After following a few possibly outdated threads I was following this tutorial I found using google search (probably should have checked around for the all in one toolkit first):

Since I looked at a bunch of ROM reviews I thought I'd try SlimRoms. So as per the suggestions under step 3, I skipped to step 4. I then followed the SlimRom instructions. I flashed Gapps successfully, but got a failed message when trying to flash the ROM. So I thought I'd just go to step 3 again. At this point I tried to reboot and got the following message
"Your device does not appear to be rooted. Install SuperSU now?..."
I choose to install and that's when my problem started. I was stuck at the htc loading screen. The following thread seemed like it would help, but since I installed the TWRP recovery 'm not sure where to find where to erase the cache and side load ROM for one.

I may have a learned a lesson, but still don't want to give up on taking a stab at having a rooted phone. I'd like to get better at it, so thanks for any and all advice.
Also sorry if the question sounds a little incoherent, I'm kinda tired and frustrated at the moment.

Here are the installation instructions for SlimRoms:
- Download latest SlimRoms and latest gapps and put them on your sd card

Thanks much man. I tried this but kept getting this error in trying to wipe dalvik and cache. So after several different attempts (rebooting, recovery, etc.) I decided to just format data. I then used the adb sideload feature in TWRP to try putting the zips back onto the sd card. Several frustrating failed attempts due to CP continuously telling me device not found. Finally, after making sure not to let my phone rest, I was able to get both on and then rebooted in TWRP. Ta da! I finally got past the HTC Logo screen and saw the sweet SlimKat logo. Now I'm just waiting for it to connect to service.
I never thought I'd be able to comprehend all this as I just started learning today, but thanks to you folks at XDA I'm (hopefully) starting to benefit. Hope to keep learning from you all. Thanks again.

I recently tried to install CyanogenMod 10.1 on my HTC One. I've rooted the device, which was pretty simple. Then i followed these instructions:
Step 1: Download the CM 10.1 Nightly for HTC One from get.cm/?device=m7 and the latest Google Apps package from here goo.im/gapps, and transfer them to the root of your HTC One’s internal storage.
Step 2: Boot your HTC One into recovery mode by pressing Power and Volume Down buttons simultaneously .
Step 3: In CWM recovery, select Install ZIP from SD card, followed by choose ZIP from SD card, and flash the CM10.1 ZIP package that you downloaded above.
Step 4: Repeat the above step and flash the Google Apps ZIP file.
Step 5: Now, after having flashed both packages, select wipe data/factory reset in recovery.
Step 6: Reboot the device
I forgot to "select woe data/factory reset", and to flash the gapps, but i could just download them in rommanager... And then i should be running android 4.2.2, but there is a boot loop, now i have waited all night, but it still shows the cyanogenmod boot animation. i have all the required files on my pc, but my backup on my phone isn't there anymore. Please help me, i'm very new at this, and i'm only 14 years old. I spent about 850$ on this phone (i live in Denmark) so if my phone is "broke" then i'm screwed. As i said, im pretty new at this, so i need step by step instructions.
Just follow CM instructions.
You really need to wipe/reset the user data and the caches, otherwise there will be trouble. I accidently forgot to wipe and it took a day and a night for the phone to discharge so I could properly install. Just follow the direction exactly.
Longpress power button.
Reboot in fastboot and try to flash the boot.img (you can found it in the zip of rom), and flash it from fastboot with the command:

After flash, type:

fastboot erase cache
Now, reboot your device with

fastboot reboot
Now it should work..

Never mind, i figured it out and it is great.
New to Android (coming from the iOS World) with my beloved HTC One.
Just wanted to get ride of all the HTC bloatware and decided to go to CM.
I rooted the phone easily but I failed to flash it with CM10, took me a while to get there with google search, you guys saved my ass big time, thanks for all your contributions.

How?i have a similar problem
Hey guys, I'm having a weird little problem here.
So I followed this guide here, unlocked my bootloader, everything fine.
So far so good. I flashed the mentioned TWRP Recovery through fastboot, no error messages, still looks like everything's fine.
But whenever I try to get from the fastboot screen on my phone into recovery mode, the screen stays black.
I tried restoring to stock, hoping that would add the original recovery back in. Nope, still black screen.
After that, I successfully flashed the recovery from this thread. Again, no error messages and everything looked fine. But still no luck. My recovery screen stays black for whatever reason.
I'm on Windows 8.1 64Bit and using the current Android SDK. The SDK setup had a little problem with finding Java JDK (64Bit) before I could set it up properly. But I solved that one by adding a system variable. Again, nothing out of the ordinary here, at least as far as I understand.
So what am I doing wrong?
I'd really like to get a custom rom with root on this baby. Don't wanna be stuck on stock. :/
Does anybody have an idea, how I could manage to access any kind of recovery in this case?

Fastboot gave you the OKAY on flashing ? Did you reboot your phone after you flashed the recovery ?
You don't have to, if you reboot you will have the stock one again = black screen.
Try like i did (assuming you correctly unlocked the bootloader, and installed MotorolaDeviceManager).
1) Get the right recovery for XT1072 from the author's post HERE.
2) Download the latest CM rom from HERE and latest CM gapps package from HERE.
3)Move the CM rom and the gapps zips on the SD or EMMC then reboot to bootloader.
4)Flash the recovery whit "fastboot flash recovery twrp-thea.img" and check if you got the OKEY from fastboot (DO NOT REBOOT YET).
5)Now pick up your phone and use VOL-DOWN to highlight RECOVERY then VOL-UP to start the recovery.
6)You should have the TWRP menu open now, go WIPE -> ADVANCED WIPE and check Dalvik/Data/Cache/System then swipe to wipe those 4 partitions.
7)Go back to main menu and select install, flash the CM12.1 zip then gapps zip and now you can reboot, first boot take a while so don't panic.

I uploaded for you the latest untouched platform-tools folder (rev22) for windows if you wish to try this one, LINK, VirusTotal scan.

Yes, another one of these threads, but I need some help.
I got the bootloader unlocked, flashed twrp into recovery using fastboot. Unfortunantly the device rebooted into stock which I'm reading got rid of twrp. One problem is I can't figure out how to get twrp on there, and to stay on there, so I can use it.
Ignoring this issue I just continued. The guide simply said flash cm11. I searched around for a little bit on how to do this with fastboot (since my twrp wasnt working), but ended up doing the stupid thing of running:
fastboot flash system cm-11-20141115-SNAPSHOT-M12-ghost.zip
I know my problem is that I flashed the system with just a zip update file. I should've performed an update.
But now I can't figure out how to find the stock system image for my phone, the actual img file, so I can flash the system again. I have a sprint moto x.

you should be able to install TWRP by using the command, "mfastboot recovery twrp.img"
regarding CM11, I don't think is needed anymore to go from CM10 to CM12. it was at one time, but I believe CM or TWRP made some updates so that step is no longer necessary.
but before we can help you further, let us know what ROM you are on now and what ROM you want to be on next.

Ok I got TWRP on there. Now I would like to get to lolipop 5.1 one way or another. I've always liked CyanogenMod (of course) but want to try CyanHacker. http://forum.xda-developers.com/moto-x/development/rom-cyanhacker-lollipop-ghost-t3033367
Right now I'm on a stock sprint rom I got with kitkat 4.4.4
Here's what I was going to do: (copied from forum)
1. mfastboot flash the latest TWRP if you aren't already at that point. I won't help you on fastboot things as you should already know- look around in the forums for info.
2. BACKUP YOUR STUFF- don't want to lose things without being able to get them back, right?
3. Flash the downloaded CM11 snapshot.
(Heres where I'm stuck. This is where I soft bricked it last night. I tried just now to apply the update using twrp and it just fails. It also fails at rooting my phone.)
4. Let CM11 fully boot and go through the setup screen- just skip everything and get to the homescreen.
5. Reboot into Recovery and wipe EVERYTHING- system, data, dalvik cache, cache, etc.
6. Flash your Lollipop ROM.
7. Flash PA-gapps.
8. Flash SuperSU.
9. Wipe cache/dalvik cache.
10. Reboot.
11. ???
12. Profit.

I'm what you'd call a super noob at this stuff and I've got my poor little SP in a crazy bootloop... I've tried LITERALLY EVERYTHING the internet has told me to try; wiping every cache out there, flashing custom roms, stock roms, you name it. It began when I just got CWM (don't ask me why it happened then, I have no clue why) and this is even BEFORE I put a ROM on! (I was all ready to put a ROM after getting CWM but bootloop happened).
Note: I have CWM recovery, I think I have lost my stock recovery, if I try some ROMS (ex. CM 12) they give me this error 7, once I found certain ROMS (PACMAN ROM, unofficial stock ROM) that would flash and install, they did but still left me with a bootloop, I have tried wiping so many caches it's not even funny, I've tried flashing the original firmware, nothing works... so I'm here for help.
I have looked at every google link and every method possible in trying to fix this thing! I really can't afford a new phone and I just love this one too much... please help!

Please go to the above link to get TWRP. Also, please keep in mind that TWRP is now the only still supported recovery around. CWM and PhilZ are both really outdated and no longer maintained due to the introduction of Cyanogen Recovery which is very minimal in features.
Once you get yourself the download, extract the files from the zip and get the twrp.img file. We will need this to flash into the boot partition of Xperia SP, granting you a direct boot into recovery once it is flashed. Please then copy the original zip to your sd card, as we will be flashing the original zip via TWRP later to give you access to TWRP in the future.
Using your preferred tool, flash the twrp.img file into the boot partition.
Connect your Xperia SP via fastboot. Make sure SP is shut down, hold volume up key and connect the SP with your computer.
If you're using a fastboot tool via command prompt, the command would be
fastboot flash boot twrp.img
If you're using flashtool, hit the lightning icon in the toolbar, select fastboot mode. Then select "Select kernel to flash". Make sure the file type is "img" else you won't able to find your TWRP img file. Navigate to mentioned img file and select it.
Afterwards, when flashing complete, you may insert sdcard and start the device. It will boot directly into TWRP as we flashed a recovery to the boot partition.
Clean wipe data, cache, dalvik cache, system. You may leave internal storage.
Flash the ROM zip you acquired (make sure file integrity is checked beforehand!) then the TWRP zip earlier. Flashing the TWRP zip grants you a persisting recovery, as explained in the linked thread.
You may now reboot and enjoy
Think i have rally buggered up and update, was going to try out the havoc rom, when i went to flash it i was told i needed to update vendor to v12 so i flashed the official miui vendor v12 rom, rebooted and now im stuck in a fastboot loop. i think my mistake was following the instructions on the havoc rom and wiping system, data, dalvik and cache without the havoc rom flashed.

So, when I screwed my phone trying recoveries, I got stuck in FastBoot. no matter what I did, even Power + Vol Up got me to fastboot, and I had just wiped my OS, so I had nothing to boot into.
This happens when you wipe your SYSTEM and/or VENDOR partitions and choose reboot recovery, or flash an incompatible recovery.
After a lot of stressful minutes, scared half out of my mind. A guy named @Shivsom helped me.
Basically what he made me do was, Download his file vbmeta_dv_shivsom.img that is given below in attachments.
Then copy the twrp.img file and vbmeta_dv_shivsom.img file to my adb Folder.
Open the Command prompt in that folder, by clicking on the address bar of File Explorer and typing > cmd > then Enter
Type the instructions

fastboot flash vbmeta vbmeta_dv_shivsom.img
Then type, (Since my twrp image file was re-named to twrp.img, if you haven't done so, do it)

fastboot flash recovery twrp.img

I was OverJoyed when this worked. However, every time I entered recovery, I was met with the fastboot logo again, Therefore, I just repeated the process I did when I first unlocked my bootloader via twrp in the tutorial above, which was, I went to manage partitions and formatted my data, wiped my rom, and did the same steps as in the tutorial when I first got my orange fox.
Now use twrp interface, copy orangefox.zip to my phone via PC, then flash that zip. Then go to settings > manage partitions > data > format data > type 'yes'. Then quickly flash a vendor, then a flash ROM (normal clean flash, tutorial given above) and then boot the ROM.
This was a nasty disaster. Glad I got through it.
Right so I tried LR-twrp (it's in Chinese, but I can change the language to English) . PitchBlack and PitchBlack-old Both broke my phone to get stuck in fastboot mode no matter what I did. .
HENCE I conclude that for all Xiaomi devices, OrangeFox [OFX] is the best recovery. TWRP is suited for Samsung.
The moral of this story is
Do not wipe vendor and/or system.
The is very little need to wipe either of these.

When you flash a vendor file, the install will wipe vendor, as part of the process, before flashing the vendor file.
I have no idea why we have all the devs giving instructions to wipe these partitions as part of the process to install a ROM. It looks like it's a case of them all copying and pasting each other, so it's getting added to the installation instructions of each ROM that comes along.
A few years back, there was some instances where wiping system became necessary, though I can no longer remember the details. Suddenly everyone was saying that a normal clean flash was no longer enough and we should all be blitzing all partitions when installing a new ROM. It feels like this mentality is still present, even though it is absolutely not required.
In future, when installing a new ROM, stick with the factory reset in TWRP, it's all that is needed. There is a very good reason it is on the first page of the wipe menu in TWRP. Doing ordinary installs there should be no reason to go into the advanced wipe menu.

Thank you for this type of answer. I was trying use all the instructions to wipe, but all the time I got brick (fastboot loop) in my Mi 9 SE. Not problem when usign the first page of the wipe.
