Galaxy Note 4, unlocked T-Mobile on MetroPCS unlimited plan.
Ok, here is what I did
I attempted wiping everything in the advanced using TWRP 3.0.2 for the T-Mobile Note 4, to no avail.
I reinstalled the N910TUVU2EPE3_N910TTMB2EPE3_N910TUVU2EPE3_HOME.tar.md5 via Odin
Booted into the stock recovery. This was an Ah ha, moment. the Stock recovery was able to wipe the encrypted partitions.
Re-installed via Odin.
Rebooted this did not prompt for a password or pin indicating the internal storage was indeed cleared and installed without encryption
I used the following to make my SD Card "adoptable"

I then rebooted and installed TWRP via Odin
Rebooted and wiped using TWRP and began the reconfigure of the phone.
Allowed the restore to complete and during the setup I was able to confirm VoIP works. See attachment, made a call to confirm.
All that is left is to root using the following threads:

This worked for me, the ROM is not debloated and has all factory programs.
Uploaded a new screen cap to show the option to encrypt the device.

Need help here. My tab 8.9 is running on Honeycomb and I have never set any security password for this unit, nor was it rooted before
Recently I have decided to root and install CWM in order to upgrade the OS.
Everything went smoothly and I managed to root the unit and verified it with Root Checker app.
Then I proceeded to install CWM and folloed the steps prescribed in this thread:

The installing was smoothly, but when I boot up the unit, after the samung bootup splash screen, it goes onto a screen prompting for password. I have never set any password at all so how do I resolve this problem? In the above thread, there is no password feature mentioned as all. Please help as I have no choice but to post this question here and not in that particular thread becos I am a new member.
PS: I am able to go into CWM recovery, and I did a factory reset, but it is still the same.

That is quite odd. You can try installing a custom rom. Put the rom.zip and gapps.zip on the sd card. Wipe(format) cache, dalvik, data, and system. Flash the rom, then gapps, then reboot system.
If you don't have another custom rom on your internal storage, you'll have to install an original rom with odin, then put your preferred custom rom on sdcard, and repeat the root-procedure, and then install the custom rom. Yes that sux.
That password stuff sounded iffy.

Hello, I'm kinda new to the android scene.
Yesterday, I wanted to restore my phone to factory settings but the s3 mini toolkit said that usb debugging was not on (but it was) so I thought I could just wipe everything using TRWP, boy was I wrong.
I tried to flash my phone back to stock via odin but my computer won't even detect the phone(uninstalled and installed phone firmware). I tried placing the md5 file into the microsd in hopes of flashing it via TRWP but it's not recognized either.
Did I brick my phone? lol
Phone does not recognise microsd or md5?
In first case, try ADB and push some custom rom to phone. Try with universal naked driver.
In second case, try .zip custom rom.
But if it is like TWRP for ASUS TF300, then wipe everything means everything, so you're doomed.

Do you even get a bootscreen? Its possible to factory reset in twrp but i guess you wiped the system partition too
thanks guys! it seems I solved the problem through further use of google
I just reflashed the rom that I was previously on with TRWP and everything works now woot!

Device: i9305
ROM: sentinel v5.0
Recovery: TRWP 2.6.3.0
Timeline:
1.ROM flashed and everything working fine
2.Phone encrypted using Settings->Security->Encrypt and Exchange email account installed. (battery drain became very high)
3.Boot to recovery but TWRP not able to mount /data (says "password failed" even though it is the right pass.. I performed many system reboots and every time the phone decrypted and booted). Only recovery not doing it.
4. attempted full wipe, format data, etc.. but it always failed
5. attempted Restoring previous nandroid (made before encryption with TRWP of ROM Rremix 4.07). It seemed to work
Current Problem:
1. On boot phone asks for password to decrypt (the decrypting little green android image shows up) but it then freezes during ROM boot
2. Cannot access recovery (HOME+VOLUME UP+POWER)
EDIT 3. I can access download mode and connect to PC and Odin recognizes it
Question:
How can I simply format the internal memory to then re-flash a recovery and ROM?
Or any other suggestions on how to get the phone to work again?

I got the correct stock ROM for my phone from SamMobile (I9305XXBMD4) and Odin flashed it (PDA option).
Odin said PASS.
However, during boot it still asks for encryption password (and the right password is not accepted). Recovery is also still not accessible.
So, I need a away to get ODIN to really format the internal SD or similar to actually remove the encryption. So, here comes the question: will Re-partition do it (correct PIT file needed and instructions)? And I know using it is half-way towards a completely bricked phone... so I accept suggestions (flash count is not 1 so no warranty service options yet...)!!

If you mess with partition or Pit you are likely to brick your phone with no warranty .
Its not just the rom counter that voids warranty .
Flashing Philz Recovery via Odin may allow you to format all .

Thanks for the idea JJEgan.
I did install the Philz recovery via Odin and was able to select "Mounts ans Storage"-> "format /data and /data/media". This one worked with no errors.
Tried "Wipe Data/factory" reset and it also worked (no errors on not being able to mount /data)!
Then Odin Flashed Stock ROM (taken from SamMobile), reboot and it again asked for password! Removed battery and this time I could access the stock recovery.
Did wipe data/ factory reset + wipe cache.
Reboot and it works!
Now I just need to Root and enjoy the phone again!

If you have encrypted the phone with stock based ROM, you can also decrypt it. TWRP is not able to mount partition encrypted with Samsung's proprietary way of doing it.
If you have encrypted the phone with AOSP based ROM, and TWRP is not able to mount it, then something went wrong.

@!crazy: I encrypted while using AOSP Sentinel v5 ROM (see OP). ROM wise the phone seemed to be ok but TRWP recovery simply could not mount /data. Indeed, something went wrong.
In any case I went back to a stock based ROM to have full encrypt/decrypt compatibility because of Exchange email accounts I need to use.
Hello everyone......I had my phone rooted with towelroot and then installed Safestrap......I loaded Unity Rom and it ran, but when I rebooted all I get is the Galaxy S5 Active screen, no custome padlock, I went into safestrap again and did a factory wipe.....nothing......tried to reload stock firmware through Odin.....now I can even get into safe strap.....how to I get back to factory.......
Your best bet is to Odin back to factory

@jinx0117 you tried to Odin back to G78oA correct? Let me know exactly what you did? Did you do a complete system wipe including ex SD card?
After using Odin to flash the stock NE4, I rebooted and then used towelroot to root my phone, then shut it down and flashed the update of OA1 using Odin. Let it reboot. I put Safeatrap on my SD card, went into the phone and loaded or installed it from there. Rebooted into recovery, Flashed 3 mint into stock slot, and rebooted. A screen came up with a sad looking robot saying Safestrap was not enabled....and I hit continue........Unity Rom I downloaded and put on my sd card into the stock slot, which I though I wiped, loaded Unity Rom in there, then tried to reboot, got the safe safestrap screen but this time said enabled and went to Galaxy S5 Active Screen......and it has been there ever since. I can no longer get into Safestrap, I tried a full wipe through it before it dissipated....Tried to factory reset through the recovery on the phone but that did nothing as well. I can only get to the Odin area and The stock recovery area......so currently I am without a phone....It doesnot even charge......

So I was running a custom rom for months now for my N910T without issue but yesterday the phone suddenly rebooted by itself while I was using it and would keep rebooting roughly every 3min.
I backed up everything in TWRP and wiped everything to reboot into a clean rom like factory reset but that didn't work as everything was still there and nothing was wiped. So I tried to flash a new rom but it failed and gave an error E: cannot execute upgrade binary.
I'm on TWRP 2.8.1.1 so I can't flash a TWRP update (have to be on 2.8.4 or above) and thought my recovery was too old. I then plugged phone in download mode and used ODIN to flash a new TWRP. It would write fail every time, no matter what I changed. I tried to install different versions of recovery and used different versions of ODIN with no success.
I also changed different cables and ports. I then tried to flash a stock rom through ODIN but it also says write fail. Basically I tried everything and searched on XDA and google and tried every method I came across. Anyone have this issue or suggestions?
tldr: Phone N910T reboots from home screen, write fail flash from ODIN and fail flash from TWRP recovery. Can't use phone because it would boot to home screen for a few min then just reboot again. Help!

Yes I tried every wipe option possible. Wipe system, boots right into rom. Wipe internal, everything still there. Wipe data, factory reset, everything still there.

Then something I'm your phones been corrupted probably, your going to have to rma for a new device
has anybody come up with a solution to this problem im having serious boot and loading issues. i have flashed many roms and have never had a problem. i odin flashed the cog2 firmware then twrp flashed the tekxodus rom. something happened changing permissions with the built in setting with the rom and twrp was lost and i cant get back to a usable phone. odin flashed everything i can find for my phone and its not working everything either falis or goes to loop.
You probably corrupted your partition doing something wrong
