Background:
I switched from AT&T to Fi a couple of weeks ago. Only since switching to Fi, my LTE connection has been unstable (constantly dropping to 3G, 1X, then nothing). Yes, I know that there are multiple threads on this problem, but I have a new (I think) question.
I contacted Fi tech support about this issue and they replied as follows:
"Thank you for contacting Project Fi Support! My name is Duane and I will be your Project Fi expert today. I completely understand how frustrating it can be when your device does not work correctly when you need it to, but we will work towards getting it resolved today. Ok, I have something that may help your problem here. This makes it so your choosing LTE only and that should fix your problem. Follow these instructions here:
Dial *#*#4636#*#* from the default dialer
Find where it says "Turn off Radio" and tap it
Right above where you made that selection it will say "LTE/GSM/CDMA auto (PRL)"
Tap that and select "LTE only"
Now select "Turn on radio"

And it works. Connection is stable.
But here's the problem. After a few hours, or a reboot of the phone and it switches back to LTE/3G/GSM/CDMA. Then the original problem returns.
Is there a way to force LTE only to stick?
nope, its always been like that. you have to reenable it after a reboot. ive been reenabling now for 3 years

I'll have to pay more attention, but it seems to me that it switched back after several hours on forced LTE, without a reboot. Have you ever noted that behavior?
lte only disabling without a reboot is somwthing new, and i have only experienced that in m(and 5.1.1 maybe).

yes, just as you posted, i posted. thats an odd new behavior. and it doesnt always happen. it does seem to happen when your lte only connection gets "stressed".

Sorry guys, this is an elaborate one.
So I was running the the 10/19 CM 10.2 nightly and I wanted to fix the GPS issue (and the LTE issue, I feel like I should include that because it might have something to so with the problem). Anyway, the GPS fix asks you to flash back to a sense based rom, download a GPS status app to reset and download new agps data, update profiles and PRLs and get a lock and you're good. After a DAY of trails and errors, got back to stock and fixed my GPS, GPS works fine now. The LTE fix asks you to open of the APN settings using the voice dialer and changing it to one of the LTE ones. I did that, LTE works fine now too. So just when I think I'm in the clear and completely stable, I try to text a friend and the test won't send. I try to call my house and the call just instantly drops, I try to call my phone from my house and it goes straight to voicemail after like a half a ring. I'm starting to think something is just wrong w/ sprint where I live but I don't know. Anybody know what's going on?
So I think I'm just out of luck until s-off becomes available w/ hboot 1.55 or they make a RUU for 3.04. I've tried everything. *sigh* Good thing I don't talk to people, lol.
I'm in the same boat. I tried flashing beanstalk. I noticed I'll have data only if my SIM card is in the phone. If it's not I won't get any data.
Took it to a Sprint store and they fixed it, I thought I was gonna get a new unit for a minute but no, they just fixed it. Funny thing is, my GPS and LTE are messed up again, but I'll live. If that Nexus 5 is under $400 I'm still gettin' it tho...and flashing back to stock (We should have a 3.04 RUU by then) and selling this with the quickness...

Alright so this is what reenable voice calls. I downloaded omjs ROM from august ( really any stock ROM should do) so I would have the ability to use all the programming codes and update the profile. Obviously do a full wipe( system,data,cache,dalvik cache) loaded the ROM once it got settled and I went past the initial startup stuff I did what's called an "SCRTN" (that's ##72786# on your phones dialer) then hit the three dots and confirmed I wanted to reset the network settings. Also an "RTN" (##786#) won't work because of the custom recovery in case anyone was gonna do that instead. Let the device come back up it should auto activate and go through the hands free activation on its own if it doesn't just go to settings and update the profile you can do the prl too while your doing it just to make sure everything is working fine. Then reload this rom and toggle airplane mode on and then off and there you go. Now this worked for me it may not work for you just keep that in mind but hopefully this helps until they find out what may cause that issue.

Also I forgot to ask did you try taking the sim card out and putting it back in? One other thing you can try is taking a pencil eraser to the back of the sim card ( the golden side) and putting it back in( clean off any pencil shavings) but see if that helps.

Yesterday, my 2013 Moto X (international model) started behaving VERY strangely. My 3g connection just died, and I stopped being able to make or receive calls - SMS works fine though, and my signal is very strong.
I can't make calls, the call just dies before even trying to ring, and people calling me just go straight to voicemail. Also, in the status menu my Network Type and Phone Number are listed as Unknown. I also tried rebooting into "Factory" mode through the Bootloader and found that my phone ICCID is also listed as Unknown. IMEI is fine though. It's so weird and frustrating.
I called my cellphone service provider (Movistar Chile), and they reset my network profile and gave me a new SIM card as well, to no avail whatsoever. The problem still persists even after a reflash (through fastboot) of the stock 4.4.4 firmware.
Is my phone dead? Or could it still be a solvable issue, be it network or phone fault?
If sms works and not phone, it's not your radio connection to the carrier, so sounds like a carrier issue if reinstalling firmware didn't help.
Did you do anything at all to your device before this happened? Though reflashing firmware should have corrected anything.
I'd be calling the carrier again.

No, that's the weird thing. I hadn't done anything to the device before it happened, I was literally just chatting through the Facebook Messenger app when my data connection died and it hasn't reconnected since then.
I did call my carrier again, and they issued a ticket to tech support, so I guess I have to wait for their resolution as well.

Precisely. I can SEND and RECEIVE text messages, but I can't MAKE or RECEIVE phone calls, and data is completely disconnected.
Not sure about phone, but for data... Google your carriers APN settings and see they match yours. In mobile settings, access point names.

1. Yes, I already tried that. APN's are correct, I deleted and re-entered them just in case anyway. No change.
2. Yeah, I actually reflashed twice with different stock firmwares for different carriers with fastboot and mfastboot.
Well, it's sounding like hardware failure then unfortunately.
The only other thing to check, is that you have the correct radio installed. If you flashed stock, you should... But I'd double check. I don't have the answer to which is the correct one for you, you may need to search on that, or hope you can find someone here to compare too.

If you are within warranty period... You may want to go that route.
With sms working, I still suspect sim/carrier issue... But if they are no help, what can you do.
Yeah I know, I starting to fear hardware failure myself. I guess I'll wait to see if the carrier fixes it, otherwise I'll have to get a new phone :/
It could also be an issue with your SIM card. Maybe try turning the phone off, removing SIM and booting. Then Turn it off again and re-insert the SIM and boot again. You can also see if your local carrier store has another SIM card that they can test it with.

I got a new SIM card from my carrier yesterday, after I complained. Problem persists.
Finally, I took the phone to the official Motorola service where I live and they confirmed it was a hardware fault, and as I had unlocked the bootloader I'd have to pay an outrageous amount for them to fix it (motherboard had to be replaced). So I decided to get a new phone instead, and now I happily own a brand new Nexus 5.

Don't know if this issue has been addressed but my Nexus 6 is 2 months old. Recently no one I call can hear me but if they call me back everything is fine. My phone is completely stock running 5.01 on Cricket Mobile with LTE working everywhere I go. Any thoughts or suggestions? I bought this phone straight from Motorola if this makes a difference.

Well, I've been searching the Q&A forum for this and at first, I haven't found what I'm looking for. I know I've read of this issue before though. If you want to try to get the OTA, here's a simple way:
Power off your Nexus. Remove the Sim card. Power on and let it sit for a while. Reminder, you won't be able to call or receive calls for this time. The OTA should appear shortly, especially if connected to WiFi. Install OTA and let the phone do its thing. Once all is said and done, reinsert your Sim.
Yeah, I have seen this problem on different forums myself with really no clear cut answer. Thanks I will try that for my update and let you know how it goes as far as the calls. Thanks!

Cricket is an MVNO for Sprint. I'd suggest a search for Sprint problems similar to yours.

Go to Settings>more>cellular networks>carrier settings.
Do you have the option there, to update PRL and device config? If so, update both if you have not.

I have no option for updating the prl. Also it gave me no options. I typed the code in and then it just disappeared from the dialer? By the way, thanks a bunch for trying to help me figure this problem out!

No problem at all. Then it appears the Sprint issues do not apply here. Also, in settings>more>cellular networks, is your "Preferred network settings" set to global or LTE (recommended)?

As it should be, but you could try switching to global to see if there's any improvement. I think the OTA may solve this as well.
There is no option to switch back to global. After a factory reset it is in global mode if changed to LTE it will not go back. Also, I'm at work so WiFi in my part of the plant is very weak so should I wait until I get home to do the sim pull and then let my phone try to receive the OTA maybe overnight on my home Wi-Fi?

That would be ideal. I'm sorry we couldn't get it straight but I'm confident that we tried all of the stock options available. Your bootloader is locked, right?

So I did what you said as far as powering off, removing Sim and powering up. Let it sit all night like this connected to WiFi and still no OTA for 5.1 Also factory reset this morning and now I can't even receive calls and people hear me. Beyond frustrated!

Are you able to contact the provider and have them reactivate? That might be necessary. You may want to consider unlocking the bootloader, at this point, since you've already factory reset. You could then flash the factory image.

I have a G4 that refuses to come out of airplane mode. The device actually appears to be fine with the exception of no signal or data. It will work on WiFi. I have already used ##72786# and an actual ##RTN#. Still having the same issue. Brand new so i cant post pictures as of yet...

How was it activated? You may need to call Sprint.

- If you go into settings, then general, then system updates and update prl or update profile, does either do anything?? Have you pulled the battery, held Volume up and while holding vol up pressed power until you are in hard reset mode and reset it this way and had it go through setup again, including activation?

I just took the OTA to ZV6, everything worked perfect until after update. If I leave wifi on I can update prl and profile, if I switch it off the updates fail but the signal indicator stays empty. 3 other sprint phones in the house as well all working normally. I've also done a hard reset with volume+power, same result and I've flashed the zv6 tot with lg flash tool same result. I downloaded a app called lte discovery and it says that the radio is disabled.

When the phone first boots, right when it hits the lock screen "service disabled" is briefly on the screen then disappears. The signal indicator is blank like it has nothing at all. Settings>Network>Mobile Data checked, network is in global when I click connections optimizer it says "setup has not yet finished" This is the current situation after flashing zv6 tot. If I go to General>System Updates> selecting any of the below options I get a "you cannot update while the device has no service" If I turn WiFi on I can select firmware, update profile and update prl. Update profile returns a "the update is complete" Update prl returns "The prl has been upgraded, press ok" If i go to Settings>Activate device it returns all the information about my account like usage and plan.
It's a known issue. You'll have to get it replaced. Hardware.

Thank you for this post I had the same problem and was pulling my hair out with sprint. Got insurance but 200 bucks. Still under LG warranty so will try that.
Any new developments in this? Having all the same problems and dont want to have to wait for the store to open tomorrow. Ive narrowed it down to the radio being disabled for some reason.

Having the same issue with the phone seemingly stuck in airplane mode. Anyone find a fix to this? I haven't tried a factory reset but done everything short of that. Does the phone get replaced under the LG manufacturer's warranty?

I am having the exact same problem. All the conditions as above, plus i did a factory reset. Still doesnt work. I put in a working sim and it still doesnt get service on my lg g5. When I try to make calls it says turn off airplane mode. Someone please help.

Can anyone tell me specifically what hardware needs replacing? I have an LG V10 with the same problem. I also have a spare non-working LG V10 I can use for parts and am willing to do the work to replace the part if that's all it needs. I would really hate to have to buy a whole new phone. Thanks.
I got my V40 remotely unlocked and rooted. Having issues with BusyBox saying it's not installed. I'm not sure what device I should say I'm using on TWRP.
More importantly, I no longer have wifi calling and that renders my phone useless to me. At home I have no service so cannot call or text.
Can I use another ROM or a stock ROM to get the wifi calling and retain the unlocked bootloader and root?

Try enabling it in the svc menu and see if the setting for it comes up. Y'all are on US carriers so I can't really help much, not familiar with most of 'em. Short of it is, some options only show up when carrier and phone model jive. i.e. carrier supports the setting and phone model. Some are just simply carrier specific, meaning if you're not using the carrier fw, you're never gonna see the setting.
Sometimes you can get the options to pop up with a couple build prop tweaks. Ive used am on my mate se and my S8. But even then its hit or miss.

I'm not sure if you figured it out (I'm still trying to get wifi calling back), but I found a great visual voicemail app called youmail. It lets you do a lot of neat stuff the stock visual vm doesn't.
