Please if someone can tell me what is the problem and how can i fix them.
You have mentioned reinstalling (reflashing?) the ROM but you haven't mentioned a factory reset so I have to ask... Have you tried a factory reset?
Should i do factory reset on rooted phone? if yes how can i do that. I read somewhere that factory reset should not be done on rooted phones. is that true?
In CWM, You always have to Wipe your Data,Cache,Dalvik Cache,Battery stats.
Optional : Format System.
For a rom to function well, else it will get all kind of problems, like you have.
Of course if you install a update for a ROM, you only have to wipe the cache/Dalvik Cache.
Remember to make a Backup of your apps with Titanium Backup.
I didnt wipe Dalvik Cache and Battery stats. I'll give it a try. What does Format System do?
The factory reset basically resets your phone to as it came from the factory. It erases all your settings and apps, and is recommended that you do that when you change from a stock ROM to a custom ROM.
Having root has nothing to do with it.
Me personally I like backing up my apps and doing a factory reset each time I change/update my ROM. This way I make sure I always have a clean system each time. Of course this might not be convenient if you like flashing a new ROM twice a week
Wipeing Dalvik Cache, Battery stats and factory reset didnt do the trick. I flashed cyanogenmode nightly now and its ok for now. Could it be problem with the rom?
"Factory reset" (/data wipe) doesn't wipe system apps. If you had on your old ROM some "broken" system app, you most likely still have it. First thing i would do is full device wipe (/data and /system partition wipe). If you dont know how to do this here is the script and how-to. Be aware of that this script can brick your phone (ofc then you can fix it with smartflash) is you won't follow instructions.
After that wipe and ofc flashing clean ROM (btw. which is it?), it still isn't working, it's most likely ROM's fault. It could be Baseband<->RIL problem eaither but i've never saw RIL problem which has influence of system stability.
Are you flashing back to stock or a custom ROM?
If you are flashing to a custom ROM, you need do all the wipe-steps and then do BacktoEXT3 before flashing the custom ROM,

Just have to take just a few option ...
Whenever I receive a call or try to make a call the phone freezes and ultimately crashes. I can hear sound of the person after a minute or so freezing. This doesn't happen at every call, like 8/10 times it will FC. This has made it impossible to use the phone.
The following message appears everytime it crashes:
The process com.android.phone is not responding. Would you like to close it?

If I am in contacts com.android.contacts crashes.
I have tried clearing the phone application cache but it doesn't help.
I am running Slim ICS (4.2) with no mod or anything installed. Its completely clean. I have tried CM9, AOKP on my phone and my brother's galaxy S. All have the same problem on both the phones.
Here is some info about the phone:
Baseband version: I9000XXJVT
Kernel version : [email protected] #` Wed May 23
Build number: 4.2
Mod Version: Slim_ICS_4.0.4
I am using this phone in India if it matters.
What can I do to fix this problem?
EDIT: I update my baseband to XXJVU. Didn't help. What now?
Have you tried clearing cache in recovery and all that jazz? Be sure to do a backup while in recovery before you do!!
Yes, I cleared all kinds of cache before installing.
I did wipe data/ factory reset, wipe cache, wipe dalvik cache.

This is guess work but maybe a different kernel would help, try out this Semaphore kernel and just flash it in recovery.
What is this kernel? Semaphore ICS 1.2.6s
The Semaphore kernel project is in a lot more detail here and here.
The problem was due to faulty proximity sensor. I had to install MIUI ROM to disable it. Its the only ROM that has the option of disabling it.
I was able to disable the driver of proximity sensor (GPA2) using hardware disabler app but the phone was too slow hence MIUI.
Okay I have this problem in JB TW based ROMs I have tried (SweetROM, ULTIMATE, SuperBean) that whenever an app is gonna force close, the phone soft reboots without the popup "unfortunately, blah blah blah has stopped...". Is this a known issue or I'm the only one facing this? Thanks
try a later build of the ROM in which this particular problem may have been solved. These problems are quite common with the primitive versions of a ROM when it's in the development stages. Try sending PM to the ROM developer if you have such problems.
Like Varad said, make sure to also perform a clean install before you flash a ROM. This includes wiping system, preload, cache, dalvik cache, factory reset.
Always make a backup too.
After flashing cm-11-20140306-NIGHTLY-galaxysmtd.zip and Semaphore_KK_3.4.0s and rebooted my phone, when it finished updating the apps. The error "Unfortunately, com.android.phone has stopped!" has appeared and everytime I click "Ok" button, it keeps appearing. Anyone can help me fix this problem? I tried flashing it many times, and downloaded the rom trice but still I keep getting the same problem.

i have found the answer to my question, thanks btw. i think Semaphore 3.4.0 is not compatible with the latest upgrade. I tried flashing the rom without flashing a custom kernel, and it worked.
