I feel like i didn't have this issue until i flashed a stock based rom. I had been on AOSP roms since i got the device.
I'm on cm12 nightly and before this I was on chroma ROM

No idea if this will work on the Note 4 or not. But go for it

On my note 4, the s health app updated automatically to the same version as on the s6. You should go to galaxy apps and tap purchases in the hamburger menu to see if there are any updates.

T-Mobile version Note 7 here. Just got my replacement phone from T-Mobile and processed all system updates. Unrooted. I have it under settings>display>resolution.
