Try to enter bootloader holding vol down and power until you get into fastboot. Then flash the stock recovery, if you are on android 4.2.2 then download the rom from here http://sbf.droid-developers.org/ghost_rcica/list.php and then extract the recovery.img image, then put it on the same folder as your fastboot and type fastboot flash recovery recovery.img with cmd.
finaly you will restart you phone and will be able to install it.
To enter fastboot normally you hold the power button and down volume for 3 to 5 seconds.
When I had flashing problems I found that holding the power button and volume down for much longer - 20 to 30 seconds and then releasing both got me into fastboot.
i tried that already but it doesn´t work

you dont need the factory cable. That is just sometimes a problem, but other cables can work as well. Plug the phone into the wall outlet. Not to a computer. Let it try to charge for a while like that. Let it sit at least an hour or something then try to get into bootloader with the volume down and power button simultaneously thing. It may not charge from a computer appropriately.

This is what you need to do, if you're seeing the green light when connecting power then there is a good chance your battery is completely dead and it needs time to charge up.
green LED is lit only when I connect the power supply with OTG cable and after a minute it is gone.

so leave it for awhile and see if that thing charges. phones can be weird. id leave it on the charger for awhile. its really your only option at this point. keep trying different chargers if leaving it on the one you have doesn't work. I would try to charge from wall outlets only beacuse i have never had any luck with computer usb charging when it comes to the phone being totally trashed.
I have the same problem.

same here, trying to to update using Sony PC Companion and was told in the instructions, under step 5, to press and hold volume don key on the phone while plugging the USB cable into your phone, tried several times, even restarted phone, not happening.. HELP!1111111111
- Make sure the phone has charge.
- Remove SD card from phone.
- Make sure the correct divers are installed:
http://forum.xda-developers.com/showthread.php?t=2153261
Scroll down to: [ before we begin unlocking bootloader ]
Here 3 packages are given for fastboot drivers:
download the fastboot package with usb drivers (alternate drivers if the ones in package dont work OR this)
- Try another PC \ USB port \ USB calbe?
- Switch the phone OFF , hold the volume UP button while you plug-in the USB. (DO NOT PRESS POWER!) Blue led?
- Switch the phone OFF , hold the volume DOWN button while you plug-in the USB. (DO NOT PRESS POWER!) Green led?
Does any of these options work?
Kind regards, Stefan.
No change for me, works fine on another PC. But i want to be able to update that phone in my own home.

So use the flashtool then, and read the stickys too.
I tried flashtool at least 20 times. Simply can not start the installation system (vol. down button + USB cable at the same time does not work!). SO, I can not put phone in flashmode enymore. Flashtool see phone in this mode for a few seconds and then stuck. On the phone i have green led for a sec, then nothing...
So any method that requires phone in flashmode is unusable.
Before this i use it (flashtool) several time normaly.

First of all, sorry for my bad english .
After i've spend alot of hours with flash after flash, fail after fail i found an easy way to rescue our Optimus 4x from dead.
You know what i mean when you get stucked with S/W screen, and you can't flash the device in any way. < should work for : 3GQCT : wParam = 2010, lParam = 210 >
Edit 1/4/2015. New method:
< Make a cable like this or buy a USB-Y cable, and try to flash with it >

Why? Because Optimus 4x needs more power for flash. Also, try to play with battery if you are still stucked. How?
Start to flash without battery inside, wait 5-10 seconds after flash starts, put the battery.
If you don't have luck, combine with the old one:
So, what i've done?
1. I've disassemble the phone, till the motherboard.
2. I've disconnected all the connectors, and let only the USB connected.
3. I've hold battry manualy, while start KDZ flash, and BINGO!
See attachments.
4. Hold battery till the end of the flash, and you should get the LOVELY :
Before that, the flash stop at 17 seconds (10%) and i get Error 207 or 210.
okay. i'll try this now.. as a last resort..
confused
take a look at the picture..
Mine phone was start automatically on S/W.
You can let the board and try with it connected. Should work.
well flashed with ics
thank you dude :good:
but after i tried to flash v20c latest, it wouldn't flash.. stops at 17 seconds.. error code(207)...
LOG attached..
After a lot of tries finally managed to flash latest v20c(germany). But sadly, that didn't fix anything..

I suspect the problem comes from the sim holder or antenna(the white cable) , if you still have you phone opens then try to disconnect only one of them and flash normally.
I did have same problem with the signal and magically it's gone... but my wifi lost it MAC.... in that case I just run a script to fake one and it works
anw, I also have the 10% stuck, I used the LGFlashTool2014 (somewhere in the forum, sorry) and did a clean flash V20A with CDMA setting (???) and it passed the 10% error
FYI: link for the tool

Another thing to try
Booting from AP fastboot to charging mode
If the Device will only boot to AP Fastboot and it says Battery Low
Connect the phone/Device ato a Wall Charger (not PC)
-In AP Fastboot Mode hold Volume down& power buttons at the same time for +10 seconds
- When screen goes black...release power button
-Then IMMEDIATELY press and hold Vol - and Vol+ to boot to Boot Menu.
- In Boot Menu scroll down with volume down to BP Tools, press volume up to select
- Device should boot to Charging Mode with battery icon
- Once battery is charged, try flashing in AP Fastboot again
This will only work if battery charging Firmware is still preset and not corrupted
If Motorola Device will not boot to Charging Mode, a Fastboot cable will be needed.

this will happen even if drivers are not correct, if not

The bad port is on the phone, yes?
You cannot hold cable during Flashing, is way to sensitive for that.
If you have to use tape or a rubber band, to hold it in correct place.
Even with a good port touching the phone or cable can cause Flashing to fail.

Ah, I thought you were talking about the computer end. My mistake.
What's the failure message? Anything specific, or just "flash failed"? Have you tried a different cable?
So last night I created a flashable zip from the UPDATE.APP with the Huawei update extractor tool. I flashed it with TWRP it flashed everything but at the end it said it was unable to mount /system. I rebooted but nothing happened. Now when I try to power on the phone the notification led blinks one time red two times green and nothing appears on screen. I can't enter recovery, fastboot or anything else.
Anyone have any idea what can i do?
Looks Bad ?

Works ADB?
Maybe u have already tried but can you manually start fastboot mode? Power button + Volume down + usb cable?
If it enters fastboot mode, flash stock recovery, BOOT.IMG, CUST.IMG, and SYSTEM.IMG.
I've never heard of a flashable zip that actually worked for P8 Lite
Does your pc detect the phone when you connect it to usb? You can try another usb port and look into device manager.
If it detects anything, then try
Code:
adb devices
to see if there's anything. If adb finds a device
Code:
adb reboot-bootloader
to get into fastboot mode and then you can flash boot.img, cust.img, recovery.img and system.img.
You can also try to boot into fastboot mode directly (it may work, even if you don't see anything on the screen) and try
Code:
fastboot devices
if it was detected. If it was, you can flash the img's.
If nothing of the above works, send the phone into warranty and act dumb ("it was off when i woke up and it doesn't turn on anymore even if i charge it for hours" for example, don't mention the bootloader, flashing stuff, twrp, etc.); the chances are that they will replace your motherboard or give you a new phone.
Good luck.
I will probably send the phone in warranty.
Hello,
I have the same problem, and i also want to send it back as a case of warranty, does it work for you? Did you get a new Phone, or did they fix it?
Wish you a nice day,
thanks for your answer!
I'm having the same problem...
Weird red LED patterns...reboots, doesnt turn off (consequence, always very low battery), only huawei logo for eternity....
Usb on or off on the phone with the VOL DOWN + POWER, doesnt go to fastboot...

Sugestions? Thank you!
As @dennyn said, try via pc. If your pc can recognize your phone, you can play with adb and start all over again.

In order for the phone to be recognized from the pc, the phone must be turned on. Try turning it on (with the notifications red or any color), connect it to pc and then use adb.
Try charging it before anything else, since you cannot tell how much energy is stored in the battery.
When you connect the phone to your pc, do you get any pop up message, that new hardware has been found?

Hey dude! I have the same issue from same mistake now, how did you enter fastboot? What buttons did you use, did you have your battery unplugged? I've been trying everything for hours now but no luck...
Cheers, hope you see this post!

Are you sure your usb cable is a data cable? Try another usb port and another cable. When the cable is connected to your computer and the phone booted in bootlaoder mode, can you see "fastboot usb" in red or does it says "fastboot" or "fastboot AC"?
What was the update number and please post the output of "fastboot getvar all" (remove your imei and serialno)

Yes USB data cable I tried it with another mobile phone and it works, it was an update to Android 5.1 bootloader after the connection has not written anything either usb or ac. Sorry for my English.

Your phone isn't in fastboot mode, press power when "fastboot is highlighted then look if you see "fastboot usb" or "fastboot AC"

Well that means both your computer and your cable are OK so probably the usb port on the phone is broken. You'll have to disassemble the phone to replace it...

Do you think that might be faulty usb? The phone is because when connected to a charger normal recharging.

The charger only use 2 of the 5 pins present in a usb port #1 (+5V) and #5 (Ground)
In order to have a working data connection you also need pins #2 and #3 (data- and data+)
So its possible to have a defective usb port and the phone still charging. That might be something else but the usb port is the cheapest part to replace so I would try to replace this part first.
