Looks like you shifted from stock to CM and restored all the apps (moto stock apps included) and sensor services is now incompatible with your current ROM.
Nah, i'm running stock, didn't do anything.
It's gone after a reboot, but curious what triggered it.

Have you done downgrade from 5.1 to 4.4.x lately?
It's usually caused by that and it does exactly what it says.
If you didn't downgrade, then maybe you've uninstalled some of the moto apps related, which might have upgraded the fw.

It happens on 5.1 stock firmware too.. I don't exactly know what it is.. and I didn't receive this norification yet, but here's a link!

Weird, also got this after a crash and auto reboot. Rebooted again and it went away.

This night my phone has got a new update ML3. Downloaded by OTA and installed. But there is a problem about front camera. Rear camera is working nicely but when I switched to front, it s freezing and saying "Camera failed". Please help me. What can I do now for solve this?

Try several different cameras, such as focal, picarts, vsco cam and see if there's a difference.
If that doesn't work you can try reinstalling the firmware yourself.

Since it's not an app issue then it's either firmware or hardware. Since hardware is the least likely and also the worst possible outcome, I'd follow the above link and reinstall the stock firmware. Try doing a factory reset first if you'd find that easier.

You'll have to choose a i9295 ROM to flash then (here). If you don't already have CWM then push it with Odin (here)

If the front camera worked properly before the OTA, then it's a software issue, if not then it may be a hardware issue.

It's somewhat unlikely that it's a hardware issue but it's still possible, more so since you've flashed fresh stock firmware. Your warranty is already void at this point (there's ways to "unvoid" it) despite the fact that you've flashed stock firmware. Flashing sets of a flsh counter regardless. I can link you to information on how to remove this later if you like...
You should try flashing a non-stock ROM such as CM or SlimBean, or even the deodexed stock ROM and see if that makes a difference. You can flash back later and use triangle away and unroot the device to return its warranty.

I flashed stock firmware from sammobile and i wont root my device and try non stock rom. So i think my warranty isnt void now. In previous firmware have camera problem too. I think it s a software problem and waited next firmware. But not solved.

I went to Samsung Repair service and they repaired my phone in 3 days. Now, front camera working well.
I could sure use some help in this one. Not exactly sure what to do. My phone was running Android 4.2.2 rooted with TWRP (2.6.3.3). I saw that the OTA update for Android 4.4.2 was available so I went ahead and downloaded it. Once I tried to install it it rebooted into recovery. I tried rebooting the system, but it takes me back to recovery. It won't do anything else. I tried getting into download mode, but it won't do it either. Someone mentioned that it may be wiped, but the memory on the phone still shows all the files in there. Not sure what went wrong. If anyone has any ideas, I'm all ears. Thanks.
Just flash back to an old nandroid for now. You might have to be unrooted or use voodoo root keeper while you update. Idk tho I am just reading up on it now. Were you just rooted on a stock ROM? Or using xposed?

You shouldn't take ota if you have a custom recovery installed.

If you plan on unlocking the bootloader, you shouldn't update. LG has a reputation of permanently locking the bootloader with their updates.

On the topic of the update - does anyone know what exactly changed with it? Probably the only thing I noticed was that you can now choose between Dalvik and ART. But that's about it. Anything else?

I'm going to hold off on updating. I remember that I updated my L9 as soon as I got it and I never was able to unlock the bootloader. I'm doing it differently this time....just in case.

Not sure about the D405, but on the D415 after enabling developer options we can switch between ART or Dalvik. I haven't updated or looked for an update for my D415.
EDIT: Just checked for an update for the TMobile D415. There isn't one.

That may not always be the case. LG is well known for purposely screwing things up for folks.

Well, I already did update my D405N, so I've already seen the Dalvik/ART change. I was just wondering if anyone knew what else has changed, because so far I haven't really seen anything all that different.
I downloaded the 150mb update and rooted my phone with towel root afterwards, everything's working

Will ghost roms work on the XT1060? I know that technically the XT1060 is classified as ghost_verizon and while I know that hardware and software wise there is no difference I'm wondering if there's a difference in radio. The reason I ask is because I haven't been able to get the CM12 nightlies to flash but I'm not sure if it's just because it doesn't fall under ghost or if it's an issue with TWRP or something else.

Were they the XT1060 or ghost roms though because cm makes XT1060 specific Roms as well?

I use the Ghost nightlies and they work well. Most people have to flash CM11 before flashing CM12. If you haven't had CM11 on your phone (you don't have to set it up) that may be the problem.

It's been discussed in other threads, but can't recall the exact reason.
However, I did try from stock 4.4.4 to cm12, and that failed. Thus, I installed cm11 first, let it boot to home screen, then rebooted to recovery and installed cm12. Haven't looked back since (2+ months).

Alright then I'll take that advice and give it a shot.
Today the notification for the official update to 5.1 apear on my Moto X. Last week i rooted the phone just to block ads. The update started the installation but it fail. I tried 3 times, and all was the same.
What can i do? I want to have the new version of Android.

If you are gonna use OTA update, your system must be unmodified : not rooted, not changing any system file (including boot logo), and using stock recovery.
