Error of OS when used headphones

Hello.
I just found a strange error in my L820. When I connect the headphones to the jack, the volume level goes to 0 and I cant increase the volume level. When using the back button only refresh the volume bar at the top of the screen. The only way to fix this is making a softreset. Had any one else experienced this situation?

Audio bug

Did you notice that when you press volume up to raise the ringer volume, the step 6 7 8 9 10 have the same level and you have to reboot it, have you noticed that?

Speaker volume fading out

Does anyone else have the issue where the speaker volume fades out and then becomes mute. Let me explain more:
Since about yesterday, whenever I'd use an app that requires sound (Spotify, Soundcloud, Youtube, Facebook videos, etc), I'd initially hear some sound (depending on my volume setting. After about 3 to 8 seconds, the volume fades out for no apparent reason. I'd check to see if the volume level is changed, but it remains at my set level but there is no sound. I max the volume and still no sound. This doesn't seem to be a hardware issue but rather a software issue.
Yes! I have to restart my device. Then it will work for a while. This is the first form I've seen talking about this issue. Must not be a huge issue. I have an S8 plus.

Volume reducing automatically.

Is anyone else having the same problem? I will launch YouTube or any music playing app or even the ringtone but the volume will play at normal volume for like 10 seconds then automatically level down drastically. Any solution will be greatly appreciated.
P.s its nothing to do with the buttons bcoz the volume slider still displays full volume. and if its the android volume protection feature how do you disable it on the device?

Sound Assist App - Broken Volume Steps

Has anyone used Samsung's Sound Assist App on the Note 9 before? I was intrigued with the option that gives you more volume steps but noticed that it doesn't scale properly.
For example setting volume steps to 1 increment, I have to raise the volume 10 times before the volume actually increases. If I set it to 5 increments, I have to raise volume 3 times before any change. Anyone else have this issue?
