I have Note 6 Pro and it's camera functions and focus properly.
in mi A2 I observed when in MANUAL mode i select Lens as LOW LIGHT, it works (but in manual mode only).
do any one has some solution or give reason to behave camera like this.

It was a February patch bug and it seems to persist in March patch... Xiaomi.

This is a hardware problem, I also had this, give the phone a guarantee.

Looks like this is a hardware issue, I'm having the same problem after 4 months of use.
Same here, I also took it to guarantee and they told me it was a software problem, but when they gave it back to me nothing had changed. Camera still not working

I had to order a new camera module, I'm still waiting to arrive to replace it..

I had ordered it from this website but I had paid the cheapest shipping, I'm from Mexico so it will take a while to arrive to my country.. maybe next month..
I had the same problem and replaced the camera module with a new one, and the problem was fixed... temporarily because 3 months after it started happening again with the new camera, so either I am super unlucky, or it must be some other defect in the phone that tends to breaks the primaey cameras...
At this point I don't think I am going to replace it again. Do you know of any way of recording videos with the low light camera?
I have the same issue. In fact this issue persists in the video mode as well. Its okay for videos of objects nearby but move even a few metres away and it becomes very difficult to focus. I was at this concert and wanted to record the performance but only got a very blurry video with muffled sound. For photos this issue is especially an inconvenience when taking photographs of documents etc where you could very well do without the unnecessary bokeh effect. In MANUAL mode this does reduce a bit, but not to any extent that helps.
I was just told by a friend that installing the Google camera mod might help, but I'm yet to try it out.
I have same problem with upper side blur. Seems that switching to "manual" mode and change "lens" from "regular" to "low light", as said by the original user. It works fine with me. Thanks for the tips ! ........ so it is not the hardware problem, it is a software problem. Changing camera may not work ! Don't know if they are going to release new firmware to solve it or not ?? Any idea how to upgrade the firmware ?

If it is a hardware problem I do not see why it would prevent using the secondary camera, it can be a problem with the lenses (Have you thrown or hit your phone?) and not with the module. There are those who have returned to Oreo and the problem persists.
Not really an issue I think but does anyone know why the front camera selfies at night appear extremely soft and bad ? I tried the native Samsung camera app, the Google pixel camera port ( the s10 camera day one ) and even tried an application called Fv-15 . Is that just the quality i should expect from the phone and does anyone have a good solution?

Actually I got a Note 9 Silver from Bestbuy, then after 6 months the device started to restart by itself.
Turns out the whole sensor array, camera, iris scanner, was broken.
The solution was a replacement, so take it to your tech support and have them run a hardware test.
Sadly QA with Samsung is just nah..
