So there I was, stupidly updating my Gapps... forgetting to make a backup. What happened? I kept getting crashes, so I try to reinstall SmoothRom. It fixes it, but in the process of it all, my Recovery Menu was changed to what I didn't want. I couldn't get it to go back to TWRP. After about 30 minutes of finkering with it, I try to load back into android... I can't, so I go to go back into my recovery, doesn't work. I could go into fastboot but kept getting [Failure (Nv3pBadRecieveLength)] Error. I came upon a thread after a little search and I see something about the usb cable... So it got me thinking about USB 3.... Low and behold though, switch my USB cable back into a USB 2 and everything works.
TLDR; DO NOT HOOK YOUR NEXUS 7 INTO A USB 3 DRIVE!
I am using windows 8 and i connect my nexus to usb 3 all the times no problems.
If ur using windows 7 (or older) u must update ur usb 3 drivers unless u want some thing not properly detected or u dont want to use that device
Let us know little more about your current situation e.g can u boot into some thing android (any rom)
what is your exact problem (cant flash gapps or cant flash rom or cant flash recover or dont know how to use fastboot)
And remember always read before posting a questions and if u have to post then please post in Q&A section.

dudddeeeee calm down. let me start off by saying your phone is fine so relax. do you have an sdcard reader or something? basically something that will allow you to put a file on the sdcard and then back into the phone? cause all you gotta do is use this "update.zip" thingy. basically you go into recovery, press some buttons and then bam clockworkmod recovery. and then from there u just do a wipe and put on a rom that works (to provide you with more comfort, as im typing this my phone is actually bricked as well lol cause ive been waiting like 10 min for it to start since i put this new rom on. so ill just quickly fix that) let me know if u can get something to read ur sdcard and ill provide you with more info
i'm following this guide..i have problem with connecbot..when i type the commands it says no such file...but in my sd there are the files!!!...
Follow this guide since you're obviously usb bricked:

I resolved with this guide..thanks at all guys!!!
Help please - I tried to upgrade to CM6.1, have done it s few times i the past with great success and no idea what went wrong. believe i accidentally wipe Davik Cache. The Rom loaded, but now I'm unable to mount SD Card. I'm also unable to use ADB on WIN7 Ultimate suddenly. Have read numerous threads and google topics and have tried several solution.
1. modify google usb driver ( R8 SDk pakage)to include legend
2. various versions of HTC Sync to get the drivers.
3. using Deview to remove all HTC devices, etc
Is there anyway to mount the SD Card without ADB, then to recovery and flashing again?
PLEASE HELP
Hi all , i hope someone can help : i was installing team hacksungs ics rom last night it seemed to be going ok at first but then got stuck at the animated splash screen (1 hr 45 mins) i pulled the battery and when i tryed to get into cwm recovery the phone keeps just bootlooping after showing google logo , i can get into download mode but neither odin or heimdall can find my phone and windows reports unrecognised usb device, heimdall reports- ERROR: Failed to access device. libusb error: -12. I have unbranded i9000. All drivers present and usb cable(s) checked and are ok....thanks in advance for any help
I have the exact problem and situation
Is the phone bricked for good?
You might need the drivers listed in the TH ICS thread. Before changing the drivers try plugging your phone into a different USB port
You might want to try a different computer, the libusb errors are common and are usually solved by using a different computer (I have seen those errors while flashing IPhones, they were resolved after reformatting or using another computer[I'm not saying that that is the only way of dealing with libusb issues, I just didnt bother with em after I found out that a fresh start/system fixes em]). Also, if you are into linux, try a linux distro on your computer and see if Heimdall works from there.
Good Luck
guys i have had the same problem on this phone. To be honest I am not sure I wanna stay on ics anymore alone because of this issue. Nothing wrong the phone is great but the driver problem is flat out pathetic when **** hits the fan.
Here is how I solved my problem. I installed and uninstalled and restarted my PC so many many times. Stuck my USB cable into multiple ports. If that didn't solve my problem i Installed and uninstalled the Samsung drivers (without kies).
Played around with device manager when it said the phone is not recognized by uninstalling and removing phone from USB cable and replugging . I installed the android sdk USB drivers and tried upgrading the not properly installed drivers with them (which I think hasnt helped much to be honest) .
All in all what I am trying to say is......that I was so ambitious trying to get it to run that at the end i got it to work after many many tries. With many tries I mean I was trying this repetitive process from 3 am to 7:30 am and it DID NOT WORK. I went to bed. Next day I tried fixing it using repeating the whole process from 5 pm up to around 830 pm.
I dunno how I got it to run at the end. All I know is the time I invested trying to get it to run were nearly 10hours of repetitive installing and uninstalling. Sadly that is the only tip I can give you. It seems to me as though it just randomly feels like working out of its ass.
I even brushed the USB port of my phone with alcohol and a brush. As mentioned above trying another PC is worth a try. I tried my sister's laptop without success though and even formatted my desktop for the sake of it.
The last thing I recall doing before getting this phone to get recognized on my PC was as follows: I charged it for an hour and then booted the phone in download mode and decided to try it again. And bam it worked.
But do give this method a try.
1. Plug phone to PC
2. Uninstall your device(exclamation mark) via device manager and then unplug your phone
3. Uninstall sgs drivers
4. Reboot PC
5. Install sgs drivers
6. Plug phone in download mode in again
7. If you get error, device not installed properly then open device manager and uninstall the exclamation mark again
8. Unplug phone once uninstalled.
9. plug it in again
Also guys, just out of curiosity what is your OS and is it 32 or 64 bit?
Good luck
P.s: I know this post isn't helping specifically but I believe the only solution is playing around with the drivers and device manager until it decides to work. I might install another rom (Ive only experienced this with ics so far. And this seems to be a common issue apparently. Does this happen to people on 2.3.x as well ?) tomorrow cause this issue is too time consuming should it occur again. Only had it with nushor kernel though.

Hi vytman , install the drivers from here : w w w.multiupload.com/BUCFT8M42N
the drivers allow your comp to recognise your device(which is now nexus after ics install) then you can reflash stock samsung firmware in odin .
u can't get into recovery at all?

I was unable to download the file...i am having the exact same problems and your solution seems logical. i tried going online and just searching for a nexus driver but that didnt seem to work. I think it might have been an old version of the driver. Is there a way for me to get this driver?

Yes it is possibe. From the root prompt on a terminal you could "dd"the correct images to the correct partitions.
However I would recommend you try to find an other PC (windows 7? ) where the usb does work, and where the correct APX driver can be installed if the phone is in the invisible recovery mode. (you will have to have a plan B if the dd fails anyway).

My pc USB ports works, my phone port doesn't, thats why i need a secure guide to going back to the oldbootloader without connecting to the PC
micro-sdcard
Try with a micro sdcard. You can see the slot if you remove the back cover.
It has saved me a couple of times
Wut?

Wait, wat? i don't get it, i don't know what are you talking about, butl i went back to the old bootloader, had to buy a new part for my phone, i think this is solved. thanks for your reply!
I am relativly new here, i now how ti put custom roms, recovery mod etc etc, and that all is fine. Two years ago i bought my ace, and root it, put recovery mod int it i beleive version 5, and all ok, i flashed about 50 custom roms, changed them until found one i liked
Now i got another phone, and my dad has the Ace, but he wants stock rom, dont ask me why
So i said sure why not, get the cable, charged it up, make backup of phonebook, connected it, installed all the drivers on my Windows XP, btw i use Pentium 3 toshiba laptop, cause i cant get drivers to work on Win 8
downloaded a stock rom, 2.3.6 version it is and it is XWKTM
ODIN 4.38 i put the Cooper_v1.0.ops file in, the md5 file, select all 3 options, and connect my phone, put it in download mode, yellow download shows, connect to usb, its original cable, windows detects, i get yellow in odin, and push start, checks md5 all ok, but gets to the setup connection and it can hang there for million years, damn..... ((((((
i changed cables, changed USB ports, reinstalled installed drivers, NOTHING helps.
Any ideas?
Oh by the way, i maybe was a smart wisea*s so, i used this
http://forum.xda-developers.com/showthread.php?t=1437426
to put original recovery mode in ti, hope i was not VERY PRETTY stupid because of this
Now i cant get into the recovery it puts me in some kind of memory ram dump arm9 crap
And btw the customrom inside the phone, works, phone works all functions work, so it is not bricked, atl least not fully.
Im thinking maybe because toshiba laptop has old 1.5 i think USB, not 2.0 so maybe that is the problem, tommorrow i will try on Windows 7 machine a my workplace, if that fails i have no ideas, so any ideas? help?
thanks for reading oh and btw, sorry for bad english, i type with one eye closed and sleepin, while other is on half soooo me make a mistaka

Disconnected the phone from the computer then reboot the computer. Does the phone now show up in Device Manager? If not change USB cables and try again. If the computer recognizes the phone, then you can move files to the phone.
Thanks

To solve the no OS part, just flash a guru reset or a nandroid backup for your device.
Hope this helps

I've never really experienced connecting my device to the PC won't work. Sorry.
Just buy an OTG cable and put the ROM in a USB and plug it to the OTG cable so you will be able to install the ROM. If you successfully installed the ROM try connecting your device to the PC again.

For the os issue this Frequently Asked Questions - Page 3 Ans E post will help.
If you are still connected to the computer when you are in Recovery, Device Manager will disconnect when the phone boots in bootloader or recovery.
