I have the following BIG problem I need help with
I tried "Darky's Raw Steel Edition" JP6/JPM kernel (from here: http://forum.xda-developers.com/showthread.php?t=814091)
For some reason recovery 2e is not working -- I cannot get the phone in recovery mode but I can get it in download mode.
Problem: I tried "adb reboot recovery" and now the phone is stuck in a loop.
I can get into "Download mode".
So, which image can I use to get out of "recovery mode" reboot ?

If you can get into recovery you will be able to reflash through odin, had a similar problem and this worked for me, reflashed back to JM8 and then back to JPM.
I think JM8 has recovery 2e.

I can get into "download mode".
However, in that kernel I had to install ClockWorkMod Recovery
Since then I can only get into "download mode" but not "recovery mode"
So, question is: How do I get into "recovery mode" now ? And, more importantly, why cannot I do that anymore ?

Hii all!
I've made a new post to explain thing better. My main problem is that I can't get into my recovery menu.
A few weeks ago I've successfully rooted my Milestone (bootloader 90.78 > vulnerable recovery From this post > Copied OpenRecory + update.zip (G.O.T. OpenRecovery) to my SD card) But then i wanted to install a custom ROM. So I thought it was good to start over with a clean phone, I followed the "[HOW-TO] Get your Milestone back to stock" guide. I downloaded the following .sbf "SHOLS_U2_02.36.0_UCASHLSMR2EMEAB1B806D.0R_PDS003_USASHLS00RTGB_P026_A019_HWp2a_1FF.sbf", flashed my phone with this .sbf, all went fine. Then the vulnerable recovery needed to be installed, I've tried several vulnerables. But after completed the install I cant get into the recovery, I'm stuck at the /!\ screen. Can someone help me?

I tried with the phone as I got it (no flash nor anything) - Could enter recovery mode (Power + CAM) but no menu display with VOLUP + CAM at the same time nor first VOLUP and then CAM.
I tried installing openrecovery and also androidiani, but after flashing vulnerable recoveri 90.78 and starting in recovery mode the same problem, no menu.
I flashed G.O.T telus 2.2.1 and still the same problem
Any ideas?

A few points:
You should ALWAYS be able to get to the default recovery. The default
recovery is not useful for flashing ROMs, but if you can't get to it, you have serious issues.
The default recovery can be accessed by powering on the phone with camera pressed down until you see the exclamation/triangle icon/cell phone icon.
RELEASE all buttons.Then press VOLUME UP (leaving it pressed down) and hit the camera button again (once). It should INSTANTLY give you a recovery menu.
You should now be in the RECOVERY (not OpenRecovery). You should a small number of options : reboot, apply update.zip,etc.
If you have not correctly installed the Vulnerable Recovery, you won't be able to use the apply update.zip option to run Open Recovery, Androidiani, etc.
The following thread is the best guide to get a recovery installed. Follow it exactly.

The best recovery for Milestone is Androidiani. GOT recovery is OLD. OpenRecovery works fine, but Androidiani has more options for Milestone.
The general steps are:
1) Flash a GOT sbf using RSD lite. This installs vulnerable recovery
2) Download Androidiani. Put its update.zip in the ROOT of you SDCARD and put the OpenRecovery FOLDER (unzipped) also in the ROOT of the SDcard
3) Enter Recovery (as described above)
4) Choose Apply update.zip to get into Androidiani
5) Follow instructions for the ROM you want to use...
I hope this helps! It has worked on thousands of milestones. I see no reason for it to fail on yours.

Post the video and we can see if you have a problem.
Did you try starting from scratch using the appropriate GOT sbf?
The recovery gets overwritten by that sbf, so if this is succesful, any corrupted recovery problem should go away.

Try plugging your usb cable between your pc and phone, and then enter recovery

I flashed GOT Telus 2.2.1 full, at least I've got froyo now, but still no recovery menu. I also tried without SD and SIM to check if something could be wrong with them but no luck.
I'll make the video in some minutes (no sound, I'm at work. I tried to make it yesterday but my daughter thought it was better to play with the computer instead)
To eneter in recovery mod slide out keyboard and pres X button + power button and release them when you see the /!\ then apply update.zip

I think that only works with newer bootloaders. I've got 90.73.

im suffering the same issue.. i get till the triangle but "vol up + camera" does nothing - through the buttons work within android itself
i've tried GOT and the vulnerable-bootloader-only SBFs
bootloader version is .72
any ideas?
Well, at least I'm not the only one. I'm thinking about trying motorola service...

I have the same problem, bootload 90.74
I'm in a messed up situation. I flashed JVB(deodex.) directly from CWM (Darky 9.5) and that installed fine, but bootloaders didn't. Now I get a funny bootloader screen. But that's not the problem. The problem is that I cannot get into CWM or DOWNLOAD mode. CWM actually works, becouse when I enter it using 3 button combo, phone starts, and I see messed up pitcure (see attachment) and When i press power key (select key) phone reboots which is the first option in CWM. But i cannot flash or do anything becouse the screen is corupted.
I now want to flash a stock JVB using odin. I go into download mode, but my pc doesn't recognize my sgs. I says downloading, I connect with odin open, but windows 7 says: "USB Not recognized" and "Windows has stopped this device because it has reported problems. (Code 43)" .
Tried on 3 PC's, it won't work. My brother has another sgs, and his download mode works on all 3 PC's!!! So the problem can't be in the drivers!
So the problem is:
-> I can get into download mode but 3 PC's won't recognize SGS! (see attachment)
Second problem is:
-> Becouse I didn't update bootloaders when flashing JVB, i can get into cwm but I cannot see anything becouse of corupted screen! (see attachment)
What I did:
-> Darky 9.5 (2.2.1)
-> Flashed JVB Deodex using CWM
-> Flashed Chainfire 3.1 with CWM using ODIN
-> Download mode doesn't work anymore
-> CWM does not work anymore due to corupted screen/bootloader
-> Flashed Supercurio Hacked Kernel with SGS Kernel Flasher -> Problems remain.
So the bottom line is:
I can boot into JVB, and it works fine except some small isuess. But I can't access CWM or DOWNLOAD mode. Is there a way to fix CWM? Update BOOTLOADERS? Fix DOWNLOAD MODE? Is there alternative flashing method that i can use directly from working JVB rom? I can't be stuck on JVB without the option of CWM or ODIN!

So my phone was working before it got stuck in a boot loop. However, the download mode did not work. Whenever I entered the download command into adb, it would restart as if it was going into download mode, but then it would reboot again.
Ironically, it is able to go into CWM recovery when I enter the command. I tried to do a factory reset on the phone in CWM and reflashed CM7 and now I am stuck in bootloops.
Every loop it displays a quick message in recovery "E: Can't mount sd card".
Someone please help! I am going to get a jig, but is there any other option besides that?

Also, my 3 Button Combo doesn't work...and I am assuming my warranty is voided so I can't send it to Samsung?
In response to your Samsung question, yes, you cannot send it to Samsung, as the modifications have made it non-warranty. Sorry to have to say that.
On the positive side, a jig should be your best bet. That should kick you into download mode where you can flash back to stock.

I just tried to root my Note 4 (SM-N910F) following the instructions of: http://forum.xda-developers.com/note-4/orig-development/sm-n910-cf-auto-root-t2897428
The process has been completed with "Pass!". However, when the phone tried to reboot, the following massage appeared in the top-left corner:
"RECOVERY BOOTING"
"RECOVERY IS NOT SEANDROID ENFORCING"
"SET WARRANTY BIT: RECOVERY"
In another thread I read that I should try re-flashing and uncheck "Auto Reboot" in Odin, and enter recovery by pressing Home+ Vol Up + Power. Unfortunately, the result did not change.
I was not able to enter recovery. The phone just rebooted and displayed the message above.
Any ideas what I should do? Any help is appreciated.
So, I managed to fix my problem.
I downloaded TWRP (the tar-file) and flashed it with Odin. My phone reboots again - however I do not have Superuser, so I guess root did not work after all.
Thread can be closed.
I believe you have to flash the superuser zip file in the root instructions with twrp.
Edit: I tried to find for you but I forget where I saw it. Or as a try download the app and also root checker to see of it workes

Boot into TWRP and flash the superuser.zip you can get it on the cf auto root site.

A newbie, I ended up lost after trying to flash the latest CM 12.1 on my Samsung Galaxy S4 Active GT-I9295.
At first, there was an issue with the recovery, which upon entering showed black screen instead of the tool itself.
Trying to fix this, I managed to remove the recovery through flashing the Stock ROM, which has failed in Odin on the "system.img.ext4" instead - could not paste screenshot link here, feel free to ask for it, please!
I must say that I have tried hard to figure out what to do with the mess I have caused, but now I am helpless.
Would anyone give me some nice piece of advice to get my phone going again?

Did you try odin 3.10 by any chance? That could be the problem. Use odin 3.07 or 3.09, boot manually into download mode with complete shutdown & using vol down + power key and odin the stock ROM, reboot and do the same with TWRP (hold down the vol up key after flashing recovery when it is rebooting)

Yes, I did exactly as you say - got to the download mode, plugged in, ran Odin (tried 3.10, 3.09 and 3 v1.85) started flashing the latest Stock ROM and the I only got as far as the "system.img.ext4", when the download failed. Sometimes that happened even earlier, saying that there was no PIT etc.
When trying to flash the CWM recovery, it goes through OK, but then when trying to get into it, the screen just goes black, as described in my previous thread - "CWM or TWRP Recovery Black Screen".
I am stuck.
But I still hope that I am not a proud brick owner

Are you using the original USB cable if not then try with another cable. Also what is the device name shown when you boot into download mode

Hi. Try this:
Remove the battery and USB cable from your phone, and wait at least 3 seconds.
Place the battery only back in your phone.
Fingers on volume down and home, then another on power. (cold boot directly into download mode - this is important!)
Press up to enter download mode.
Insert USB cable into phone and lie it down somewhere it won't move or be touched during the flash.
Download a good current copy of Odin from:
https://idlekernel.com/flash-tools/Odin_3.10.7.zip
Extract the archive and run Odin.exe
Download this emergency recover tar for your device (newest firmware revision available as of right now):
https://idlekernel.com/flash-tools/recover/BL_I9295XXUDPA4_RECOVER.tar.md5 (also has official TWRP 3.0.0-2)
Insert "BL_I9295XXUDPA4_RECOVER.tar.md5" in the [BL] slot.
In the Options tab put an [✔] in the "Phone Bootloader Update" checkbox.
If you don't have anything you need on your internal storage, put an [✔] in the "Nand Erase All" checkbox.
Click [Start].
Once the progress bar is near finished, place your fingers on volume up and home to enter directly into official TWRP 3.0.0-2.
If you used the "Nand Erase All" feature earlier, you will need to go into Wipe -> [Format Data] -> type "yes" for your data partition to be usable again.
Hopefully this will get you past your black screen issues.
PS. Remember to flash stock boot.img with your system.img.ext4, as you cannot boot a ROM without a matching boot image.
