Suddenly my note gave me an error message that my sd card is being used as mass storage & so data cannot be accessed. Well rebooted the phone to discover alot of files do not exist! Deleted?!!!?
Seems all deletes files are data files related to applications on the phone memory itself but not the applications or its settings!
Note: All files on the movable 16GB memory card are not deletes.
Examples for the files have been deleted:
Recently captured pics & vids.
Scanned documents by Camscanner application.
Scanned business cards by Camcard application.
SMemo files but not the folder structure I created.
SPB Wallet source file.
All WhatsApp downloaded pics was available in WhatsApp pictures folder.
All downloaded books to Kobo book & Mantano reader.
Please help as I am getting crazy with the amount of data I lost suddenly especially that I do not have any explanation to what has happened to my phone in order avoid it again in future!

Well now, that is a poor situation and atleast some of those files will not be possible to get back. Most of them possibly can, just as long as you do not write ANYTHING more on the SD card, not even SMS.
You could root the phone, take a .tar.bz2 image of the SD card partition on the external SD, copy the archive to your desktop and use Linux in a virtual machine to rescue the files from there -- the tool is called magicrescue.
Well this seems very complicates for me especially what's related to lunix & the virtual machine )) Anyway! Do you have any idea what might have caused this silly sudden problem so as for me to avoid? Is this common & subject to repeat? Is my device has any problem that I need to fix?

I know what caused the files to disappear; there was clearly filesystem corruption after improper shutdown and Android repaired it by removing affected files, but I do not know what caused your issue to begin with. Possibly something you've installed on the phone. In any case and just to be safe it, might be a good idea to backup everything, do a full factory reset and check you are running the latest firmware available.
Your explanation does make sense to me as I remembered that has happened just after I uninstalled the sslauncher application! But I also remember that the uninstallation has completed successfully with no problem!
Anyway, thank you for the valuable advice to backup & restore, I will make sure to do it today! Good day

Download App2SD from the market. You can move almost all the apps to the SD card.

I noticed this too when I installed the Amazon MP3 store although If you create a file on the External SD card and manually move your files there the music app will see it and play it from there. You can alse mount the phone on your computer and manually move them or use Astro File Manager to move them on the phone. It does suck though to have to jump through those hoops. Why you can't just change the default location is ludicrous.

I've been using since day one and it works fine. I haven't lost any apps and haven't had to re-install any of them.

You can use the program to move apps to the media area without issue. As for moving them to the microSD, it won't let you do that, even with root. So, I'm not sure what the fuss is about?

When your phone boots up, it fails to properly recognize apps saved to the media area.
To prevent it, just keep all your apps saved to phone until an OTA arrives addressing it.
Search the forums for disappearing apps to get filled in.
OK I thought the media area was the SD card. But I still haven't had a problem with the apps disappearing.

Neither have I, and I've been using it for almost a week. FWIW, I haven't seen any of the reports of this problem mention Gingerbread or the Bionic. Not saying it can't happen, but I think app updates have fixed the problem.
UPDATED: Been losing apps left and right today. This isn't an APP2SD issue, but a Bionic issue stemming from the way those tards at MOT decided to partition the internal storage. Why do companies do stupid crap like this?
While I was playing around with my lapdock I discovered something intersting.
I was installing some apk files from my back and discovered that my lapdock had several partitions mounted which started with "com."
It didn't take me long to realize that they were the programs moved to my media area.
They apeared as Ian drives and couldn't be innounted.
I devices to test out a suspicion and restarted only to find that those apparatus disappeared.
After some more testing I realized that by moving the files on the phone between the storage and phone until it no longer loads that app as a drive, then that app won't disappear when the phone is powered off or rebooted.
I know that not everyone has a lapdock to get around the disapearing apps, but this is a temporary solution until a fix is actually released.

i'm so confused about this, i had noticed when i loaded my sd card nothing was getting recognized like widget locker themes stuff like that. then i notice that everything is getting downloaded to the internal memory. i went into settings> storage and changed download location to sd card, but everything is still downloading to internal memory. i just don't understand why it isn't looking at my sd card automatically like all of my other phones did, droid x, t-bolt, og. it's driving me effing crazy though. can someone maybe explain in dummy terms what is going on?

Look at it like this...
Your phone has 16GB of internal memory.
4GB you never see. The system uses that 4GB
8GB is read as internal storage. Motorola designed that for media. That means your videos, pictures and documents.
4GB is for apps. This is where it gets complicated. These 4GB is dedicated to your apps. It was designed to be unintrusive with your main partition.
You shouldn't have to move to SD. Not needed.
Some wishets will only be recognized when the app is on your phone partition.

Just go into "settings, applications, manage applications" and click the "media area" tab. Any apps that have a "check mark" in them are stored to what is called the "media area." If it reads "com.*" then it has already been uninstalled. Just go reinstall it. Then go in there and all the apps with the check mark... click on the app, and then click the "move to phone" button. Wait a few seconds while it transfers it.
Sadly this is annoying and also sometimes makes the market think that the apps are not installed.
Freaking Motorola. They make such good hardware, it's a pity they put such crap inside.

Since i upgrated the os from GB to ICS, i have a problem with the music player and the gallery they showing empty duplicated files. For example, if i go the the music player i will see every song that i had before i upgraded the os twice, one is empty and does not play, the second one is the real file songs that i addes after the ICS upgrad, showing well, only once. I can't even delete the empty song from the music player.
Same on the gallery app
How can i fix this?
Maybe there is an index file for those files so i can delete it?

And I thought I was alone on this.
I tried unmounting SD and remounting, did not work.
Unmounting and rebooting neither
I can cancel these files one by one, after first finding out witch one if the wrong not working copy, but we are talking thousands of files, so not an option.
Any suggestions ?
ps. this is not happening to all files but I have not found a pattern yet.
Update: I now took the SD out and put it in the pc, did a little cleanup (but no double files to see on the pc), did a defragmentation (do not think it had any effect but it is a healthy thing to do), at the same time I rebboted Gnote without the SD.
After this, replacing the SD in the Gnote it appears my problem has gone....nice

I just tried to unmount&mount, unmount&reboot without sd and i saw still dublicates but now both were empty because the music is on the sd that i put out that time. I tried and it didn't work

I had them in triple... This is what I did.
First off, I had all my music and photos on my external SD card, I don't think this would work if they are on the internal SD storage.
Shutdown phone
Remove SD card
Restart phone
Music Player, All, Left click, Delete, Select all
Same for Gallery
Then shutdown, put the card again and restart
It took a few minutes for the indexes to be rebuilt while ICS was scanning the SD card and now all's good.
-- Be careful if you have Music and/or Photos on your internal storage because this may delete it forever!!! -

I had all my pictures and music on the ext. SD.
Took it out
Rebooted GN without any SD card
(did cleanup and defragment on PC during this but do not think that helped removing the dublicates)
reinserted the SD
Viola ! it is working fine without the dublicates
PS: reading bcn_nospam's post above I think he/she ? has the better solution (now i do not have the problem so I can not test it) but a corrupt cache as reason for the doubles makes sense.

This would be the ideal solution. Do not panic after doing this.
This process basically cleares the cache and the Android OS will start finding all the media files (Photo, Audio and Video) .. then you should have a single copy of the ..
I did not try the Mounting an unmounting method...
I deleted the data from the music player and gallery application through: application manager> all application and search media storage, music player, gallery application. I then unmounted the SD card and re-mounted it. All issues corrected.
Is this a bug with ICS on the note??? I am routed and I have the Speedmod kernal running on the standard ROM. I would be good to know if the cause is a standard bug or something I have done?

How do you remove the duplicates from the Cloud version of Google Music?

I have the same issue, and working on my second phone.
Initially I thought this problem to be caused by Google Music, downloading and adding music files to the phone. So, I did a restore and stopped Google from accessing the web and syncing with online. But then it happened anyway. I'm also having issues with the photo gallery giving blank thumbnails for photos and not being able to access them.
Samsung has no clue about these issies. I contacted Samsung today,919/12 and spoke with a rep. The only advise the rep had was to clear the application cache, which DID NOT WORK for me. Or do a restore which I'd already done and landed me back at the same place.
Switched from Sprint to AT&T to get this phone but could have stayed with Sprint and gotten the Galaxy S3 which I understand is not having these issues.
As yet, a 2nd level rep has contacted me back with no fix, but suggested it could be a bad batch of phones and said I should try another from another city. So I'm going to take a 30 mile drive to the next city and do the second exchange I've done in the last 10 days.

Clearing data and cache solved the issue for me till next restart. What solved it was clearing data cache and dalvik cache. Try that.

I think its a bug with ics. Because my Sony Xperia ion is having this problem and my brothers atrix hd Had this problem
Welcome to the wonderful world of android. Erasing the media cache will solve the problem temporarily, but every time you reboot the device it will scan and duplicate all of the media database entities and create new empty duplicates. This renders any android device (nearly) useless as a media player.
I have verified that this issue exists on multiple roms on the following devices:
Droid Incredible 1: GB
i-9100: GB
i-9100: ICS
N7000: GB
N7000: ICS
In fact, I have yet to use an android device that doesn't encounter this issue. It seems like a pretty major issue for a smartphone to be unusable as a media player but no one else seems to complain much. I've wasted hours on this issue.
I just tried to transfer files from my PC (Win7) via USB MTP onto my Nexus7 which seemed to work as usual. When I tried to locate them on the tablet, I noticed that the complete /sdcard folder is still there, but empty. Also the /storage/sdcard0 and /storage/emulated.
This actually happend the second time, but the first time I just had updated to 4.2.1 and I thought I must've missed some instructions about backing up the files first, so I (falsely) assumed it was my error.
The device is the 16gb wifi only version running current android 4.2, it's been rooted after purchase in summer using Nexus Root Toolkit 1.5.3. I just use the usual popular play store apps like nova launcher, total commander, firefox and stuff, nothing special.
I couldn't find anyone on the internet with a similar issue, so I put this here.

I fail to see how that should affect why I don't "see" my files anymore. I haven't switched the user, I only have one: the owner. May you please make your point more clear?
You can alter where things are stored. Have you connected back to pc to search tab that way. Check storeage settings did you use ptp or mtp for transfer

I used MTP. The tablet looks empty from "within" via Totalcommander & EStrong Filemanager. Also other apps like music don't see my files. Storage shows suspiciously much free space, I guess it's really gone. From PC it looks the same, completely wiped. It recreated the /sdcard/Android-Folder, and the Apps I've used since recreated their folders... I can't find where you meant I could alter where things are stored. Storage just let's me delete stuff and users just let's me edit the away-message.

Ok so you're not using an app designed specifically for moving other apps/etc to your SD then. With that being the case you'll need to be very careful in where you're pulling the data from if you expect it to continue functioning as it used to.
You mentioned initially that your moved pictures and videos still show up in the app drawer but are greyed out and say not installed....this is a bit confusing as pictures and videos are usually just single file images or single file video clips and would not ever actually show up in the app drawer and are instead accessed via an app from the app drawer like Gallery or Google Photos, etc. Generally speaking, the apps themselves are usually much smaller than the total amount of space taken up by your videos or (if you have a lot) pictures....you don't even need that many pictures to start adding up to a respectable size if they are all High Quality images.
So, I would recommend you attempt to undo the moves you've made thus far (though if you can't get the apps to show back up as normal then just reinstall them then clean your SD card and internal memory with a junk cleaner) and focus solely on ".img, .png, .mov, .mp3, .gif, .jpeg, .mp4" or any other similarly formatted files if you're doing the moves freely with a root explorer like ES.
If you really want to move your apps over, or at least some of them, try using an app specifically designed for that purpose as this will hopefully ensure they continue to function after the move.....If you're on the stock ROM you should be able to move the apps just by going to Settings > Storage I think or Settings > Apps. One of those two (pretty sure it's storage but I'm on AOSP rom atm so can't check sry) and then click on one of your apps you should get options to Force Quit, Clear Cache, Clear Data then below those you should see a Move to SD option.

No you were very clear I wasn't clear when I said the pictures and videos in the app drawer I should have said from the gallery they where showing up as an empty space Now I believe my card was just going corrupt because it's corrupted now

i have this issue as well. When i transfer files like pictures and music from my internal to my external it says its transferred which is cool but when i reboot the device the files are gone. I have tried the sd card fix apps in the play store and it says its fixed but obviously not. It wont let me format either on the computer or on the phone or it says it does but it actually doesnt. Install DiskUsage from the play store. It will show you what is using most of the space on your phone and you can you can then use the "My Files" app to remove what you want. Also get an external memory card you can put on a keychain which you can plug into both your phone and a USB port on a PC/Mac to copy files around that you want to keep. Also having "just enough" free space is not enough. Your phone will be a lot faster if there is plenty of free space available on your phone. Uninstall apps you don't use.

Its anew phone and I also debloated it iv hardly used any storage on it at all I'm not understanding why it's already low on memory with all the free room it has??
Maybe your debloating process disabled something dealing with memory management.
I would try undoing your debloat and see if it still does it...

Just re-bloat and let is know if the problem goes away first. If it does, then re-debloat only one thing at a time until the problem recurs. Then you will know what had to stay running (not be debloated) to fix the isue. It's not rocket science but it is tedious and you just need to stick with the process and be patient.

Why would we look through your list? Undo it, reboot, and see if it still happens. If it doesn't then redo it slower and figure out which one breaks it.
If it does still happen after undoing the debloat and rebooting, then your phone is screwed as it doesn't happen to anyone else. Factory reset.
