Sorry but IMEI changing is illegal in most countries nobody here can help you enter an IMEI into your phone.
If however you flashed a bad radio you could flash your stock radio and that might bring back your IMEI if it is missing.

You can try to flash it via RSD lite.
Wipe it at first, and flash it.
It will let u input the code

[ROM][STOCK][LP 5.1]XT1095 flashable stock rom

I have created a stock rom of 5.1 lollipop that you can flash via custom recovery. The only modifications that I have made are to remove the motrola OTA service, that way there will be no bricks from people taking an OTA and it won't bother anyone about OTA update messages. The recovery flasher was removed to prevent it from overwriting the custom recovery.
I have uploaded a new version which has the stagefright patch(23.16.6). The old version is also still available as well.
Installation:
1) If you are coming from another rom, or a different version of android you must do a factory reset.
2) Install the zip file through TWRP/CWM/etc.
3) Enjoy.
Download 5.1 23.16.6

Just wanted to say awesome! Thanks and am running right now with absolutely no issues!!! Noticed it's funny little differences in the ROM, from the Brazilian version ex. Status bar, the H was huge in the Brazilian version, very small for the T-Mobile unlocked xt1095!
So I'm running rooted stock 5.0, I can just flash this and run with it?
Yes. Just remember that my rom only flashes the kernel and the systrm. Nothing else gets upgraded. So your modem, partions, and bootloader will still be the 5.0 ones.

Will work for xt1092, but I do believe there is a stock ROM 5.1 for the xt1092, might be a better fit?

It should work. It only flashes the kernel and system, so your radio stays the same which means you should have signal.

I see I just knew it was for the xt1092 Cuz I was using it before the stock 5.1 ROM xt1095 was created, and it worked great! What's the difference between retde, and reteu? The country being used in?

The languages you can use. Not everyone speaks English.

I'm coming from an5.1.1 AOSP rom. I did a factory reset then installed the zip. Everyone installed perfectly but when I try to set up my device it says that this device has been reset and I need to verify my Google account. Once I do this it has me accept the Motorola terms and then it asks me to verify my Google account again. I cannot advance from this step. Any thoughts?

Why can't you advance past it? Is there some error?

I'm not sure. I think it's a 5.1 security feature.
Yes it is a 5.1 security feature. Is there an error, does it freeze, does it say or do anything? Or you don't know the password, or it is someone else's account?
I need more info to know what is wrong.

Yes, if you flash a GSM rom such as the xt1095, it will work for GSM companies. But you will lose CDMA which means it won't work with verizon's service.

I flashed over the latest version of CM and lost my CDMA LTE service. My phone still registers as a XT1096. Is there a way to get my service back? I'm with Verizon. Or do I have to flash back to stock, relock bootloader and start all over again?

As I said in the post you quoted you WILL lose cdma service if you flash it. There is no way to have a stock rom with marshmallow and cdma.

I'll second this, latest CM nightly has full CDMA LTE support. Been 3 or so hours haven't seen any major bugs, or anything that would be considered experience breaking.
Only thing your really lose is Moto Voice, as ambient display works basically the same as Active Display albeit without the slower co-processor.
i think the chop for flash light and twist for camera works better then moto tbh and no real bugs yet either but havent played with it much
I'd love a stock rooted 6.0 rom. I'm stock rooted on 5.1 right now and everything is working great. Anyone have an update on CM now that it's been a couple days? Has anyone attempted to add Xposed to 5.1?
I'm not that experienced with this but I have an xt1096 and unlocked the boot loader and flashed twrp on it then did a clean install of cyanpop 5.0(runs android 6.0.1 http://forum.xda-developers.com/showthread.php?t=3277786) it runs nicely in my opinion other than a couple bugs that need squashing
