I'm just speculating here, since (A) I haven't tried flashing it yet, and (B) I don't have any official-source knowledge of this answer, but I would expect "the latest" RSD Lite; I think I have 5.6.4 on my Android dev machine at home, but I'm not home at the moment so I could be wrong. I set it up only a couple of weeks ago, so unless the download link I used was already stale, that should be current.
To be more certain than just me talking out of my ass , I recommend googling around as well as waiting for someone knowledgeable to answer here.
ETA: According to this tutorial video on fastboot flashing the Razr, 5.6.4 is the version they were using. The general guidance I've seen is that the Razr is architecturally very similar to the D4, so that is probably the best answer unless a newer rev has leaked in the intervening month or so.
ETA2: A little forum searching and I find this post with a download of RSD Lite 5.7. I stand corrected, I think. I'll be downloading and trying that tonight.

I can confirm that this recovery xml.zip, using the tools and procedures kholk posted, completely recovered my phone from boot-loop bricked state.
After the "decompress and start flashing" part of the instructions, it took about 15 minutes to flash and about another 10 minutes rebooting into normal recovery (the little android and open box icon), then one more reboot and I was re-connecting to my gmail account.
After that, the kind of magic Android Google settings recovery thing happened and the phone even remembered and reconnected to my home wi-fi.
As it stands, my phone is now pristine, fully stock, and with my settings and /sdcard stuff fully back. 4G and phone number too. (That probably wasn't in doubt, but I'll say it for completeness' sake.)
I judge that this fastboot recovery file to be perfect, and kholk's instructions were flawless.

I followed the steps as shown in the video... and I'm still bootlooping... this isn't good.

Someone posted over on droidforums.net that this failed and now she's getting a "AP Fastboot Flash Mode (S) (Flash Failure)" message:
SO I definitely just tried this and it definitely stopped on 17/18 and it said Flash Failure. And now I can't even get my phone to turn on.
It says:

I did what it said to do. Now what do I do?! I tried to go back to the screen where I clicked on AP Fastboot Flash Mode, and I tried clicking on Normal reboot, recovery, and just about every other option available, and nothing worked.
Someone please help me ! I just got this phone a month ago, and I'm going out of town tomorrow and need to have a working phone!

If you are bootlooping after a fxz flash go into stock recovery. Power off then power back on while holding both vol UP and down. Then at the android and triangle press both vol keys to enter stock recovery. Wipe data and cache then reboot and you should boot up to setup screen.

[Solution] HC 3.1 OTA Difficulties 13% + Stock ODIN

Here's a solution that worked for me
Flash the below with ODIN (thanks to bonzai3558), extract the .md5 file from the zip, and in odin choose PDA and point to that file, keep everything else blank/default. Connect Tab with USB (after it is in download mode by power on +Volume down then volume up, so it shows the yellow android with shovel) then hit Start.

Now when you check for OTA once your back up, itll work fine and get past 13% and go all the way (it is normal for it to stay a long time on 76%)
Best of all tablet is ROOTED
so this odin file is surely stock (and plays nice with the OTA update)
-PS: This odin file also will save your device if it is bricked or frozen (boot into download mode) power button + volume down button, then press volume up to select

this version of odin isn't seeing my tab. I have the samsung drivers installed. :/ But it still says unknown device when I go into fastboot mode and odin doesn't see it
edit: not sure what it is, my gigabyte mother board seems to hate samsung android devices. Worked like a champ with my wife's laptop. Thanks!

SS2006, you are awesome! Thanks for posting this. I flashed recovery.img, flashed boot.img, and restored all the original apps from the system dump, but I was still stuck at 13%.
Was totally WTF...?

doesn't work on retail with that ver. of odin (and trying to flash in a tar'fyed cwm).
either it too is locked, or there is another means of interacting with it.
ill kep my eyes peeled for retail.
So I did this to a retail locked 16gb. I get force close messages like crazy now. Any idea on how to get back to stock?
I tried this after an adb reboot,,, sent me into a boot loop and it did not clear it up so i did it the old fashoin way, hopefully can get it to work later would def help!

This saved my LE I/O SGT! Was stuck in boot loop; couldn't connect with adb or fastboot, but could get into ODIN download mode. After flashing a couple other versions that either didn't work or had other issues (no USB, accelerometer, etc), I finally found this thread. Flashed this PDA and now I'm back up and running!
However, I had already messed up my bootloader. I flashed it with this version: http://forum.xda-developers.com/showthread.php?t=1083517 which is from the 10.1v retail edition. Now when I try to do the 3.1 OTA update, it won't boot into update mode. It fails and says to try doing the update with Kies. Do you have a bootloader that I can flash with ODIN for this same PDA version?
It did same for me, when i tried to do OTA update, it says failed to update and try to update with kies. I ended up factory resetting the device. Its still rooted and now I am trying to update again. Hopefully it works this time. For some reasons I can't launch any apps. They are all crashing. Any ideas what might be wrong?

Any luck? I haven't had the app crashing problem, but it seems pretty common from reading around the forums.
I was able to install Kies and get it to recognize my tab (had to disable USB debugging first), however, it gives me the following message: "This version of the device cannot be updated." So, it seems I am still unable to get the HC 3.1 update... Anyone have a solution? I still think the original ODIN flashable bootloader from the same PDA as the OP might do the trick for OTA updates... Just not sure where to find it.
FWIW: Kies reports my current firmware version as: PDA:KE3 / CSC:KE3 (XAR)
It doesn't work for me either. I get the same error and when I tried to do OTA update, it reboots after the download saying it couldn't be updated. I think we need PDA with 3.1.
I haven't been able to use anything on my tablet. every app running in the background crashes and theres. I did system reset and factory reset but no luck.

SS2006, you are a god send! I was in a boot-loop somehow on my Android 3.1 10.1v after trying out the http://forum.xda-developers.com/showthread.php?t=1130574 thread..
Flashed with this ODIN package (that is Android 3.01) and did the successful upgrade back to 3.1! Sweet!
So does anyone have more details on these ODIN packages and how they are setup?
odin is crashing es soon as i open it, anybody having the same problem?
update: nevermind, im using odin 1.82 and now im rooting my tab.

can i just flash the root exploit? using stock recovery?
just wanna make sure before i did something stupidly irreversible.

for those of you on locked 3.1 P7500 (indonesian edition) you can just skip the Odin flashing and directly place the root exploit to your root sd and then apply the update (just tried it and it works) i have root now.. moving on to CWM

I dont understand I got PDA_SIGNED_HRI83_BUKDF.tar.md5 to work once but now its not and PDA_SIGNED_P7105.tar.md5 never works by the way which one is the retail one anyone know??

So, is there any way now that I can brick my phone into an unsalvageable mess short of physical damage? That's the situation I had with my OG droid with sbf files. I'm still not completely sure this the case with a locked bootloader. If it is, I'm looking so forward to tinkering with my phone again, as I have shown restraint since there was no way to unbrick my phone.
That should solve for all the bricking issues with the Droid 4
The restoring to stock just won't work for me at all... I'm about ready to give up.

That's really strange. I've never seen 6.93.215 anywhere...and Googling it only brings up your post. I would imagine you shouldn't have a problem flashing 6.13.219 but I'd try and determine how you got that system version first, just to be safe.

Ya I tried googling it too and got nothing. I got the OTA update this morning and now it says 6.13.219 so I guess I'm good. It was just weird cuz I didn't see it anywhere.

I think the only way to really screw it up (someone correct me if I'm wrong), is the same way you could really screw the OG Droid up, by interrupting the flash procedure during RSDLite.
With the locked bootloader, we should be even safer (I think) than with the OG Droid. Although I never tried this on the OG Droid, on the Kindle Fire you can replace the bootloader. If you hose that, pretty sure it is game over... Don't think this is possible on the D4, since it needs to be signed...

Just read something about some Razr owners bricking their phone trying to go back to stock using RSDLite after flashing a leaked ICS build. Not sure what the leak could have done to mess that up but just FYI.

Ya a few people were having problems with the razr leak. I think its more because of the leak than anything else.

It's not like the D3 you could restore back to stock without rsdlite. When I flashed mine I didn't get the msg saying "process completed".
If your phone keeps bootlooping after flashing it to stock with the 6.13.219 via rsdlite v 5.7
You can try tne following. Works like a charm for me
1-unplug the phone from the pc
2-Hold down the power button + volume down to reboot onto recovery,
Select Recovery from the list.
3-when the screen with the android and the exclamation mark comes up press both volume buttons to get onto the recovery options.
4-Wipe data and cache and then select "reboot system".
Then you should be good to go.

Hi,
I am in dire need since my phone is always stuck at this screen and can't use the phone for couple days now...I even tried to use the "OneClickFastbootRestore" script that I found but after rebooting the phone and came back stuck in the same position...
Any help would be greatly appreciated!

Have you attempted to flash the device back to .902 using RSD Lite? You will need to enter the bootloader(hold the PWR + Vol up + Vol Down), then select AP Fastboot -> Open RSD Lite(make sure you have the drivers installed) -> Plug in your phone -> verify RSD Lite sees your device -> Now select the file -> Then select Decompress and Flash. you need android sdk installed download the rom you have installed and extract it to the android directory (c:\users/appdata/local/android/platformtools. once you have the grfs.img file inside the android folder, hold shift and right click inside the android folder and choose open command prompt here, at the command prompt type fastboot flash webtop grfs.img this will reinstall webtop
and enable you to access your phone again.

i have purchased cdmaflash software but need to put the phone in airplane mode with no simcard and i can not even get past activation when i reboot normally as it just comes up to flash failure. thank you if you can help.. been over a week with no phone..

im also trying to flash my bionic to page plus using the same tutorial listed above and keep getting the same error as above.. if tried everything in this thread bus still can not get the phone to boot. if someone could just point me in the right direction that would be great. ive searched goolge high and low. i am currently using my D3 but the digitizer is cracked and that makes it hard to use my phone. any help would be greatly appropriated! :fingers-crossed: Thanks

My phone is stuck in only showing the Fastboot page. Basically, I am trying to turn my device into Verizon. I had it compelety updated (so probably JB) but was running CM 11. I wiped the cache and /system and it would go past the splash to a grey screen. I tried RSDing it and now I am stuck in the Fastboot.

unreadable display after Flash to Safestrap 3.73 - what to do?

Dear all,
after rooting and flashing Safestrap 3.73 on my Motorola Droid 4 I have big display problems... All the rows are displayed very weird, like pixels are replaced and put in a new order.
You can see shades and colored areas, but it's impossible to recognize any font or proper geometric details.
The touchscreen navigation works normal, after having a working Droid 4 with 3.72 next to my broken one to check where to push on the screen.
But even with that, I dont have any sucess in flashing CM11.. I can't read the console so I don't know where the problem is. goign back to stock rom is also not working, it's in a endless loop.
Does anyone have any Idea how to get this to work? I would try Safestrap 3.72 (my friend uses this).. But how can I flash it?
Or how can I remote flash any firmware on my phone??? Its not recognized by my pc in Safestrap boot screen!
If I could redirect the display output (e.g. via hdmi) it could be possible

If you can not get to stock, you will need to flash stock firmware/XML with rsd lite

Please Help..Sprint Note 5..Boot issues?

Ok here is my issue. I have been flashing droids for years without a single issue. I got my Note 5 about 2 months ago and decided to flash it since it was possible for the Sprint model. Flashing went as normal, perfect. I flashed a custom ROM, MOAR, and it was working great. No issues, very amazing ROM by the way. I then went and made a back up with TWRP and saved it to my OTG flash drive. I then proceeded to change the boot image, the one that says "Samsung" logo, because I had seen a tutorial on how someone did that on a Note 5. Now comes my problem. After flashing the new boot image, my Note 5 WILL NOT boot up. It will charge, as it does show it charging, I can get it into Download Mode. and that's it, just Download Mode. No recovery mode, no boot image, just Download Mode. I want to know if it's possible to use the backup that got from TWRP, which was a full backup, and use it with Odin to flash it back to the way phone was before this mishap, reinstall the correct boot image, or WHATEVER I need to get my phone up and running again. Thank you very much to anyone and everyone that can help me out
I would use download mode and a full firmware package to get it back working and booting , and once that works , I would use my backup to return to where i was.

Do you know where I could get one? I need this phone up and running really bad. I just went to Sprint and they said they were going to charge me $75 to look at and if it needed to be fixed it would be $350. I can't afford that. So ANY help would be greatly appreciated. Thank you.
Sorry about not answering earlier. You should get your firmware from www.sammobile.com , just search your model # (Note 5 are : SM-N920XX ) where the XX is carrier dependant. Sign up to their site and take the slow download. Then flash the firmware in download mode , using odin on your pc.

Just wanted to say thank you for the reply. I've been downloading the firmware for the past 2hrs now.lol. It was the "newest" upload they had for the firmware. If for whatever reason this doesn't work, GOD please it work, what should next? Thank you.
Well I flashed the new firmware(N920PVPU2BPG1_N920PSPT2BPG1_SPR) last night with Odin and to no avail it did not work. I still get only a black screen when trying to normal boot, still can't boot into recovery, but still can boot into Download Mode. I don't know what to do at this point. I have never had this happened.
