Soooo i was a big dumby and i wrote over the modem in order to get the update.
Now i have no signal/service available , or compatible baseband flashed.
Wifi works, everything else is amazing !!

Thanks!!! I tried flashing the FSG but it wants the file as a mbn. I tried changing the extension but it still didnt take it.
Although the modem bin file flashed fine, rebooted but still no service
I think I did extract it with an mbn extension, don't know if it will make a difference but I'll upload shortly.

You need the following commands. Just keep your mbn file.
fastboot flash modem NON-HLOS.bin
fastboot erase modemst1
fastboot erase modemst2
fastboot flash fsg fsg.mbn
I would love to know if those modem files work for the Telus phone after flashing, as the only thing holding me back is that it currently won't let me back up my own. If it does then lollipop here I come!

Hi floepie, does the adb shell and su commands provide temporary root? ie Upon rebooting root will no longer be accessible? I'm looking to back up my modem files before flashing to xt1095 Lollipop.
Also, for anyone else following this thread, I'm a bit confused by the discussion about fsg.bin vs fsg.mbn; are there two different fsg files or is it simply that when pulling fsg.bin you save it as/rename it to have an mbn extension?

I'm pretty sure that in order to make an image of your baseband files, you need to be rooted. Root access is granted by SuperSU. As for the extensions on the fsg file, it doesn't matter what you call it. You just have to be consistent when copying it or applying it back to your device. I think someone posted on the forum using mbn and then used bin for applying it, which for obvious reasons wouldn't work.
This might be a dumb question, but do you have to always flash the baseband files or modem files (not sure which is correct) every time there is a new update once you have converted to XT1095

Ok, so I unboxed my new phone UK (XT1092), unlocked the bootloader after it updated to Lollipop, and messed up somewhere causing me to have to install stock rom again. After installing the stock rom (4.4.4) I do not have signal and the baseband is unknown as with the OP. I have attemped to follow multiple guided concerning the same fixes but have gotten no where and am now extremely stressed and confused. Previously if I had problems with signal I just reflash a radio but times seem to have changed. The phone now shows as a xt1095 when plugged into pc.
Where I went having messed up:
Where I then went having found I had a xt1092
Update on xt1092 worked! I did this:
1. Downloaded the 4.4.4 system image from xt1905 (first post for download link)
2. Extracted the zip
3. flashed over mfastboot.exe

mfastboot flash boot boot.img
mfastboot flash recovery recovery.img
mfastboot flash system system.img
mfastboot flash modem NON-HLOS.bin
mfastboot flash fsg fsg.mbn

5. reboot

Then realising I had no signal I followed this post:
YES!
I finally have Lollipop on my XT1092!!
I followed steff999's guide but encountered several problems with the modem and data connections etc.
I just couldn't figure out why flashing the XT1092 modem files didn't work.
Then I went back to my original firmware and did the procedure again.
But now I did not let it boot after the OTA Update and directly booted into fastboot mode and flashed the modem and fsg files.
Then after reboot I immediately had mobile service. IMEI and Baseband is back again.
I use Linux and it is very easy with Linux.
In a terminal just go to the folder with the files (no matter where they are!!).
After getting Android SDK I did:

$ sudo apt-get install android-tools-fastboot <<-- to install fastboot
$ cd /directory with your files...<<--- choose your folder
$ fastboot flash logo logo.bin
etc
after flashing the files put OTA.zip on phone. Go in recovery mode and apply update.
Then when it reboots directly go in fastboot mode and type:
$ cd /directory with your XT1092 modem files/
$ fastboot flash modem NON-HLOS.bin
$ fastboot flash fsg fsg.mbn
$ fastboot reboot
now it boots into lollipop and it should recognize your sim card.
This is how it worked for me with Linux. Should be similar with Windows but I think the files for flashing have to be in fastboot directory then.
I did not use erase modem commands
I hope this will give hope back to people who encountered similar problems with the modem. I was about to give up but decided to give it a last try.

I converted my xt1097 to xt1095, with the tmo 4.4.4 firmware, I boot it but complete no setup, and just copy the OTA from PC to phones internal storage. No adb sideload.
I've had Brazil radios work on my rogers RAZR before. There is a full lollipop BR firmware in the development section. Is might try it.

Any chance you have a copy of the modem files for your moto x 2014 (XT1097 ) on Wind? I flashed the T-Mobile 5.0 xt1095, and don't think I properly backed up my files.
If you can explain to me how to extract the files from my phone using adb, sure. Coming from a nexus, not so good with adb

Please just do it the fastboot way...just too many help threads that incluce "I used MythTools". Once you do it manually and you know your PC is set up properly, then start using Myth.

as @skeevydude told, its better to use fastboot... this may be useful to you..
1)download the required firmware from here and recovery
2)extract the zip and place the recovery image and fastboot binaries in the extracted directory and open terminal in this directory.
3)type the following commands in order

If you want to get good at it have a folder for development with everything in there and point the command line path to the commands so you don't have to keep tabs on where you are

Also, there's any risk by flashing the Mexican version and then turning back to AT&T (in case I don't like it)?

Just backup your stuff to the EXT SD card not the internal one , just in case it gets wiped.

there are no risks as @Youngunn2008 told, i too have flashed it innumerous times.. and about improvement, you can always work on it, some common things are system app modifications as you told like freeezing, removing, adding etc and performance modifications like editing build.prop or working on kernel or editing framework_res.apk etc

Its your wish... I recommended Mexican retail radio because it supports more base bands than att and on the whole, both should work fine... And the radio is VERY VERY IMPORTANT...

I have an canadian Rogers Moto X 1058 which I use here in Europe. To get european languages & the 4.4.4 Update I went back to Stock Recovery & French 4.4.2 (RETAIL-FR_GHOST_KLA20.16-2.16_cid7_CFC_1FF_v2.xml.zip).
Then I wanted to update via OTA to 4.4.4 but it failed. Doing this via adb sideload it also fails with this error:
"/modem/image/dsps.b00" has unexpected contents"
What I did by flashing the french 4.4.2 was replacing the NON-HLOS.bin and fsg.mbn with the ones from the Rogers 4.4.2 to have Radio / Modem working with the Hardware.
What can I do now to get 4.4.4, please?
a) Flashing the Verizon 4.4.4 over my 4.4.2 like mentioned here http://forum.xda-developers.com/moto-x/general/update-moto-x-mexico-to-4-4-4-vzn-firm-t2828190 and using the Rogers Modem Files again
or

Since you have already changed ROMs (I normally don't suggest doing this), here is my recommendation:
You can flash the entire French 4.4.2 again (baseband and all) and then accept the OTA to 4.4.4.
Keep in mind that you can omit the "erase userdata" command and it will preserve your data and apps.
Also note that we have received reports that using a 4.4.2 modem with a 4.4.4 ROM has resulted in issues for some. This includes no signal, or even missing IMEI. So I really would not suggest flashing a 4.4.2 baseband after you accept the OTA. If you really want to be on the Rogers baseband (keeping in mind that there is no need to do so - the FR baseband should work fine for you...), you will need to wait until the 4.4.4 full SBF for Rogers is released.

Exactly when I changed the firmware version I got missing IMEI error. Mobile fails to detect network.

Well, ideally, having a nandroid backup of your EFS is ALWAYS a good idea.
But without that, you can try to restore the stock firmware for your model/carrier and same android version. Do not downgrade! Like so many have done.... If you flashed a different carriers firmware, and a higher version that your carrier has available....then have big issues, You may be putting the phone away and waiting for files you can flash for your model... On the same version of android you are now on.
I flashed 4.4.3 and got no IMEI. While downgrading with 4.4.2 got gpt.bin error. And again no IMEI. Then I flashed every other file of 4.4.2 except gpt.bin and now on 4.4.2 with IMEI back. But afraid to take ota update. And now waiting for full firmware file to leak.

Glad it's back.... And no.... I would never take an OTA again.... To be safe.
You're not missing anything special... And I'm sure flashable files won't take too long.

i know just some minor update to camera and dial pad.
Ok, so I unboxed my new phone UK (XT1092), unlocked the bootloader after it updated to Lollipop, and messed up somewhere causing me to have to install stock rom again. After installing the stock rom (4.4.4) I do not have signal and the baseband is unknown as with the OP. I have attemped to follow multiple guided concerning the same fixes but have gotten no where and am now extremely stressed and confused. Previously if I had problems with signal I just reflash a radio but times seem to have changed. The phone now shows as a xt1095 when plugged into pc.
Where I went having messed up:
Where I then went having found I had a xt1092
Update on xt1092 worked! I did this:
1. Downloaded the 4.4.4 system image from xt1905 (first post for download link)
2. Extracted the zip
3. flashed over mfastboot.exe
Code:
mfastboot flash logo logo.bin
mfastboot flash boot boot.img
mfastboot flash recovery recovery.img
mfastboot flash system system.img
mfastboot flash modem NON-HLOS.bin
mfastboot flash fsg fsg.mbn
mfastboot erase cache
mfastboot erase userdata
4. sideloaded the OTA update from first post
5. reboot
6. flashed back the original modem and fsg from xt1902, without erasing modemst1 and modemst2 -> https://www.androidfilehost.com/?fid=95784891001608300

Then realising I had no signal I followed this post:
YES!
I finally have Lollipop on my XT1092!!
I followed steff999's guide but encountered several problems with the modem and data connections etc.
I just couldn't figure out why flashing the XT1092 modem files didn't work.
Then I went back to my original firmware and did the procedure again.
But now I did not let it boot after the OTA Update and directly booted into fastboot mode and flashed the modem and fsg files.
Then after reboot I immediately had mobile service. IMEI and Baseband is back again.
I use Linux and it is very easy with Linux.
In a terminal just go to the folder with the files (no matter where they are!!).
After getting Android SDK I did:

$ sudo apt-get install android-tools-fastboot <<-- to install fastboot
$ cd /directory with your files...<<--- choose your folder
$ fastboot flash logo logo.bin
etc
after flashing the files put OTA.zip on phone. Go in recovery mode and apply update.
Then when it reboots directly go in fastboot mode and type:
$ cd /directory with your XT1092 modem files/
$ fastboot flash modem NON-HLOS.bin
$ fastboot flash fsg fsg.mbn
$ fastboot reboot
now it boots into lollipop and it should recognize your sim card.
This is how it worked for me with Linux. Should be similar with Windows but I think the files for flashing have to be in fastboot directory then.
I did not use erase modem commands
I hope this will give hope back to people who encountered similar problems with the modem. I was about to give up but decided to give it a last try.

I am unsure how to get the OTA update onto the phone memory unless I power up and download, attempt at sideloading via adb resulted in the phone "aborting" at 100% sent.
So, could someone here please attempt to explain exactly what needs to be done

The multi-tool did not work for me aside from rerooting after the problem was solved. Thanks anyway.

I will be honest wit you people, I really mess up on my phone.
I was trying to upgrade to lollipop on my AT&T moto X.
I miss the step that I need to unlock my bootloader, so I just came directly to install the pure edition Brazilian rom.
When the phone restart I notice that my phone was not reading my simcard, and It is because my EFS file was deleting and Now I dont have IMEI...
Unfortunately I did not backup my EFS file.
Now my radio is not working, I did already search on all the webs a solutions but I with no luck.
Can anybody help me please!
How did you flash the ROM? Via fast boot? If that's the case you only mess with the modems. Try to download the firmware of at&t and flash the modem and the fsg files. How did you update then? Side load update? The problem you have is the modem as I said, nothing more
here is the firmware 4.4.4 of AT&T http://pan.baidu.com/wap/link?uk=3927248220&shareid=4222363237&third=0 unzip the file and try to flash the modems via fastboot, that should be enough.

I did try with on the motorola page, but it said that my phone is not compatible to unlock, so I just try installing the braziliam rom, In the process the phone shows a few error and when it restore there was not signal until I realize that I do not have IMEI on it.

Do you have knowledge of fatboot and bootloader ? in that case unzip the file of your firmware in the same carpet you have the adb and fastboot files, be sure your phone is on "USB debugging mode" and plug your phone to your PC (phone must be power on), then type
adb devices (to see if the phone is recognized)
adb reboot bootloader (to reboot into bootloader)
fastboot devices (to see if the phone is recognized in fastboot, because sometimes it doesnt)
fastboot erase modemst1 (delete the old modems)
fastboot erase modemst2 (delete the old modems)
fastboot flash modem NON-HLOS.bin (flash the modems of your phone model)
mfastboot flash fsg fsg.mbn (flash the modem of your phone model)
mfastboot reboot
After the reboor you will have signal again after a couple of minutes, and of course your IMEI is back too.

I found the at&t modem NON-Hlos.img but I can't find the fsg fsg.bn. and the first one is not working.

OK, let me tell you what I did.
I follow the steps that you gave me but it did not work.
Then I tryed installing the at&t room but it just mess more the things, because when I reboot the phone it went straight to the boot loader, and I was not able to turn on the phone. at this time I was really scared xD
so I create a new folder with the at&t room and the fastboot.
open fastboot and start flashing all the files in the folder, but this time I did not flashed the non-hls neifghter the fsg file.
then I just reboot the phone and read the sim card quickly.

I've unlocked my bootloader but I had problems doing this, so I flashed the same rom. After this, I successfully rooted my device but, normal calls through 4g/3g are working without any sound.
WhatsApp calls, or any media app is working properly, only calls trough 4g/3g are muted.
My question is: can I flash only modems partitions? I don't wanna loose any data and personal files.
If I go to settings>about phone my IMEI is listed normally. 4g volte is enabled. Data connection is perfect.
Recently Motorola has launched a new software, PPSS29.55-37-7-2 and I wanna know too if I can root this version, or, I can upgrade to this version without loose personal data through fastboot.

Thanks a lot for your help! But was unsuccessful. I flashed modem partitions but i'm facing the same problem.
Before I flash all system, a question: my root will be removed? I ask this because I think magisk do some modifications to /system partition, so is really a pain root this OS, hehe
Just one more, if you allow me:
To update to lastest security update I'll download lastest zip, and flash all partitions, but without erase userdata, right?
Then, if I wanna my device rooted, i have to do all root process again right?
