Most of the people who flash CM12 Roms face bootloops.Yesterday i found the issue.Not sure if this is so but it might help.Yesterday while flashing the Cm12 roms it showed me a successfull flash but i too ended in bootloop.Later i found that while flashing the /System gets suddenly unmounted and you face error "Device Busy and cannot be mounted" but your flash will be successful resulting not all System files are copied.So follow the below steps for successful flash.
Wipe Cache,Dalvin cache,System,Format data(if required)
Reboot
try to flash the Rom
Again Wipe cache,dalvin Cache( This step is Must )
Try to flash the Rom again[The status "Device Busy and cannot be mounted" should not be displayed else repeat the process]
Reboot
Note: This is one of the cause i faced for Boot Loops

[Q] HELP! Boot loop and Mythtools freezing when flashing stock firmware

Okay, I have an Atrix HD that I have rooted and unlocked the bootloader and I have been running the SlimKat ROM with little to no problems and Philz Touch recovery for about a week until yesterday when my phone reset itself and started to boot loop. They only other thing I had flashed was a new logo to get rid of that stupid unlocked bootloader warning. I am fairly savvy when it comes to rooting and flashing roms because I have done this for years with various Samsung devices but the Motorola is a little new to me with having Fastboot.
Let me tell you what I have tried...
I first did a factory data reset in Philz and that did nothing.... then I tried to recover it back to my stock rom that I was running before using a nandroid backup that I had made with Philz Touch and same thing happened, it just sticks in boot loop. I then try to flash the PAC rom after I do a factory data reset and all the wipes I need to flash a new rom in Philz Touch to see if I can get a different rom to load and nothing happens but boot loop again.
Next I go to the forums to find answers and I start to set up my ADB on my computer... After I get ADB setup I first do fastboot -w to do a factory data reset through fastboot, download the stock Mexican rom and tried to flash all the components individually with ADB in fastboot the way skeevydude mentioned in one of his threads (sorry I can't remember which one) and I got a failed error "FAILED (remote failure)" when I tried to flash the system.img.
I then went to Mythtools v1.3 because I thought that I was doing something wrong with the ADB and fastboot... I put the stock Mexico retail rom into the firmware folder in Mythtools and tried to flash the stock firmware through Mythtools. Mythtoold froze when it rebooted the fastboot at 50 of 100%
Now I am at a loss, I have done everything that I know to try to fix this and I have searched everywhere for help and I need help. This is they only phone that I have because I just switched over to to a new carrier and just purchased this phone.
Any help would be appreciated!
UPDATE
I went into Philz Touch and formatted the /system so I could try and flash it again and now the phone boots straight to fastboot and Philz touch if I hold volume+ and power. I am going to download the stock mex retail rom and try and flash it through Philz touch.

I got an Error mounting /data when I tried to format the /data and when I tried to intall the Mexico retail rom I got a "set_perm: some changes failed E:Error in /storage/sdcard1/Atrix HD Mexico Retail 4.1.2.zip (Status 7) Installation aborted."
UPDATE
I formatted /data and /data media in Philz Touch and then it started mounting /data so I wiped and install PAC rom and it booted up just fine.

[Q] Phone totally bricked, please help!

I have a Moto G XT1068. I've flashed the latest ROM from the [ROM][5.0.2][titan][XT1068/69]The Titan Prime ROM[MAR 2](Version 1.2.1 RC) thread, then attempted the OTA null-null update, then upon rebooting it got stuck at the loading screen (colorful balls floating around) and whilst stuck it ran out of battery and switched off. Now I am only able to go to bootloader and recover (TWRP 2.8.5.0) (all other options take me to that same loading screen) and everything I try to do in TWRP fails. I cant do anything from adb and fastboot or mfastboot, it all fails. Anything I try to copy from pc to phone internal sd card while in recovery (in bootloader it doesn't see the phone as a storage device) either fails to copy or does and then disappears forn TWRP after reboot. I don't know what else to do, please help, I'm desperate!

Few details are missing
1. Which rom where you using before
2. Did you wipe fully (Data, System, Cache, Dalvik (if))
3. Did you do factory reset before switching roms
4. Incase you did all and when you did the Update did it require any wiping and did you do it
5. Did you try flashing another kernel
6. Did you try with another SD Card
Try these it may help i am a very tech savy but these things usually helps
Download the ROM to a PC, transfer it to your EXTERNAL SD, and clean flash with TWRP.

Help with flashing CM13 to Moto G 2014.

I had recently tried to root my Moto G 2014 (running 6.0) so I could install the latest CM13. I followed all the instructions and even came across the red WARNING BOOTLOADER UNLOCKED SCREEN every time I try to reboot the device(status code=3). After installing TWRP 3.0.2.3 and making a backup of current state, I ran into a few bootloops which I managed to fix by flashing it back with a stock as shown on this thread http://forum.xda-developers.com/moto-g-2014/general/official-stock-firmware-t3018682 (I downloaded both the download links and extracted everything to a single folder before following all the commands)
The SIM wasn't receiving any signal thereafter, which I luckily managed to fix with help from another thread.
My problem is that I've tried to install SuperSU more than a couple of times with different versions and it almost always sends my phone into a bootloop. The few times where I was able to boot into system, it kept giving me the error that the necessary binaries weren't installed and I was not able to do as well, due to the bootloop issue. Working RootChecker told me that my device wasn't rooted properly, which I wasn't surprised at.
Although I read somewhere that rooting isn't necessary to flash a custom ROM like CM13, so I decided to negate SuperSU (I did the unroot everything option on SuperSU and factory reset the device, cleared data, cache and dalvik too) and try flashing it directly instead through TWRP. It kept giving me the Error 7 even after I had removed the assert checks.
Can someone tell me what I did wrong and what can I do to go ahead? Also, is my bootloader completely unlocked? I tried checking *#*#7378423#*#* but nothing happened, so I rebooted into bootloader and used the 'fastboot oem device-info' command but it said that the command was invalid or something.
As of right now, the phone is running 6.0 with all the stock apps. The bootloader unlocked warning still shows up every time I reboot the phone though.
Boot into twrp and wipe cache , dalvik cache, system and data and flash cm13 and wipe cache and dalvik cache and reboot you should be good to go if u want root access flash the latest version of supersu or you can for a rom which has preinstalled supersu such as dirty unicorns.

while @kushal_10 explained most of the things to do, i would like to remind you that CM13 has inbuilt Root. You just have to enable Developer options, then in Developer Options,under Root Access, choose Apps and ADB. Don't flash SuperSu

I read somewhere that wiping the system isn't advisory because in case something goes wrong, there will be no way to restore. Can you confirm this?

help Moto G 2 1078, can't install any rom

Good night guys, first of all I'm sorry if the text is confusing, English is not my strong point, i'm Brazilian.
I had the cyanogenmod in my phone(Moto g thea 1078) and as it was giving so many problems I decided to change the rom to the crDroid, but by a huge oversight of my part when I wipe I marked all the options, now I am not able to install any rom, not even the stock rom, some I can install by twrp without any error (crDroid, Viper), but when I try restart the system after installing twrp says that I am without any rom's installed, and if I ignore and restart anyway my cell phone is eternally on the logo screen, there is only one rom that I was able to install and start the phone, the 1068 rom stock, but chips and wifi doesn't work, when I'm going to install a 1078 stock of error in primary_gpt, in boot and the sparsechunks

does anyone have any ideas to help me?
Copy a proper ROM for Thea and Gapps on a micro SD card using a computer or a friend's phone, then place the SD card on your phone and install it through TWRP.
Of course you needed to wipe everything - you always need to delete everything before flashing new ROM, just NOT the external storage, because that's where your installation package is. Try any of the following (some of them don't have SuperSU included, so you will have to flash it separately):
- Viper or
- [proper] crDroid or
- DarkNess reDefined or
- Paranoid Android etc.
Don't forget about the Gapps package and SuperSu (the zip file).

i'v tried this once, doesn't work, but now i can't even install twrp or similar recoverys, always gives an erro "mismatched partition size (recovery)", i google it and people says that this error is normal, the twrp should start even with this errro, but when i start the recovery i got 'boot up failed' or starts the original recovery

Your phone is crippled for life, you installed a firmware for the wrong device. Go install a Titan ROM and TWRP, there is nothing to do. Your phone is an XT1068 now. You lost your LTE ability forever.

Stuck in TWRP while formatting data

Rookie here.
So I was on my way to flash a custom rom for the first time after downgrading from MIUI 1.2.0.7 to 1.2.0.3, and managed to install custom recovery.
Before anything, I used the "Wipe data partitions" to format it before flashing the rom. However, after doing it it got stuck for over 20 minutes still loading, with an error message saying
E:Unable to load '/twres/languages/zh_CN.xml'
with 'MTP Enabled' just below.
I really don't know what to do now, as I'm scared to force reboot it and potentially brick my phone or damage it.
What do I do just now? My phone isn't fully charged, and I don't know what could happen if it runs out of battery while still formatting data right now.
I'm in your exact same situation, with "Wiping internal storage" still running after more than 10 minutes. Did yours eventually finish? Or did you have to force reboot it?
You can flash the STOCK recovery, then format data.
After then, you can flash the TWRP again and then try to flash the custom rom.
Remember to try to flash the custom rom AFTER to format data and BEFORE to boot.
