I've had my turbo since December. It would restart every now and then but now its getting ridiculous. I've wiped the cache many times and did an FDR. It hung right after it rebooted after the FDR. It seems to be getting worse. Any suggestions?

I'm sorry I can't be of much help man but I'd return it since you should still be under warranty. You've done all the things that should solve the problem so it seems the bug cannot be user fixed.
[Some Moto phones just seem to have the random reboot bug. My girlfriend's Droid Mini did the same thing... we just upgraded her to a Turbo.
I bought a new Turbo at the same time in March; it stopped charging three weeks later, just days after the exchange period expired.
The first "like new" replacement from VZWhad a spot where the nylon back wasn't properly attached and the rough edge was exposed. The second one, received days later, had the reboot bug. Even after a factory reset, it randomly rebooted.
The third replacement came directly from Moto. The Moto rep claimed it was provided as "like new" because it had been removed from the box by Moto techs, examined to make sure it worked properly, then reboxed without the charger and manuals. (He said the VZW replacements were display models or returns that bad been refurbished by the carrier.)
It seems to be free of functional and cosmetic defects. I've had the phone a few weeks and it's working fine.

A week or so ago i dropped my phone in my truck and broke the screen. Asurion sent me a new one the next day and it has been nothing but problems. The battery they sent with it wont last more than 2 or 3 hours with the phone sitting on the desk not being touched. I tried my old battery and it had the same results. I made a nandroid of my old phone and restored it on the new one and nothing worked correctly. I did a full system wipe/restore and the phone seemed to be working but it won't detect a network after a reboot unless i restart the phone again. Asurion is sending me another phone tomorrow so hopefully it will work better than this one. With the battery issues, i tried everything from the forums to calibrate it and turn off things to lenghten but nothing worked. I hope the next replacement is a better device. I love my D3 but i can't live with a phone that has no battery life.
Also, when i did the full system restore, my google account did not sync any of my old information. The market would give me an error (even after accepting the ToS) and none of my contact information (from google or verizon backup manager) made it to the new device.
UPDATE:
Asurion sent me a new D3. Hopefully this one won't suffer from the same problems as the last few.
I always thought a nandroid could only be put on the phone it came from. Even thought its the same model phone. As far as the other issues you are having sorry, it sounds like you got a lemon.

All Asurion phones have a one year warranty through Verizon IIRC. Call them up and ask.

Agree with MrObvious. I just got a replacement D3 from asurion and I couldn't be happier. I guess it is just luck of the draw with the one they send you.
I had a Droid 2 originally.
-1st replacement they gave me the backlight on the keyboard didn't work
-2nd replacement the audio jack crackled when i moved the headphones and the phone restarted itself any time I turned on the GPS.
-3rd replacement was a D3 (current phone) and it has GREAT battery life. (still 90% after a full day of work). Make them replace it for you, they likely will. Good luck man.
It was a lemon
The first one they sent was a lemon. The new one came with a battery that had a charge of 44%. I put it in when i got home, rooted and restored all of my stuff with titanium backup, used the phone for a few hours (light use, few phone calls, some market use), and turned the alarm on for this morning. When i got up this morning it had been roughly 15 hours since i put the battery in without charging and it was at 21%. I think this one will keep me happy for a while.

Just bought this D4 last week off CraigsList, nearly mint, had no issues. The first week the screen went dark but phone was still obviously on (flashing light and keyboard always lighting when pulled out), found how to force restart, and it was back to normal. A few days later (last Wed I believe), it did it again and now I can't get it back for the life of me. I only bought the thing for $100 anyway so not a HUGE deal but is this common with this device? I'm tempted to force wipe but at this point, not confident that will do anything..
In regards to it being common, seems I can find a TON of **** online, but not necessarily here on XDA for this issue..
I've got a Verizon N4. I have had it since *October 23rd*edit. I haven't had any issues with lag until two days ago. Yesterday it got to the point where I couldn't even turn on my screen or navigate anywhere. When I did a hard reset the phone would get stuck in the Verizon boot screen until I pulled the battery. The phone got catastrophic lag twice yesterday and locked in the boot screen twice. I took the phone into a Verizon store and they swapped it out for a new one no questions asked even though it was past the grace period. I was worried since it wasn't showing any issues at the store but that wasn't an issue. Hopefully this new one won't have any issues. Just wanted to give you guys a heads up in case there are more similar issues.

So long story short, the lens jumps up and back down while holding the phone trying to take a picture. It stops however when the phone is stabilized on a table.
I guess my question is, is there anyone else who is experiencing this? Do I have a defective device, does the OIS think I'm moving much more than I really am?

Have already seen this issue. Factory reset seems to solve it.
It was doing it before I unlocked and rooted, which did a factory reset no? Also different camera apps reproduce the same issue.. I can't seem to find another thread with the same issue, can someone post a link? I'm pretty concerned and am thinking about getting a replacement...

My nexus 9 did this. I could hear It move after taking a picture or while panning around. It was rather annoying which is another reason why I returned it.
I've just had this exact same issue. I got my phone on the 8th of Dec and it started for no reason on the 25th. I tried a few full wipes, and also tested it on a couple of alternative camera apps with no joy so it's been taken back to store today.
I called google and just got an RMA to send it back once I receive the new one.. They sent me a link that took me directly to the check out portion of play devices.. Play store shows them out of stock however my order was successful.. Now let's see how long the new one takes to arrive.
Edit: ordered it today (26th), email confirmation said it should leave the warehouse by the 30th. :thumbup: ups standard shipping 2-5 days.

I'm seeing the same issue you're seeing. Motorola customer reps sucks too. I'm trying to do the advance exchange program but my card got rejected 3 times from motorola's system. This also happened to me when purchasing the damnnn phone. Hopefully 4th time is a charm if not, motorola better figure this **** out. They even asked me to use a friend or family member's credit card. WTF is that!!! First time it failed because my CC billing and shipping was different. So I called my CC to change my billing addy to my work addy. Tried again and still got rejected.

Ok so i have had my N6 since December but only used the video yesterday. It suffers the same problems as above that I will not focus on a object and keeps going in and out of focus on its own.
My dilemma, I bought it full retail from T-mobile, did not activate it on T-Mobile, instead I dropped in my Verizon SIM and have been using it on Verizon ever since. So I notice the camera problem yesterday and I speak to T-mobile today. In a nutshell they are refusing to replace it through a warranty exchange (new or reconditioned) because I never activated it on T-Mobile and I don't have an account with them.
T-Mobile sent me to Google (30 minutes of my life that I will never get back) who tried to tell me it was a software issue. When I told them I'm sure it was a hardware issue they sent me back to T-Mobile despite telling them that they sent me here in the first place. Round two with T-Mobile and they again refuse to warranty the phone and give me the number to Motorola (which actually makes more sense).
So the question is now, how is the Motorola warranty process and will I have to play "20 questions" like with Google? Will I have to send them the N6 first or can I hold onto it while they ship me another one? How long will the process take?
Another option I thought of, I could activate the N6 on T-Mobile with a pay as you go plan and then have them warranty it since now I do have an account with them. I still have the T-Mobile SIM that came with N6 so I don't think it would be that difficult.
Or should I just raise a stink with T-Mobile and demand they warranty the N6 regardless of my account status.

I did the exact thing you did and I called Motorola. They sent me a new phone in a factory sealed box (no refurb.) Just call them, tell them your issue and you'll be taken care of.
it's hardware issue, send it back to Motorola for a new replacement, or repair by yourself
Ok so I activated the N6 on a pay as you go plan which cost me a total of $13.00. Well turns out that T-mobile would have to send in the phone first before a replacement is shipped on any pay as you go plan. If I had activated on a monthly plan then they would have sent the replacement first.
I ended up calling Motorola and after a lengthy hold time my replacement N6 is enroute. Hopefully this will end my camera problems and not start any new ones.
I have been rooted and using custom ROMs for over a year now on my note 4 but recently started having my TekXodus ROM crash occasionally. So I updated to the factory latest DOK2 using Kies and I have an issue where when I reboot or shutdown. When i do this I have to pull the battery out for a few minutes before it can boot again. I have tried factory restoring with Kies several times and still have the same problem. Root + custom ROMs also have the issue. Any ideas?
Similar problems in another thread. I'm still having issues although I've gone back to stock as well.

People are starting to pop up everywhere with this issue, ROM's, stock, rooted, it doesn't seem to matter. It seems like it's either a software update they pushed to all carriers or maybe hardware failure of some kind. The batteries do seem to go bad fairly regularly around 9-12 months. That's fixable with that Anker 2x + wall charger kit, but if it's hardware failure for this it could get ugly.
Yeah mines getting worse. Gonna try to pick up a motherboard.

My phone died completely after a week of ongoing problems. Won't boot or anything any longer and I don't think its software related.. Let me know if you have any success picking up and installing a motherboard.
My Note 4 I bought a year ago was having this issue a couple months ago. Did the usual, factory reset, blah blah blah, was getting the mmc read fail error. Called tech support told them everything I did because I knew they would want me to do it so I headed them off at the pass. They said well it seems you did all the work for us and I had a replacement the next day for $5. Worked fine for 2 months and now it's doing it again so I'll be calling for another replacement tomorrow.
Strange thing is I wasn't getting the same mmc read fail error although I saw something similar only 1 time. This time though I did get a dm-verity verification (?) error. No idea what that even means.
What sucks is they really should offer me an upgrade at this rate but I have no interest in the Note 5 or the S6 without an sd slot or removable battery. Hoped to hold on to my N4 for at least another year.
where can we even buy motherboards for this note 4?

Yep, my time w/ the note4 was good while it lasted. I purchased it for $350 back in october and was great. sold it "as is for parts" on ebay for $160 yesterday, and am done with samsung for a while. took that $160 and purchased a LG G2 that i'll have to use for another year or more. i was really getting used to the note's size and stylus features as well.

Good for you man! i really am gonna miss it too! i'll probably sell it for parts while i still can. i got a new nexus 6p that i really am enjoying as well!
I ended up getting a motherboard and replacing it. Worked out fine except I used a cheap lens cover replacement while I was at it and no my pictures suck. Not even sure if the actual is available anywhere.
