Is it a new phone? If not, where did you get it from? Is it stock? Custom ROM? Rooted?
Full wipe > reinstall ROM.

If you click into the .rar file you will see a file called androidinfo.txt i think
You can edit this using winrar. You can change one of the model ID's to the file, save it and then reflash

Indeed, a RUU would be the first thing to try. Its a well known issue to loose baseband/imei, its either caused by a software incompatibility (a will be able to fix that) or its an hardware problem. If your problem appeared right after flashing a rom then its most likely a software problem only.
The 7.19.401.51 RUU is the lastest version for your phone (see sticky threads in the general section).
