Whenever I turn on the front camera it just turns black and then says "camera failed" or "unknown error occurred" then it proceeds to close itself.
Clear cache partition, camera force stop, cache clear, factory reset whatever all did but my front camera just don't work.

No i didn't and my phone is encased in a transparent bumper case no scratches or anything since I've got it less than 3 weeks ago. It seem to happened right after I update my phone recently about a week ago but I'm not sure.
Then it sounds like it's time for a warranty exchange.
At this point I'm still kinda pressed in beliving it's most probably a software issue because the flashlight icon is lit up (turns blue) in the notification bar when the flashlight isn't even turned on.
If the camera works before the update, it should be an software issue. Wait for an update, and check camera and the flashlight again. If it still don't work, then it's time for a warranty exchange.
This is becoming a widespread issue:

This may have been caused by the CRHA update that added AR Emoji and Super Slow-mo:

Hello,
I bought a Nexus one brand new from Amazon.
I accidentally dropped it onto a concrete floor.
After I dropped it, the camera stopped working(the led flash would flash at random times) and every time I called the screen would shut off and would never turn back on(had to do battery pull).
The led flashing at random times was getting annoying, so I covered the back with electrical tape. As soon as I did this, I noticed everything started to work again - including the camera. So I cut little slits in the electrical tape for camera flash and camera lens and this is how I have been using it for the past few weeks.
But today, I dropped it again and the same problem is back. No amount of electrical tape seems to fix it. I might try again and tape more strongly.
I can live with a broken camera and since I covered the flash with electrical tape, the random flashes won't bother me.
But the phone screen going off during a call and not turning back on is a problem that i have to fix. I simply cannot battery pull and restart the phone every time i make a call. Also during the phone call, when the screen turns off, I have tried: raising the volume button, pressing the power button, and touching the trackball - nothing works to bring it back on.
I tried various market apps for disabling the proximity sensor but nothing works
I am on 2.3.4 stock ROM but I can easily root. Is there any hope or do I have to buy a new phone?
look at my posts, i posted a way to disable the sensors.
anyway you have to send it to RMA. I did it with mine with the same problem.

there some scratches on it including a cracked camera lens.
i don't want them to send it back when its obvious that it was my fault.
you sure it would be covered in the 1 year warranty?
Just flash stock rom and recovery and send it. You can't loose anything.
If they refuse just tape flash led and disable sensors.

About a couple weeks ago I cracked the digitizer(touch screen) on my Lg Thrill and replaced it. Before doing this everything was fine with the phone...I upgraded to GB and it is also rooted but like I said everything was working just fine until I replaced the digitizer. Now every time I make or receive a phone call the phone goes black like it was close to my ear. This only happen if I answer the call and if I make a call it happens right away. The phone is still on and the calls go through but the phone goes black and is useless and I can't even hang it up until the other person hangs up. Not even pressing the power button on the top helps. I have checked and nothing is blocking the sensor on the front of the phone and the new digitizer I got from ebay but again it is a perfect fit and nothing is blocking the sensors...Any Ideas or suggestions? anything would be a great help thank you!

The sensor that the phone uses in order to register face proximity is just to the left of the forward facing camera. Ensure this is connected nice and solid (reseat the wire if necessary) and make sure there is no dust in it as this will likely cause it to behave as it has. Simply replacing the screen won't do this, but improper cleaning before replacing the screen could. You may have to tear the phone down again just to reseat the screen itself.

hmm i had the same issue BUT mine would do it when i cracked the digitizer, which is weird.. replacing my digitizer fixed the issue or it just cleared up itself, try resetting the phone.. Or like zombie said, Tear it down and clean the connectors and sensors

I have taken the phone completely apart and used a can of compressed air to clean it I double checked the sensor connector and made sure there was no dirt or dust in or around the connections. I put the phone back together and made a test call...still doing the same thing
But I did notice that when I made a second test in a dark room the phone did not go black and when I moved the phone close to a light the phone went black....needless to say I am so confused now more then ever. I did a third test and the same thing happened. So any other ideas as to what may be going on??? Seems like the sensor is working no? just in reverse??? Maybe the tint in the new digitizer is too dark? but this still makes no sense as to why the phone goes black when it gets exposed to light. Anyway any other help would be great right about now THANK YOU!
My Z3C started having a problem about 2 weeks ago.
It seems the drop down notification is being too sensitive or activating when it shouldnt.
Eg - on the home screen it seems ok
However, if you start a SMS or keyboard app or navigate Facebook, the nav bar keeps dropping (and obviously interfering with the screen)
Since then I have reset to stock and eliminated any applications.
What could possibly be happening and how to fix or turn off nav bar?

I have had the same issue today and I have tried everything possible to fix it. You know your desperate when you search the tenth page on Google for help like I did... I didn't find ****. I then decided to turn of my phone completely instead of pressing the restart option and it fixed my problem! I cant guaranty it will work for you but its worth a shot if you haven't tried already.
Check if the glove mode option is turned off in your settings, Glove-Mode makes the screen more sensitive so it might be causing your problem!
I've had this same issue. A couple of weeks later the top side of the screen stopped functioning completely. The touch screen was broken. I got it fixed for free under warranty, Sony did a great job, within 2 weeks!
Test touchscreen
Faced same problem. Found out that it was touchscreen issue and got it replaced under warranty.
Run Touchscreen service test from the service menu and check that the top part of touchscreen is detecting touch or not, in my case top left corner was not working.
Android 5.1
Build Su 4tl-44
Camera app version 5.0.21.2
Flash led on my recently bought Droid Turbo stopped working.
It was OK for 5 days. At one night I tried to switch on the flash light to use it as a torch. At first it turned on and blinked three times then went off.
Then I tried again to switch on but the right led just blinked once and turned off. Left led do not have any response.
I switched on the camera and take a picture. Led behaviour was same (right led just blinked once for a short time then switch off while left had no response).
Then I changed the 'Flash' settings in camera from Auto to On and again tried to take a shot but the Led behaviour remains the same.
Any help in this regards would be appreciated...
After googling i found a solution that suggested that draining the battery completely and fully charge after that will solve the problem.
However, i tried this method twice but it did not solve the problem.
Moreover, i tried some typical things like uninstalling the updated camera app to factory.
Factory resetting the phone and clearing the cache from recovery.
But none of the tricks solved the problem...
Now Fingers crossed....
Solved
I had the same problem. It was strictly mechanical.
I removed back panel and clear from dust and it solves it. I recommend to replace 2 sided tape too. MOTO XT1254

The tape actually MUST be replaced because as you said, the flash is a piece connected to the back casing of the phone. If it's not securely connected the insides of the phone, the points that touch the flash board won't be touching consistently and you'll wind up having to take it apart again. You've got to go the extra mile and buy GOOD 2 sided tape as well.. The cheap stuff turns loose as soon as the temperature gets to high or to low. To many people bring their phones in because their fix stopped working.
While pressing both flash LEDs firmly turn on the torch
It's supposed to be water resistant, but I've never dunked it intentionally under water. Bought this in 2017. Unfortunately, I had an accident and it fell inside a sink full of hot water (while I was taking care of a patient) and it stayed there for a minute or two.
What happened are the following:
Power button no longer works - It was a little wonky as it kept turning the screen on and popping up the camera. I tried to turn it off, but it kept turning back on.
Stock camera app - doesn't work anymore. whenever I try to activate the app, it just crashes.
Rear camera hardware - focusing is gone. Using another app (camera zoom fx) I can access it, but pics are blurry. It can't focus on images 12 feet away.
Front cam hardware - there was a mist that fogged the camera. but it seems to be working fine after.
Screen - the screen was relatively ok after the dunking. However, water got inside the S-pen port. I immediately removed it and started drying the phone for 48 hours. I tried to return the s-pen after, but it seemed to trigger something and the screen started flickering. it lasted for a day but it's now completely normal.
Aside from those, everything is fine with the phone. I managed an online backup to Samsung account and everything.
If you're still here, here are some of my questions. I appreciate all your help.
1. I've now turned it off for a complete drying. Now I can't turn it on. Is there a way to turn it on? Is a USB C jig available to do this?
2. I left some sensitive data in the phone *coughbootypicscough*, is there a way to access the memory and wipe it? or maybe factory reset the phone without turning it on?
3. Can the cameras be replaced / fixed?
Hoping for all our help.

The fact that water was hot made it easier to gain access to the inside of the phone and personally I'm going to say it's now a hardware issue and the phone is likely no good anymore.. If you're worried about certain pictures on the phone just destroy the phone so no one could possibly take parts out of it and try to use them. Smash it with a hammer, drive over it, you get the idea.. Just get another phone at this point the reason I know it's a hardware issue is because of what your camera did so the water made it past the seals mainly because it was hot and loosened the glue to get past it.

it actually still works. I tried hard not to turn it off because the power button is broken. Last night, I fell asleep and forgot to charge it and battery drained and phone turned off.
It still charges but I can't turn it on because the power button is broken.
I was hoping if anyone knows if a USBC jig can help and turn it on for a bit or any other way to trigger download mode and then restart.
Try to hold Volume down + Bixby button and plug the cable in, it may be goes in download mode amd than you can reboot you phone. If not try only with Volume down and cable.

I'll try that again. For now, I'm placing the phone inside a box with desiccant to ensure that it dries completely. Hopefully, it restores some function to the powerbutton.

Try opening and removing the simcard tray while the phone rest in the box with desiccant. Hopefully can remove water vapors better and faster.
